How Do I Create a Claim

Claims are filed and settled directly with the Transportation Service Provider (TSP) in DPS

Once submitted claim status changes to ldquoSubmittedrdquo TSP receives email notifying them a claim has been

filed TSP update member via email regarding status of

claim Member responsible for checking status in DPS TSP has 60 days to settle claim Member can transfer to MCO after 30 days

Highlight the correct shipment by clicking on it and then click the lsquoPickrdquo button

The page will refresh click the checkmark next to the ldquoBOLGBL Numberrdquo field Select shipment from the display box by clicking the lsquo gt rsquo symbol and click ldquoPickrdquo

How Do I Create a Claim

The page will refresh again and Claimant and Shipment information will be populated Please review for accuracy Items with a red asterisk ( ) are mandatory fields

Provide answers to all of the Mandatory Fields and click ldquoSaverdquo When the dialogue box appears click OK to confirm

How Do I Create a Claim If you have submitted a LossDamage Report select ldquoImport LossDamage Itemsrdquo and the system will pull the information you have previously submitted Otherwise click

ldquoAdd Claim Itemsrdquo and fill in mandatory fields and as much other information as possible Click ldquoSaverdquo after entering information for each item

NOTE Repeat for each item you are claiming until all items are added

If all items have been entered scroll to the bottom of the page and click the red SUBMIT CLAIM TO TSP button When the new dialogue box opens click OK
