AbstractThe purpose of this paper is to fulfill an assignment for the Nursing 440 course, Leadership in
Nursing, in the RN-to BSN program at Ferris State University. To complete this paper group
participation in an Organizational Strategic Plan project was necessary to provide a hypothetical
healthcare service, the Good Samaritans Clinic. After an external and internal environmental
assessment, the group chose to present an indigent clinic in St. Louis, Missouri. This paper will
detail the elements involving creation of a mission and philosophy statement, developing
organizational goals, objectives, job descriptions, job advertisement, organizational charts and a
strategic plan including a budget proposal. A Power Point presentation will present the Good
Samaritans Clinic to the other students for evaluation.

Step OneExternal Assessment
Good Samaritans Clinic will be a non-profit, non-denominational organization providing
high quality health care services for medically indigent adults and children in St. Louis,
Missouri. Like many other clinics in the surrounding area, this clinic offers basic medical
services along with very specialized services. According to the Centers for Disease Control and
Prevention, over 46 million Americans were without health insurance in 2009. Six point six
million of those Americans were children under the age of eighteen (Recent data on health
insurance, 2009, para. 1).
St. Louis has a population of 998,692 people, of which 23% of that is children under the
age of 18. St. Louis’ population consists of 71% white and 23.4 % black. The median household
income is $57,561 and 6.7% of families with related children under 18 have an income level that
is below the national poverty level. Approximately 7.1% of St. Louis’ population are
unemployed, there are 1,046 people in St. Louis that have graduated with a high school diploma
and 21,291 people that have a less than a 9th grade education (State & county quick facts, 2012).
According to the Centers for Disease Control and Prevention, 12.4 % of people in
Missouri delayed medical care due to cost, resulting in 9.4 % not getting prescription drugs and
14.1% not getting dental care (Health status and determinants, 2011, Table 81). The Good
Samaritans Clinic can help bridge this gap of delayed medical treatment by providing specialty
health care services for the people of St. Louis, Missouri. There are many clinics available in St.
Louis to help with the needs of the medically indigent or underinsured, but with St. Louis being
so heavily populated we needed to offer an additional clinic that could support all aspects of
health care. The Good Samaritans Clinic offers general medical care with sub-specialty services

that include prenatal care, breast feeding support, child birth classes and also pediatric services.
Same day sick appointments will be available for acute illnesses as the provisions for walk-in
appointments.
Internal Assessment
In development of the Good Samaritans Clinic, it is necessary to perform an internal
environmental assessment. Our development group determined through our external assessment
that there was a definite need for the proposed type of services. It was felt to secure the success
of this endeavor financing would be met through grants, private organizations, fundraising
efforts, donations, and services from local businesses. For this clinic to serve the indigent
population it will be necessary to be located in a heavily populated area, such as the chosen city
of St. Louis, MO. The Good Samaritans Clinic would provide its services as an outpatient
department of the hospital, thereby benefiting the hospital, the residency program and the
patients. The main goal of its location would be to be in close proximity of available services
like lab and x-ray to better serve the needs of the patients.
The clinic would have thirty exam rooms of which Internal Medicine would occupy
fifteen of them providing five ½ morning clinics, four urgent care ½ day afternoon clinics. Two-
faculty member would supervise three years of residents and each year having eight residents.
Pediatrics would occupy six exam rooms and have clinic five full days a week. There is would be
a full time Pediatrician, one full time Nurse Practitioner, and 1 resident. OB/GYN would occupy
eight exam rooms and provide five ½ day clinics, one– ½ day colposcopy clinic, one ½ day
afternoon Pap clinic. and one ½ day high risk OB clinic. One faculty member would staff the
OB clinic; supervising three years of residents with each year have eight residents. The
remaining exam room being divided into an eye exam room and the waiting room for the eye

room. In the afternoons, each area of OBGYN and Internal Medicine would have a specialty
clinic that volunteer physicians would come to offer their services to our patients only. It would
be necessary to staff the clinic with seven full time equivalents (FTE) RNs, six full-time
Medical Assistants, four full-time billers, seven full-time front desk and phone staff, three full
time Social Workers, one FTE Dietician, two full-time financial counselors, three full-time
interpreters, one full-time program coordinator, one full-time business supervisor, and 1 full-time
Nurse Manager.
The human resources department of this clinic would facilitate all employee relations,
including recruitment, education, and evaluation of staff; this department would also serve as the
information systems manager. The Missions Team and residency program director would
manage the research and development. It would be geared from the review of public demand
that would then be supported by the hospital and the clinic as deemed necessary. The
management team would facilitate and be responsible for all standards, policies, and guidelines
to maintain accreditation standards.
Step Two
Mission Statement
The Good Samaritans Clinic is a nonprofit organization that provides access to health
services in St. Louis, Missouri. Our services will include primary and specialty care including
dental, vision, mental health and outpatient services to persons in need.

