– CTV haut risque (66-68Gy)= GTV (T+/-N) + 10mm– CTV risque intermédiaire (60Gy) = CTV haut risque + 2mm + niveau(x) ganglionnaire(s) envahi(s)– CTV bas risque (50Gy) = CTV risque intermédiaire + 2mm + niveau(x) ganglionnaire(s) à risque d’envahissement > 10% : niveau I pour les
cancers de la cavité orale, niveaux II-III pour toutes les localisations, niveau IV pour les localisations hypopharyngées (ou en cas d’atteinte du niveau III des localisations laryngées ou oropharyngées), niveau VI en cas d’atteinte sous-glottique, rétropharyngée pour les envahissements de la paroi pharyngée postérieure. Irradiation cervicale prophylactique bilatérale à l’exception des tumeurs oropharyngées ou de la cavité orale bien latéralisées.

La lente agonie de l’induction (GORTEC 2007-02) « Combo » RT-CT-CETUX en alternative au standard RT-CDDP
(GORTEC 2007-01) Le variant KRAS pour sélectionner les patients RT-CETUX ? La déflation thérapeutique :
• Encore HPV !• Volumes prophylactiques • Doses prophylactiques
Et pour prévenir la mucite : doxepin, DLA ? La réirradiation doit être moderne mais résistera-t-elle à l’arrivée
bruyante de l’immunothérapie ?
