OPERATIONAL PROCEDURES
The State of Oklahoma has entrusted to all Motor License Agencies equipment, supplies and this manual containing the guidelines for the issuance of Oklahoma driver licenses and identification cards. You are, therefore, responsible for safeguarding these items from theft and misuse.
The Oklahoma Department of Public Safety (the Department) hereby sets forth the following requirements:
(a) No documents, equipment or supplies shall be removed from the agency by anyone, unless authorized. Contact Driver License Services for approval.
(b) The Driver License equipment shall be used solely for the issuance of Oklahoma driver licenses or identification cards. Licenses and cards must be made in accordance with and to the specifications in this manual. If they do not meet these specifications, do not issue the license or card. Any substitution of equipment or alteration is prohibited.
(c) Agents are responsible for the safety of any documents in their possession containing the personal information of any applicant. All documents should be secure and/or sent to the Department at required intervals for processing. Disposal of secure documents at the tag agency should be done through shredding or in a manner to sufficiently protect applicants from fraud.
(d) At the close of business each day, the agent shall remove the "Security Laminate", PVC card stock,
and security key and lock them in a safe place and shall also destroy, by shredding or burning, all used color print ribbon. In addition, all printer supplies, DL10, address change and MVR consent forms shall be locked in a safe place. "To lock in a safe place", as used in this Section, is defined as a burglar safe that cannot be picked up and carried out. If the safe is less than 250 pounds, it shall be anchored to a permanent fixture to prevent removal. The safe shall have a combination lock, key lock, or both combination and key lock.
(e) In the event an agency closes, any equipment, supplies and documents issued to the agency by the Department must be returned. Any items not accounted for may result in an investigation by the Department.
(f) In the event an agency relocates, the agent shall notify the Department in writing of the new address and telephone numbers. This must be completed no more than ten (10) days after opening at the new address.
(g) In the event of the loss, theft, or misuse of any equipment, supplies or documents, the agent must notify Driver License Services immediately. The agent shall take an immediate inventory and list all missing items and equipment by number when applicable. The list of any missing inventory, a copy of the police investigation and/or incident report shall be immediately forwarded to Driver License Services. In cases where fraudulent licenses or cards are involved, the agent must immediately contact the Fraud Investigation Unit of the Department. If the Fraud Investigation Unit is unavailable, the agent must notify Driver License Services.
(h) 595:10-1-51: New employees 18 years of age or older, whose duties include the issuance of licenses and cards must apply for and receive a personal access code prior to training or issuing licenses. (See NEW EMPLOYEE AUTHORIZATION)

(i) Driver licenses and identification cards must be issued if the agency is open for business, if the agent is not issuing driver license the Department must be notified of the reason.
(j) The sign notifying of the consent to be registered for Selective Service shall be visible close to the signature pad. Anyone who refuses to sign shall not be issued a driver license or identification card.
(k) Agencies are prohibited from accepting any altered or unapproved documents for the issuance of
Oklahoma driver license or identification cards. (l) Each agent is responsible for informing their employees of the penalties for fraudulent and erroneous
misuse of driver license or identification cards
(m) Agencies can change mailing and residence addresses on the driver license and identification card file, upon request, by using TAGU and having the applicant fill out the address change form. When the residence address is changed, the agency is required to update the county number. This change is required by law. The address change form is to be sent to DPS in the semi monthly reports.
(n) Changes not authorized at an agency, must see an examiner are:
a. First and middle name or correction of last name b. Sex c. Date of Birth d. Endorsements and Restrictions e. Class of license f. Race g. Eye Color h. Adding titles to names, such as sheriff, officer, judge, senator, representative, Dr. etc. i. An address or information to indicate professional status on the face of license such as:
• John Doe, Oklahoma Highway Patrol; or • Robert Doe, M.D.
j. All addresses on driver licenses and identification cards may be a residence or mailing address. Both addresses must be collected and the applicant will have the choice of which address will be placed on the license.
k. County of residence must be entered for all applicants.

(o) Actions not authorized at an agency without other authority are:
a. Giving temporary receipt or authorization to a driver due to system problems. b. Placing anything on license such as stickers for civic clubs, medical symbols, etc., except those
approved by the Department of Public Safety. c. Producing more than one (1) license or card for an applicant at the time of application. d. Adding service charges to application fee for any reason. e. Providing information from license application to private businesses, persons or government
agencies other than the Department of Public Safety, unless authorized by the Department.
(p) It is very important that the tag agent check their email once in the AM and once in the PM to ensure that the office has the latest information regarding the help desk and other information about equipment or training.

