MISSIONANDVISION
TheHLeeMoffittCancerCenter&ResearchInstitute(Moffitt) was created by the Florida Legislature in1981 and was officially opened in 1986. In 2001,Moffitt became a National Cancer Institute (NCI)‐designatedComprehensiveCancerCenter.
AstheonlyNCI‐designatedComprehensiveCancerCenterbasedinFlorida,Moffittisafull‐service hospital and the third largest cancer center based on outpatient clinic volume.
Employingover5,000physicians,faculty,andstaffincludingover340facultyphysiciansandscientists,Moffittincludesaresearchinstitute,afreestandinginpatient206‐bedhospitalwith36dedicatedbedsfor bone marrow transplant, an active outpatient
clinic, and a high‐risk screening and prevention center.Moffitt also has awide range ofoutreachandcommunityserviceactivitiesandcommunityhospitalandacademicpartnernetworksthroughoutFlorida,thenation,andtheworld.
Moffitt has made a lasting commitment to the prevention and cure of cancer, workingtirelesslyintheareasofpatientcare,research,andeducationtoadvanceonestepfurtherinfightingthisdisease.Moffitt’spriority isensuringitspatientsreceivethe latesttreatmentoptions and thebest chanceof beating cancer. Itsmultispecialty teamof experts takes ahighlycollaborativeapproachtopersonalizedcarebyprovidingdiagnosis,treatment,andsupportinasingle,convenientlocation.
Moffitt’stransformationalclinicalandresearchexpertise,coupledwithvolumesofcasesandtreatmentoptions,keeps itat the forefrontof transforming cancer care and findingcures,notonlyinthenation,butaroundtheworld. PURPOSEANDSCOPEThe Patient Protection and Affordable Care Act (PPACA) of 2010 established four newrequirementsforallnon‐profit501(c)(3)hospitalorganizations.
1. Conductacommunityhealthneedsassessment(CHNA)everythreeyearsand adopt an implementation strategy tomeet needs identified in theassessment.
2. Adoptawrittenfinancialassistancepolicythatincludeseligibilitycriteria,methods used to calculate charges, applications for assistance, andactionsassociatedwithbillingandcollections.
3. Limitchargesforservicestolevelsequivalenttoamountsgenerallybilledforinsuredpatients.
4. Make reasonable efforts to determine an individual’s eligibility forfinancialassistancepriortoextraordinarymeasurestosecurepayment.
ThehealthneedsassessmentmandatedbytheAffordableCareActwasconductedearlierthis year and is published on Moffitt’s website (moffitt.org/publications/community‐benefit/).ThepurposeofthisdocumentistopresenttheimplementationplanadoptedbytheBoardofDirectorstoaddressthehealthneedsidentifiedthroughtheCommunityHealthNeedsAssessment.Thehealthneedsassessmentconductedearlierisbasedonananalysisofpublichealthdata,demographicandsocio‐economicdata,interviewswithkeyinformantsand stakeholders, a community perception survey, and a series of focus groups to helpidentifysignificanthealthneedsinthecommunityservicedbyMoffitt.
CHNAAPPROACHANDPROCESS
InSeptember2015,MoffittcontractedwithLegacyConsultingGrouptoconductitssecondCHNA. In October 2015, Moffitt collaborated with the Florida Department of Health inHillsborough County to complete the CHNA and later establish Healthy Hillsborough.HealthyHillsboroughisacommitteeoflocalhospitals,thecountyhealthdepartment,andFederally‐Qualified Health Centers. The purpose of Healthy Hillsborough is to meet thefederalCHNArequirementsforallentitiesandtoworkcollaborativelyintheprocess.TheorganizationsinvolvedinHealthHillsboroughare:

COMMUNITYSERVED
Based on an assessment of Moffitt’s patient origin and discussions with key Moffittstakeholders, the “community served” is defined as all residents of Hillsborough, Pasco,Pinellas,andPolkcountiesinwestcentralFlorida.Thisareaishometo3.4millionpeoplewith an anticipated growth of 5.5% by 2020. In addition, a secondary service areacomprisingresidentsofCharlotte,Citrus,Desoto,Hardee,Hernando,Highlands,Lake,Lee,Manatee, Sarasota, and Sumter counties has also been defined for Moffitt’s researchinitiatives.Theprimaryandsecondaryserviceareasareshowninthemapbelow.

