Complete Heart Block - First Manifestation of Congenitally Corrected Transposition of the Great Vessels

IntroductionCongenitally corrected transposition of the great vessels (CC-TGA) is a rare congenital heart disease (less than 5% of the congenital cardiac pathology) characterized by the association of atrioventricular discordance and ventriculoarterial discordance.

Case presentationThe 45 year-old woman, with no known cardiovascular pathology, mother of four (on-term complication free natural births), three days prior to the hospitalization, started feeling dizziness, palpitations and fatigue. ECG revealed complete heart block. Transthoracic echocardiography revealed congenitally corrected transposition of the great vessels.

An MRI-conditional dual-chamber pacemaker was implanted.

ConclusionSeemingly typical cases of medical emergencies (such as a complete heart block) may unravel extremely rare pathologies. The case highlighted above may foster discussions on the complications and the evolution of congenitally corrected transposition of the great vessels.

Case ParticularityThe main particular aspect of the case is the rarity of the pathology (less than 1000 cases reported in the literature). The long symptom-free period (45 years, including 4 births) is also a rather rare occurrence.
