are animal pathogens or
commensals.
• Aerobic and facultative
anaerobic.
• 1μm in diameter.

• Is the most virulent of all the S.
• Coagulase-positive.
• Found mainly on the skin and
mocous membranes of animals.
• Normal microbial flora.
• Produce toxins and virulent
enzymes.
Cause: Foliculitis, osteomielitis,
necrotizing pneumonia, endocar-
ditis, meningitis and others.

GENERAL OBJECTIVE
To describe the clinical and molecular
epidemiology of current invasive infections
caused by CA-MRSA in adolescent and
adult patients in Argentina.
MATERIALES & MÉTODOS
DISEÑO DEL ESTUDIO:
• Estudio de tipo prospectivo, multicéntrico,
observacional.
• Diseñado para evaluar las características
clínicas y moleculares de las infecciones
invasivas por CA-MRSA.
• Argentina, marzo de 2010 - diciembre de
2011.
• 11 hospitales participantes, situados en
la región central del país.
MATERIALES & MÉTODOS
SELECCIÓN DE PACIENTES:
CRITERIOS DE INCLUSIÓN
Pacientes ≥ 14 años con infección masiva de MRSA
diagnosticada al ser admitido en el hospital o en 48 horas.
CRITERIOS DE EXCLUSIÓN
Si cumplían con alguno de los siguientes ítems
en los últimos 12 meses:
Hospitalización.
Diálisis.
Residencia en centro de atención durante
mucho tiempo.
MATERIALES & MÉTODOS
RECOLECCIÓN DE DATOS: Historias clínicas
• Variables socio-económicas.
• Comorbilidades.
• Uso de antibiótico previo.
• Presentación clínica.
• Resultados principales de laboratorio al inicio del estudio.
• La fuente de aislamiento (Ej: sangre, líquido articular, pleura).
Los datos de seguimiento se pretendieron obtener 90 días
después de finalizar el tratamiento.
MATERIALES & MÉTODOS
ESTUDIOS MICROBIOLÓGICOS:
• Identificación de S. aureus = procedimientos
de diagnóstico convencionales.
• Almacenamiento a -20°C.
• Llevados a un laboratorio de la U. de Buenos
Aires.
• Pruebas de sensibilidad a los antimicrobianos
= Métodos de difusión de acuerdo con las
directrices del Instituto de Normas Clínicas y
de Laboratorio. (CLSI, 2009).

McFarland: 1. 4-5 colonias de 16-24 h de cto en
una placa de agar.
2. Se suspenden en caldo en o
suero fisiológico al 0,85%.
3. Se compara la turbidez de la
suspensión con el estándar 0,5 de
McFarland.
Usado en la preparación del inóculo
para la prueba de sensibilidad a
antimicro- bianos o antibiogramas.
Inóculo suspendido VS.
Ácido sulfúrico 1% +
cloruro de bario 1,175%

AMPLIFICACIÓN POR PCR:
De una región de ADN que
corresponden a un gen o parte de él.
Las porciones molde de ADN
son flanqueadas por dos
oligonucleótidos que inician la
reacción de polimerización.
Con la DNA polimerasa, Mg como
cofactor, un buffer y dNTP.

Electroforesis en gel de
campo pulsado (PFGE).
Endonucleasas de restricción
de ADN infrecuentes. (SmaI)
Fragmentos de gran tamaño.

SUSCEPTIBILIDAD ANTIMICROBIANA:
• Las pruebas de sensibilidad antibiótica
revelaron resistencia a la oxacilina (100%),
la eritromicina (9%), clindamicina (6%),
gentamicina (11%), y quinolonas (4%)
RESULTADOS
SUSCEPTIBILIDAD ANTIMICROBIANA:
• Todos los aislamientos de MRSA fueron
sensibles a vancomicina, teicoplanina,
linezolid, rifampicina, doxiciclina y trimetoprim /
sulfametoxazol.
Ninguna de las cepas fue multirresistente.

• MRSA fue aislado del
70% de los pacientes
y la clona ST30 fue
predominante en el
68%.

En los últimos 6 años la clona que predominó en Argentina fue: CAA clone: ST5-
SCCmec IV-spa t311. Fue reemplazada por la clona ST30- SCCmec IVc-spa t019.

PERSONAL CONCLUSIONS
The main factor which has allowed to the massive
proliferation of this bacterial strain is the antibiotics
indiscriminate use. This explains the resistance to
oxacillin, erythromycin, clindamycin, gentamicin and
quinolones, presented by this bacterial strain.
Knowing the clinical and molecular features of bacterial
infections is very important to the development of a
health plan and an appropriate treatment, because
these microorganisms are constantly mutating and
becoming resistant to different antibiotics.
PERSONAL CONCLUSIONS
The knowledge of the genetic variability involved in the
virulence and antibiotic resistance determined by PCR
and PFGE methods can provide molecular targets for
antibiotic treatment.
By means of this research it was possible to verify that
the new clones of CA-MRSA have a better capacity to
expand, because they possess more virulence factors
and are better able to survive on the skin of human
beings, which gives them an advantage over existing
clones.