PREVIOUSCHNAPRIORITIES
In2013,Moffitt’s lastCHNAdeterminedfiveprioritiestoaddress.BelowisasummaryofthoseprioritiesandhoweachpriorityareahasbeenaddressedsincethelastCHNA.
CulturalCompetence.EffortstoaddressCulturalCompetencefocusedonprovidinginternaland external cultural and linguistic competence (CLC) initiatives. The Language Servicesteam averagedmore than 7,000 interpreting encounters andmore than 450 translationprojects each year. Also, language services education was provided to clinical teams toimproveefficacyincommunicatingwithpatients,families,andwithmedicalinterpreters.ACLCSteeringCommitteewas created to improvepatient experience, CLC‐education, datacollection,andlanguageandcommunicationservices.AnExternalAdvisoryCommitteeofnationalexpertsalsohelpedguideMoffittonimplementingCLCinitiatives.Additionally,anonline CLC‐training was piloted for clinical faculty in 2014 and will be implementedorganization‐widein2016.
Prevention,Screening,Education&Outreach.TheMoffittProgramforOutreach,Wellness,EducationandResources(M‐POWER)averagesnearly170healtheducationworkshopsoncancerpreventionandscreeningandreachesover5,000individualseachyear.MolePatrol®organizesover15skincancerscreeningandeducationeventsinthecommunityandreachesover1,000 individuals each year.Moffitt’s Lung andThoracicTumorEducation (LATTE)programconductsover20communityoutreachandadvocacyeventsonlungcancereachyear.
HealthDisparities. Research and educational effortshavebeen implemented to addresscancerhealthdisparities. Specifically,healtheducationhasbeendelivered toCommunityHealthWorkersacrossFloridaviawebinarstohelpreachspecificpopulationswithrisingcancerrates.MoffitthoststheMen’sHealthForumtoprovidehealthscreeningsformenwhoare medically underserved, reaching over 500 men each year. The Population Sciencesresearch program focuses its research strategies to address and reduce cancer healthdisparities.TheClinicalTrialsTaskForcedevelopedstrategiestoincreaseminorityaccrualinclinicaltrials.ThePhysiciansRelationsteampartnerswithphysiciansservinginminoritycatchmentareasandeducatesthemonhealthdisparities.
Access. No‐cost mammograms and prostate cancer screenings have been offered to anaverageofover600womenand200menwhoareuninsuredeachyeartoalleviatefinancialbarriersforaccessingcare.Also,PatientandFamilyServicesofferscabvouchers,roundtriptickets,giftcards,andhousingservices,expendingover$450,000,tohelpprovidesupporttohundredsofpatientsandtheirfamiliesinneedeveryyear.
Community Benefit Structure. The internal infrastructure of community benefit wasimprovedbycreatingaCommunityBenefitCoordinatorpositionwhichwasfilledinJanuary2015.Also,ahospitalboardmemberwasidentifiedasaCommunityBenefitliaisontohelp

engagethehospitalboardregardingcommunitybenefit.Lastly,aCHNASteeringCommitteewasestablishedconsistingofMoffittemployeesandcommunitymembers.

IDENTIFICATIONOFCURRENTHEALTHNEEDS
Basedontheresultsofthecommunityhealthneedsassessment,LegacyConsultingGroupidentifiedtwelvesignificanthealthneedswithinthecommunity.Thesetwelveneedscanbegrouped into four basic categories – Access to Care, Screening and Prevention, GeneralCancerEducationandAwareness,andObesity.

Atthisprioritysettingmeeting,eachofthesetwelveidentifiedhealthneedswereplacedonseparatewallchartsanddisplayedthroughoutthemeetingroom.Eachparticipantwasgiventwelve separate “sticky dots” and asked to vote on their perceivedmost‐pressing healthneeds. TheprioritizingcriteriawerebasedonalignmenttoMoffitt’smission,theabilityandfeasibilitytoaddressaparticularhealthneed,andtheresultingimpactandbenefittothecommunity.Eachparticipantcouldplaceallorpartoftheirallotteddotsonasingleneedorallocate them tomultipleneedsaccording to theirownperceptions.Following the initialvote,participantsregroupedtodiscusshowthevotesweredistributed.Eachparticipantwasgiven a chance to re‐allocate their votes (dots) at their discretion.When panelistsweresatisfiedwiththeresults,votesweretallied.Asaresultofthismeeting,sevenhealthneedswereselectedforinclusionintheimplementationplan.Thesesevenhealthneeds,inpriorityorder,areshownbelow.