Philosophy
The philosophy of the Good Samaritans clinic is to integrate education and patient
centered care by cherishing each patient, seeking out and putting the needs of our patients and
their families first, and allowing access to excellent health care services to the underprivileged,
underinsured, and uninsured. We promise to provide quality, comprehensive, culturally sensitive,
professional medical care.
Step Four
Organizational Goals
Yoder-Wise (2011) states, “Goal-setting is the process of developing, negotiating, and
formalizing the targets or objectives of an organization” (p. 313). Goals must be specific and
have a deadline, show goal commitment by giving the employees instruction and support,
provide feedback and monitor performance (Yoder-Wise, 2011). The Good Samaritans Clinic
(GSC) goals are as follows:
Good Samaritan’s Clinic will deliver high standards of excellence in all programs and
services.
Good Samaritan’s Clinic will provide high quality healthcare services for
underprivileged, underinsured, and uninsured adults and children in St. Louis, Missouri.
Good Samaritans Clinic vows to value our patients, to understand patients’ experiences
from their perspective, and to provide an environment in which healing can occur.
Good Samaritans Clinic plans to support our staff, residents, and community, our greatest
resource for achieving excellence, to strive for excellence and continual improvement in
all that we do.

Good Samaritan Clinic will integrate quality education and patient centered care by
providing preventative, curative, supportive and palliative care to our patients and their
families.
Step Five
Organizational Objectives
According to Yoder-Wise (2011), objectives need to be specific, measurable, agreed on,
realistic, and provide a timeline to achieve the activity or improvement stated. Good Samaritans
Clinic has the following objectives:
To integrate quality patient and staff education to identify and reduce risk factors for
disease prevention, providing resources throughout the community to improve health and
wellness within the first year after opening GSC.
To support and increase community awareness about healthcare resources and needs to
improve quality of life immediately after opening GSC.
To serve as the primary care access center for healthcare needs in place of the emergency
department to decrease costs of healthcare and decreasing emergency room visits.
To create a positive work environment by displaying compassion to enhance patients
healthcare experience.
To implement current research and residency education to enhance the quality of
healthcare delivered.
To strive to maximize financial resources to enhance clinic operations by securing grants,
donations and charitable contributions to keep clinic open.

To increase customer satisfaction by providing multiple specialties within the clinic to
eliminate the need for going to several offices as evidenced by the positive patient
comments.
To respect others and support spiritual, ethnic and cultural diversities through our actions
and behaviors and provide ongoing staff training to maintain competence in these areas.
Step Six
Organizational Chart
With flat organizational structures, decentralized decision-making replaces the
centralized decision making typical of functional structures. Flat structures allow the delegation
of decision making to the professionals doing the work (Yoder-Wise, 2011, p. 147). Personnel
policies and programs, such as shared governance, the positive image of nursing, and
professional development within the structure of a flat structure provides an innovative
environment where strong practice prevails. Shared governance goes beyond participatory
management through the creation of organizational structures that facilitate nursing staff more
autonomy to govern their practice (Yoder-Wise, 2011, p. 150).
According to Yoder-Wise (2011), flat organizational structures are less formalized than
hierarchical organizations (p. 149). Good Samaritans Clinic recognizes decisions are being made
at unit level with a focus to improve patient safety. Shared governance vests the necessary levels
of authority and accountability for all aspects of the nursing practice in the nurses responsible for
the delivery of care (Yoder-Wise, 2011, p. 151). As shown in Table 1, the organizational chart
for Good Samaritans Clinic uses the flat organizational structure starting with the Chief
Operating Officer of Physicians Organization groups, moving down to the Vice President of
Adult Medicine and then the Nurse Manager of Good Samaritans Clinic. The Nurse Manager is

then responsible for the following branches in the organizational chart: Clinical Nurses,
Dieticians, Medical Assistants, Interpreters, Social Workers and Resident Coordinator, along
with being responsible for the Business Supervisor, who oversees the Medical Receptionists and
Financial Coordinator/Billers.
Step Seven
Job Descriptions
According to Yoder-Wise (2011), the position description provides written guidelines
detailing the roles and responsibilities of a specific position within the organizational context.
The job description reflects functions and obligations of a specific work position. It should
reflect current practice guidelines for individuals and may have competency-based requirements.
Clear and concise job descriptions are important because they provide the basis for roles within
an organization (p. 295).
A job description will inform the employee what the worker is responsible for
performing, how it is performed, and how it relates to other positions within and outside the unit.
The job description should accurately and completely describe the permanent duties and
responsibilities that are assigned and performed and establish performance standards and goals.
A review of a job description may be a good way to start a performance appraisal. See Appendix
A-J for a full job description of the positions held at Good Samaritans Clinic. Each job
description has a disclaimer that these job descriptions are not intended to be all-inclusive and
employees may perform other related duties as negotiated to meet the needs of the organizations.

