Enrolment and getting startedOnce you are enrolled we will forward you the required resources for you to commence submission of your histology
samples – contributing to your CPD program requirements. Your Clinical Labs representative may also have provided you
with the specific ‘Skin Excision Evaluation Program request form’ when you completed the program registration form.

Request form completionPlease note that coding must be entered on the reverse of the
form for program inclusion - clinical notes included on the front
of the form is not considered adequate. The request form is
structured to clearly separate your patients biopsies from patient
excisions. Biopsy specimens in which the goal is simply diagnosis
do not have margins recorded in the audit, but excisions do.

Program criteriaYou must submit 40 histology samples per 12 month cycle
(excision or biopsy) using the Skin Excision Evaluation Program
request form (pictured left) with all clinical coding provided on
the reverse. Completion of an assessment and evaluation activity
are also required for program completion.

Skin Excision Evaluation Reports4Reporting on successfully submitted skin excisions allows
participants to download the charts and tables to support
ongoing education. A feature of this program allows participants
to confidentially review their progress with peers, as well as state
and national benchmarks. To access these reports you will need
to login to: www.clinicallabs.com.au/harp (your login details will
be emailed to you).

comparison between the precision of your provisional
assessment in contrast to your peers and against all other
participants in your state and nationally.
The chart above helps you compare the percentage
between the complete and incomplete margins amongst
the excisions submitted based on type.

In this table, you can view a breakdown of the number of
samples you have provided to the program for the month.
Through the Melanoma Data table, you can compare
your number of patients diagnosed with invasive and
non-invasive melanomas against your peers and all other
participants in your state as well as nationwide.
The Provisional vs. Histological table allows you to assess the overall results of your provisional diagnosis once histology
has been completed for you to determine areas for improvement.
