Monitor health status to identify and solve community health problems. Diagnose and investigate health problems and health hazards in the
community. Inform, educate, and empower people about health issues. Mobilize community partnerships and action to identify and solve health
problems. Develop policies and plans that support individual and community health
efforts. Enforce laws and regulations that protect health and ensure safety. Link people to needed personal health services and assure the provision of
health care when otherwise unavailable. Assure competent public and personal health care workforce. Evaluate effectiveness, accessibility, and quality of personal and

Countryside PH is one agency serving five counties. Staff travel to all counties to work in their designated area.
Milan/Appleton have a large Micronesian population. Other cultures in our counties are Hispanic, Somali, and Upper Sioux.
Mass Dispensing Plan is for one large POD and EMs will bus county residents to POD.

Emergency Preparedness Highlights Student Vaccination Clinic: August 2014 It provided free
vaccinations to teenagers needed for school and CPH practiced our Mass Dispensing Plan
Sheltering Training and Planning: attended regional EM training on sheltering and working with the five Emergency Managers on plans
Family Service Staff EP 101 Training: emergency preparedness training provided to each county Family Service staff
MRC Unit Management: conducted a call-up of all MRC unit volunteers and send out periodic newsletters. Will hold an MRC Training in October.

Emergency Preparedness Highlight◦ Development of a Mass Care Planning Team as a result of Region 5 Mass Care
Workshop held January of 2015.◦ Des Moines Valley Human services has taken the lead in the development of
Cottonwood and Jackson Counties Mass Care plan. County Emergency Managers, Faith based coalition, Red Cross, and EP Coordinator are the other team members responsible for developing: Shelter Sites, Pet Shelters, Memorandums of Understanding, as well as, additional training and education.

KANDIYOHI/RENVILLE PUBLIC HEALTH
Emergency Preparedness Highlights:◦ Avian Influenza: the CHB is engaged with partners dealing with
the AI outbreak. Kandiyohi County assisted with developing & updating mental health resources and disseminating to partners. Renville County supported employee safety related to communicable disease issue.
◦ Mass Care: the CHB is working on Mass Care Plan with EM: Developing & up-dating of Sheltering & Family Assistance Center plans in collaboration with Human Services in each county.
◦ Mass Dispensing: the CHB had an opportunity to offer FREE vaccine in 2014 at school located clinics. Renville County will offer this again in 2015. This allows us to exercise our mass dispensing plans and practice with partners.
◦ MRC: Each county manages the MRC unit of volunteers to be ready in the event of a public health emergency.

Special Ebola funding from MDH Funds available through June 30, 2016
Finalize Isolation & Quarantine plans, develop a list of Essential Services.
Organize meetings with leaders from West African population in Nobles County to conduct recovery needs assessment specific to Ebola.

Develop Ebola Community Recovery Plan to address psychosocial recovery needs.
Offer cultural training for health care providers.
Meet with EMS agencies in Nobles County to complete an assessment of the EMS Ebola plans.