UNLAWFUL USE OF LICENSE OR IDENTIFICATION CARD
Each agent is responsible for informing their employees of the penalties for fraudulent and erroneous misuse of driver license or identification cards.
O.S. Title 47, Section 6-301:
“It shall be unlawful for any person to commit any of the acts specified in paragraph 1 or 2 of this section in relation to an Oklahoma driver license or identification card authorized to be issued by the Department of Public Safety pursuant to the provisions of Sections 6-101 through 6-309 of this title or any driver license or other evidence of driving privilege or identification card authorized to be issued by the state of origin.
1. It is a misdemeanor for any licensee:
a. to display or cause or permit to be displayed one’s own license after such license has been suspended, revoked or canceled or to possess one’s own license after having received notice of its suspension, revocation, or cancellation,
b. to lend one’s own license or identification card to any other person or knowingly permit the use thereof by another,
c. to display or cause or permit to be displayed or to possess a license or identification card issued to oneself which bears altered information concerning the date of birth, expiration date, sex, height, eye color, weight or license or card number,
d. to fail or refuse to surrender to the Department upon its lawful demand any license or identification card which has been suspended, revoked or canceled,
e. to permit any unlawful use of a license or identification card issued to oneself,
f. to do any act forbidden or fail to perform any act required by this chapter, excepting those acts as provided in paragraph 2 of this section,
g. to display or represent as one’s own, any license or identification card not issued to such person, unless under conditions provided in subparagraph e of paragraph 2 of this section, or
h. to add to, delete from, alter, or deface the required information on a driver license or identification card.
2. It is a felony for any person:
a. to create, publish or otherwise manufacture an Oklahoma or other state license or identification card or facsimile thereof, or to create, manufacture or possess an engraved plate or other such device, card, laminate, digital image or file, or software for the printing of an Oklahoma or other state license or identification card or facsimile thereof, except as authorized pursuant to this title,
b. to display or cause or permit to be displayed or to knowingly possess any state counterfeit or fictitious license or identification card,
c. to display or cause to be displayed or to knowingly possess any state license or identification card bearing a fictitious or forged name or signature,

d. to display or cause to be displayed or to knowingly possess any state license or identification card bearing the photograph of any person, other than the person named thereon as licensee,
e. to display or represent as one's own, any license or identification card not issued to him, for the purpose of committing a fraud in any commercial transaction or to mislead a peace officer in the performance of his duties, or
f. to use a false or fictitious name in any application for a license or identification card or to knowingly make a false statement or to knowingly conceal a material fact or otherwise commit a fraud in any such application.
3. It is a felony for any employee or person authorized to issue or approve the issuance of licenses or identification cards under this title to knowingly issue or attempt to issue a license or identification card or to knowingly give approval for, cause, or attempt to cause a license or identification card to be issued:
a. to a person not entitled thereto,
b. bearing erroneous information thereon, or
c. bearing the photograph of a person other than the person named thereon.
Such conduct shall be grounds for termination of employment of the employee.
4. The violation of any of the provisions of paragraph 1 of this section shall constitute a misdemeanor and shall, upon conviction thereof, be punishable by a fine of not less than Twenty-five Dollars ($25.00), nor more than Two Hundred Dollars ($200.00); the violation of any of the provisions of paragraph 2 or 3 of this section shall constitute a felony and shall, upon conviction thereof, be punishable by a fine not exceeding Ten Thousand Dollars ($10,000.00) or a term of imprisonment in the State Penitentiary not to exceed seven (7) years, or by both such fine and imprisonment.
5. Notwithstanding any provision of this section, the Commissioner of the Department of Public Safety may, upon the request of the chief administrator of a law enforcement, military, or intelligence agency, authorize the issuance to and display, and possession by a person of a license which would otherwise be a violation of this section, for the sole purpose of aiding in a criminal investigation or a military or intelligence operation. While acting pursuant to such authorization by the Commissioner, such person shall not be prosecuted for a violation under this section. Upon termination of such investigation or operation or upon request of the Commissioner, the chief administrator shall forthwith cause such license to be returned to the Commissioner.”

