Key accomplishments - Prevention & Detection
• Two pilots completed (Montague Pilot Group Lifestyle Balance
Program and TOOLS for Wellness program Harbourside).
Committee examining recommendations for a primary care model
for PEI targeting pre-diabetes and obesity
• Nutristep screening tool launched within Public Health Offices for
toddlers and preschoolers
• CANRISK tool (diabetes risk calculator) promoted as part of
Diabetes Awareness month in November 2016. Posted on Health
PEI website & distributed to Primary Health Networks
• Continue to examine opportunities to work with Dept of Health &
Wellness on risk reduction strategies

• Provincial Hypoglycemia Medical Directive
• Use of insulin pens in all acute care sites
• Changes to documentation and ordering in CIS, creation of an glance “diabetes dashboard”
• Patient education materials and Nursing Education tools
• Discharge planning processes to improve communication and referral process
Long Term Care
• Provincial Policy for Management of Hypoglycemia and Hyperglycemia (July, 2016)
• Pilot on the use of Insulin Order set for LTC frail elderly, Beach Grove (May, 2017)
• Continued education to community care and LTC staff through workshops
• Diabetes modules for community care / LTC staff to provide education, under development

• C-Change workshops in 2015 and 2016
• Collaborative physician office visits with diabetes educators, continues to expand

• Basal insulins (levemir, glargine) under Special Authorization
DPP-4 inhibitors (sitagliption, saxaliptin, lingliptin) under Special Authorization
• SGLT-2 inhibitors (canagliflozin, dapagliflozin and empaglifozin) under Special Authorization
• Diabetes Medication and Supplies Recommendation committee – newly formed

• Provincial Diabetic Foot Ulcer Screening tool – in primary care, with plans to expand