Step Eight
Job Advertisement
The Good Samaritans Clinic will advertise its open positions with postings through the
affiliated hospital. Circulation of advertisements to the outlying community through local
newspapers and professional nursing journals are also necessary. The position this group created
a job advertisement for is the Clinical Nurse (See Appendix J). This colorful advertisement will
catch the attention of interested Clinical Nurses who desire to provide quality medical care for
the indigent adults and children in the St. Louis, Missouri community. Pricing received from a
large local newspaper (Appendix K) and a professional nursing journal, The American Journal of
Nursing, (Appendix L) provides a proposed financial investment for advertising open positions.
Further plans include collaborating with local colleges and universities in an attempt to recruit
advanced practice nursing students and taking part in a job fair to advertise for all positions. A
community job fair in coordination with the local hospital would also be utilized to recruit
various disciplines to the clinic.

Budget Proposal
As noted in Yoder-Wise (2011), a budget is a detailed formal financial plan for carrying
out the activities an organization wants to accomplish for a certain period stated in terms of
dollars and includes proposed income and expenditures (p. 239). At the beginning of each fiscal
year, an organization finalizes their operating budget for the year going forward. It encompasses
the projected units of service that are expected and the revenue anticipated. Deducted from the
total operating revenue are the operating expenses, which consist of total salaries and fringe
benefits, total miscellaneous fees, total drugs, total medical supplies, and total other patient
supplies. The remaining deductions encompass total other supplies, like housekeeping supplies
and office supplies; total services, like courier services; and total overhead which includes things
like postage, phones and pagers, and community charitable expenses. In order to come up with
net income you also need to include deductions like building improvements, which all add up to
total expenses subtracted from total operating revenues
To operate the Good Samaritans Clinic and meet the Joint Commission and the
requirements of the Accreditation Council for Graduate Medical Education (ACGME) it will
take an average of 39.5 FTEs. The total salaries and contract wages will equal $1,955,251.00 for
the year; if you include fringe benefits, it will total $1,982,148.00. The staff mix will consist of
registered nurses, medical assistances, interpreters, dieticians, social workers, billers, triage
nurses, medical receptionists, business supervisor, nurse practitioner, pediatrician, and a nurse
manager. Because the Good Samaritans Clinic hours of operation are 8:00 a.m. to 4:30 p.m.
Monday through Friday, there is no need for any staff to be on call other than the pediatrician
who falls in a rotation with the pediatricians in the hospital.

The Good Samaritans Clinic’s operating revenue will come from Medicaid, Medicare,
self-pay patients generated from the care given by the residents both in the clinic and in the
hospital. Revenue will also come from the nurse practitioner, the pediatrician, support from the
hospitalists, and from private donations. Private donations come from local corporations that
support the mission of the clinic, from hospital fundraisers, and from private benefactors.
Total expenses for the Good Samaritans Clinic total $2,681,271.00. What is
encompasses this total includes medical supplies, patient supplies, other supplies like
laundry/housekeeping and printing. The total also includes total services like physician
purchased services (hospitalists), overhead expenses that include phones/pagers, mileage,
postage and community charitable expenses (outside interpreter services), and building
improvements.
After totaling the expenses then consideration for the total depreciation and amortization
will be necessary to get the total expenses for the clinic. With careful monitoring of the budget,
the expenses necessary to support the clinic will be less than the revenue earned equaling a
positive net income. The 2013 proposed operating budget is included in Appendix M.
Conclusion
The Organizational Strategic Plan (OSP) group created the Good Samaritan Clinic to be
located in St. Louis, Missouri after identifying the need for an indigent clinic in the community.
By forming this OSP, it has helped each of us recognize the workings of an organization. This
project investigated what it could really mean to be responsible for running the entire aspects of a
clinic. It has also allowed us to work together as a team and combine our ideas to run the clinic
into a fictitious situation that could actually be real to us in the future.

Group:I can clearly recognize the hard work put into this particular assignment. Excellent job! In reviewing the philosophy, consider stepping back for a moment then read again. Sometimes we can write something two different ways and mean the same thing. I say this because the rule of thumb (no particular author) when writing objectives is not to go pass 5 objectives (same for the goals...which by the way looks great!). Why? Generally, when more than 5 goals/objectives are written, it is easy to lose the stakeholder; makes achieving more difficult; and most importantly, the goals/objectives become less hardwired. In the business world, the less the better...no need to change anything on your project...just food for thought for the real world service lines.