NEW EMPLOYEE AUTHORIZATION
All applicants are required to have their passcode and finger image on “Capture” before issuing driver license and identification cards. Access codes will not be assigned to anyone under the age of 18 or any person whose driving privilege is suspended, revoked, cancelled or denied. New employees must apply for and receive a temporary pass code (Certification Number) prior to any driver license or identification card training. Trainees are NOT allowed to use a pass code which is issued to another person.
NO EMPLOYEE MAY USE FINGER AUTHORIZATION FOR ANOTHER EMPLOYEE. Employees may not share access codes with another employee. The tag agency is responsible for the training and conduct of each employee.
Pass codes will not be issued until the completed application is approved and on file with Driver License Services.
After temporary approval is issued all operators are required to have their pass code and finger images added to capture by the Department before issuing driver licenses and identification cards.

Follow all directions for original applications carefully. If any step of the original application process is skipped, the application will be denied.
If the application process is not completed within 90 days of Driver License Services receiving the original application, the applicant’s temporary access code will be deactivated and the process will need to be started again.
New application instructions are as follows:
1. Complete sections A and B of the Application for Certification form; fax or mail to DPS. (See form in Section FORMS AND USEFUL INFORMATION or use fillable PDF located at http://ok.gov/dps/mla )
Upon receipt of an application, and after a limited criminal history background check is approved by the Oklahoma Highway Patrol, Driver License Services will process it and advise the agent by telephone of their temporary personal access code and default Password.
The Department will mail a background check packet to the applicant once the application has been approved.
2. Complete the requested information on the fingerprint cards provided by the Department.
The applicant can go to their local police or sheriff’s department to have their prints rolled.
3. Complete the OSBI Record Check Form (Provided).

This must be filled out completely by the prospective employee within 14 days of receiving a temporary access code from Driver License Services.
4. Mail the following:
a. Cover letter to OSBI (Provided by the Department in the background check packet)
b. Completed OSBI Record Check form
c. One (1) completed fingerprint card (Do not bend or fold card. Keep the second fingerprint card for the FBI check)
d. One (1) $19.00 cashier check or money order, payable to “OSBI”

5. The day the fingerprints are mailed to OSBI, the agent must advise Driver License Services by email at:
If notification is not received within 14 days the temporary pass code will be deactivated.
Use the following format to notify the Department that the prints have been submitted:

OSBI will notify the Department of the results.
Driver License Services will notify the agent when they have received confirmation from OSBI.
NOTE: Only after the agent receives confirmation of clearance of the OSBI check from Driver License Services should you proceed to Step 6.
6. Complete and sign the written request for a copy of the applicant’s FBI record (Provided).
7. Mail the following:
a. Completed, signed record request
b. One (1) completed fingerprint card (Do not bend or fold card)
c. One (1) $18.00 cashier’s check or money order, payable to “US TREASURY”
d. Self-addressed, stamped envelope (to the applicant’s mailing address)

The completed record will be mailed to the Oklahoma Department of Public Safety.
8. If the DPS has not received a reply regarding the FBI check within 60 days, the employee shall call the FBI Customer Service Department at 1-304-625-5590. Their hours are Monday through Friday, 8am – 8pm.
The Department shall cancel the access code to issue driver licenses or identification cards upon determination the person's criminal history background investigation does not warrant certification, based upon a misdemeanor conviction for larceny, theft, fraud, perjury, or if the applicant is found to have made false statements on the application within the ten (10) years immediately preceding the application and/or any felony conviction.
If the search results in “no arrest record”, Driver License Services will notify the agent that the employee has attained regular status, thus completing the application process. A certificate of authorization will be mailed to the agency when the applicant has been approved by the Department. This certificate must be posted in the agency in full view of the public.

EMPLOYEE CHANGES Agents are required to notify Driver License Services immediately, in writing, when an employee is no longer employed or allowed to access the Department files so that the employee can be removed from the system and Capture.
Agents must submit the Employee Status Change Notification form and provide the name of the employee, their certification number, and the new agency number if the employee is working for a new agency.
Employees who meet the following requirements must complete a new background check:
A criminal history investigation shall be required if an employee has any break in service and the result of the previous state criminal history investigation on file with the Department for the employee is more than one (1) year old, if the employee changes agents and it has been more than one (1) year since the last background was done, or if a criminal history investigation was not previously required of the employee. The Department may, at its discretion , require a current criminal history investigation of any individual operating the driver license equipment.

EQUIPMENT AND SUPPLIES
The Morpho Trust USA (MTUSA) camera and printer shall be used solely for the issuance of Oklahoma driver licenses and identification cards.
Agents are responsible for keeping all equipment clean and protected.
If an agent provides training for new employees, the agent or their designee is responsible for maintaining proper operational techniques.
Equipment Problems:
In the event you experience malfunctions of the photographic equipment and are unable to fix it or need to ask a question regarding equipment performance, the operator may call the help desk toll free at 1-866-464-0340. The help desk will attempt to help you correct the problem. If the problem is beyond the scope of the help desk, the help desk will contact MTUSA. (Do NOT call a help desk employee on their private line – USE THE HELP DESK NUMBER)
The help desk’s hours of operation are Monday through Friday from 8:00am – 5:30pm and on Saturday 9:00am – 1:00pm.
Printer Supplies:
At the close of business each day, the agent shall remove the "Security Laminate", PVC card stock, and security key and lock them in a safe place and shall also destroy, by shredding or burning, all used color print ribbon. In addition, all printer supplies, DL10 and MVR consent forms shall be locked in a safe place. "To lock in a safe place", as used in this Section, is defined as a burglar safe that cannot be picked up and carried out. If the safe is under 250 pounds, it shall be anchored to a permanent fixture to prevent removal. The safe shall have a combination lock, key lock, or both combination and key lock. Ordering supplies is generally unnecessary provided the agent keeps proper inventory usage updated in the SIMS program found on the desktop of your computer. In the event that an inventory problem occurs, the agent shall contact SIMS at 1-877-314-5649 and they will check on your inventory or shipment. If the agent does not log their daily usage, SIMS will not know when shipments need to be made.
Upon receipt of new supplies, assure the inventory received matches the shipping invoice. Be sure all supplies are free of defect before acknowledging the shipment. This includes checking that the double OK is on the middle of the license cards. If there are any discrepancies, contact SIMS.
Driver License Manuals:
Driver License Manuals for Class D licenses are available at tag agents that have chosen to purchase and sell the manuals. A list of participating tag agents is available on the DPS web site.
Class D, CDL and motorcycle license manuals can be found on the DPS web site at https://www.ok.gov/dps/Obtain_an_Oklahoma_Driver_License_ID_Card/Manuals.html and are free to print for personal use.
A limited amount of CDL manuals can be obtained at Driver Examining sites at no cost.

DRIVER LICENSES AND IDENTIFICATION CARDS
TYPES OF LICENSES OR CARDS Identification Card – issued as a form of identification for qualified applicants who show sufficient identification upon
issuance. Identification cards are issued with a red background.
Class D License – Issued to serve as proof of the applicant’s privilege to drive a regular motor vehicle. Class D Licenses are issued to applicants who have shown sufficient identification and sufficient skills through written and drive testing. Class D licenses are issued with a blue background.
CDL Learners Permit – Starting 11/01/2013 all applicants for an original CDL License or anyone applying to add any endorsement or remove a restriction that requires a skills examination must obtain a CDL Leaners Permit.
CDL Licenses – Class A, B, C licenses are issued to serve as proof of the applicant’s privilege to drive commercial motor vehicles (as defined by the Department). CDL licenses are issued to applicants who have shown sufficient identification and sufficient skills through written and drive testing. CDL licenses are issued with a yellow background.

CLASS A VEHICLES: Any combination of motor vehicles with a Gross Combination Weight Rating (GCWR) of 26,001 pounds or more provided the vehicle being towed has a Gross Vehicle Weight Rating (GVWR) of 10,000 pounds or more

Pro-Rated License Fees
Oklahoma Statutes provide for a pro-rated fee to be charged for a Class D license. There are no pro-rated fees for CDL licenses. These fees are automatically assessed by the computer. The pro-rated fees start in the calendar year in which the applicant turns the applicable age.
The pro-rated fees are as follows:

Pro-rated fees do not apply to any testing fees. All testing fees are still required.
Applicants who are 65 years of age or older will be issued an identification card at no cost.
DRIVER LICENSE EXPIRATION [47 O.S. Par 6-115] Every driver license shall be issued for a period of no more than four (4) years. The expiration date of an initial license shall be four (4) years from the last day of the month of issuance or four (4) years from the last day of the birth month of the applicant immediately preceding the date of issuance, if requested by the applicant. The expiration date of renewal licenses shall be four (4) years from the last day of the month of expiration of the previous license or four (4) years from the last day of the birth month of the licensee immediately preceding the date of issuance, if requested by the applicant.
The agent does not need to calculate the date of expiration, as the computer will automatically calculate it.
Any license that has been expired for more than three (3) years may not be obtained at the tag agency. The applicant must return to a Driver License Examiner. The applicant may have to retest to obtain their Oklahoma license.
Licenses and identification cards that are issued in temporary status will be issued for a period of time determined by the Driver License Examiner. Licenses and identification cards that are labeled Sex Offender will be issued for a period of one (1) year from the date of issuance.
Identification cards issued to applicants who are 65 years of age or older will be issued for an indefinite period of time.
Licenses and identification cards which are expired over 1 year require the applicant to show a birth certificate or a valid unexpired passport to renew. Only agents who have been trained in accepting birth certificates may process these requests. Only Driver License Examiners may process these requests using a passport.

APPROVED SYMBOLS ON LICENSES AND IDENTIFICATION CARDS Special symbols are approved through legislation to appear on Oklahoma licenses and identification cards. No other symbols may be added.
Deaf or Hard of Hearing:
Title 47 O.S., Section 6-111 provides for a symbol to appear on the license or identification card, to show that the applicant is “Deaf or Hard of Hearing”. This symbol will appear at the bottom right of the license or identification card as an ear with a line crossed through it diagonally.
If a licensee needs to have the Deaf or Hard of Hearing symbol added or removed, refer them to a Driver License Examiner so the file may be updated. A replacement license will need to be purchased by the applicant.
Organ Donor:
Effective January 1, 1984, Oklahoma statutes provided for the Oklahoma Organ/Tissue Donor Program. Applicants are able to note their choice to be organ and tissue donors on their license.
If the applicant chooses to be a donor, this symbol will appear at the bottom right of the license or identification card as a red heart.
If the applicant chooses not to be a donor, no symbol will appear.
The choice to be a donor or not to be a donor will appear on the signature pad during issuance of the license or identification card. Applicants must check the box next to the option to participate and be a donor. Applicants may change their answer each time a replacement or renewal card is issued.
Applicants under the age of 16 will not have the option to be an organ/tissue donor and display the red heart on their license or identification card.
An organ/tissue donor designation on a driver license or identification card takes precedence over the wishes of the family (O.S. Title 63, Section 2214.E).
VET:
Title 47 O.S., Section 6-124 provides for a symbol to appear on the license or identification card to show that the applicant is a Veteran. The symbol will appear at the top left hand corner of the license in lieu of the Osage shield which appears on regular licenses and identification cards.
The applicant must show proof of eligibility to receive this symbol:
a. A valid Uniformed Services Identity Card, or
b. A United States Department of Defense Form (DD) 214, (DD) 215 or World War II discharge document WD AGO Form or NavPers Form that shows a discharge status of “honorable” or “general under honorable conditions”, or
c. A United States Department of Veterans Affairs photo identification card.

TEMPORARY:
Statutes require this symbol for applicants who are temporarily issued a driver license or identification card, upon Driver License Examiner approval. The symbol will appear at the top of the license above “Oklahoma” in red letters.
MODIFIED:
Applicants who are under administrative action on their license file may be required to carry a modified license. This symbol appears next to the Class type of the license in red letters.
SEX OFFENDER:
Statutes provide this symbol for those individuals who have a previous conviction and the system deems it necessary. This symbol appears at the bottom of the applicant’s picture and under the Oklahoma logo in red letters.
INTERLOCK REQUIRED:
Operators who are required to have an ignition interlock device installed in their vehicle.

REASONS AN APPLICANT MUST SEE AN EXAMINER The most common reason a tag agent will send an applicant to see a Driver License Examiner is due to prompts from the computer system. At any time the system prompts the applicant to see an Examiner the tag agent shall relay the information. Agents should not call the help desk to inquire for the reason behind the prompt, as the help desk cannot release the information over the phone. If the applicant asks for reasoning, the agent shall tell them they do not have any other information on their system. The agent shall instruct the applicant to take any identification they have with them, including but not limited to a birth certificate if available.
Other reasons applicants must see an Examiner are to:
a. Be issued an original driver license, permit or identification card. Testing may be required.
NOTE: Tag agents are allowed to issue original identification cards provided the applicant presents a valid unexpired Oklahoma driver license. TAG AGENT PROCEDURES subsection ACTIONS ALLOWED AND REQUIRED BY TAG AGENTS for more information.
b. Update, renew or replace any type of CDL license
c. Update, renew or replace any type of card using INS documents
d. Change or correct first or middle names
e. Change or correct eye color, race, date of birth or sex.
f. Renew a driver license for members of military using military orders
g. Update or renew licenses which have expired over three (3) years.
At any time an applicant sees an examiner, a DL10 form should be issued. (See next page for DL10 information.)

THE DL10 FORM The DL10 form is the official communication from the Driver License Examiner to the tag agency. It should always be signed and sealed by a Driver License Examiner. DL10’s can be one or two pages in length.
The DL10 form is used for:
1. All original applications.
2. Changing license class.
3. Examiner authorized renewals and replacements.
4. Any corrections that cannot be authorized by the tag agency.
5. Adding or removing endorsements and restrictions.
DL10 forms will include:
1. The file number applicable to the driver license or identification card, located in the top right corner.
2. The issue date of the form, located in the top left corner.
3. The expiration date of the form, located in the top paragraph.
4. The type of transaction for which the DL10 was issued – ORIGINAL, REPLACEMENT, or RENEWAL, located in the top paragraph.
5. The applicant’s personal information including name and demographics, located just below the type of transaction.
6. The type of identification that was shown to the Driver License Examiner, located under the applicant’s address.
7. The application and testing fees which are due to the tag agency, located under the identification.
8. Questions applicable to the type of application for which the DL10 was issued. These questions are determined by the computer.
9. The applicant’s signature.
10. The Examiner’s signature and seal.
Once a DL10 is issued by a Driver License Examiner, no changes can be made by the tag agent. The applicant must return to the Examiner for any changes needed. This includes changes to the file number. If a DL10 form lists a social security number on the top left corner, the applicant must return to the Examiner. Tag agents are not authorized to change a number on a DL10.
A DL10 must be processed before the expiration date listed on the form. If the applicant fails to have the form processed by the expiration date, they must return to the Examiner.

MILITARY PERSONNEL Any person or civilian contractor and/or spouse or dependent of a person on active duty with the Armed Forces of the United States having a valid driver license issued by the State of Oklahoma for the operation of motor vehicles upon the highways of the state shall have without additional charge, a valid license for the duration of such service and for a period of sixty (60) days from and after the return of the person or spouse/dependent of the person to the State of Oklahoma.
Upon returning to Oklahoma, if a member of military, spouse or dependent of a member of military wishes to renew an expired license after the extension period, they must see an examiner with their orders showing they were out of state when the license expired.
RENEWAL BY MAIL Oklahoma residents who are temporarily out of state may apply for a renewal or replacement license or identification card by mail. To do so, the applicant must fill out the request form found on the public website at https://www.ok.gov/dps/Renew_an_Oklahoma_Driver_License_ID_Card/Driver_License_or_ID_Card_Renewal_or_Replacement_by_Mail_When_Out-of-State.html Applicants must provide appropriate forms of identification when requesting a license or identification card by mail. Full instructions and a list of acceptable forms of identification can be found on the request form.
CDL license holders are not qualified for renewal or replacement by mail, even if they wish to drop to a Class D license. These applicants MUST return to Oklahoma and appear to a Driver License Examiner.

TAG AGENT PROCEDURES
Licenses and identification cards must be made in accordance to the specifications in this manual. If they do not meet the specifications, the card shall not be issued to the applicant. Any substitution of equipment or alteration is prohibited.
[Title 47 O.S. 61, Sec 6-101] No person shall be permitted to have more than one (1) valid driver license at any time.
Applicants must surrender all CDL licenses and out of state licenses when obtaining an original, renewal, or replacement Oklahoma license. Tag agents must forward all surrendered licenses to the Department with their semi-monthly report.
Licenses and identification cards which are not CDL or from out of state must be given back to the applicant after cutting the corner or punching a hole In the license. Do not cut the corner or punch a hole in the license until a new license is printed, passed inspection and is issued to the applicant. Agents are not to confiscate or keep these licenses.
All applicants for an original, renewal or replacement driver license or identification card shall be given an opportunity to make a minimum voluntary contribution of One Dollar ($1.00) to be credited to the Oklahoma Organ/Tissue Donor Education and Awareness Program Revolving Fund. Any voluntary contribution shall be added to the driver license or identification card fee and deposited with the Oklahoma Tax Commission, who will forward donations to the Oklahoma Organ/Tissue Donor Education and Awareness Program Revolving Fund.
a. Applicants will be required to check a box on the signature pad, if they wish to donate $1.00.
b. Applicants may donate more than $1.00. If this situation arises, the agent must call the tag agent help desk to request the donation to be added to their report.
IDENTIFICATION REQUIRED AND ACCEPTED All applicants for an original, renewal or replacement license and identification card must furnish proof of identity.
If a file has been previously established by the Department, the tag agent shall use finger image (FI) and digital photograph (DP) comparisons for primary and secondary forms of identification.
*If the finger image comparison fails during a transaction, the agent may override the failure and the system will do a digital photo comparison. If the photo comparison fails the operator must see 2 other forms of identification to substitute the failed comparison. One primary and one secondary. This applies to transactions with or without a DL10.
A DL10 is not a form of identification.
When substituting identification for finger image or digital photograph comparisons many types of identification can be used. All identification used must be original and/or certified copies. No photocopies are accepted. Accepted identification includes (See next page):

Agents who have not received and passed training from the Department of Public Safety in accepting birth certificates shall not accept them as proof of legal presence for renewing a license expired for more than 1 year. At any time, the Department may require additional documentation.
ACTIONS ALLOWED AND REQUIRED BY TAG AGENTS Agents are NOT allowed to make any changes if the applicant has a DL10. The applicant must return to the examiner to fix any errors. This includes changing the license number from a social security number. Applicants who present a DL10 with a social security number listed as the file number MUST return to a Driver License Examiner.
The following are actions for which the tag agent is authorized.
Changing a Last Name:
Agents may change only the last name on an Oklahoma file provided the applicant shows proper documentation. Acceptable name change documents include a divorce decree, adoption decree, name change decree, marriage license, common law marriage affidavit or establishment of record of birth. Petitions or applications for changes are not accepted.
If any name change document requires more than the last name to be changed, the applicant must see an Examiner.
a. Oklahoma marriage licenses which note “Name To Be Known As” under the signature: Agents must use the name the applicant signs as their new legal name.
b. For a common law marriage, “Affidavit of Common Law Marriage” containing the notarized signatures of the husband and wife must be present.
Documents provided by the applicant for a name change must be the original and/or certified copy and not photocopies.
Agents may correct simple spelling errors on last names provided the applicant shows a valid name change document or a birth certificate.
Updating Height and Weight:
Agents are required to update height and weight information upon each issuance of a license or identification card.
Updating Addresses:
Agents are required to verify the applicant’s mailing and residential addresses are correct upon each issuance of a license or identification card.
If changes must be made, the agent must verify and enter the appropriate county code which corresponds with the applicant’s county of residence.
Applicants MUST provide an Oklahoma residential address. This is to keep the Department in compliance with 911 operations. A PO Box may not be used as a residential address unless the applicant provides proof that they are an approved participant in the Address Confidentiality Program.

Any change to address or name will require the Address/Name change form be filled out and sent in with the semi monthly report.
Number Changes:
Agents may change the license or identification number for applicants not presenting a DL10 upon renewal or replacement.
For driver license number changes, see page 34.
For identification card number changes, the agent shall call their help desk for assistance.
Removal of a G Restriction:
Agents may remove a G restriction from a Class D license without a DL10 only if the system prompts them to. The computer will prompt for this during the replacement or renewal procedures. Follow all prompts on-screen.
If an applicant believes their restriction should be removed and the agent was not prompted, refer the applicant to a Driver License Examiner.
Removing a G restriction requires the applicant to pay the appropriate license fees.
Free Driver Licenses:
1. Oklahoma statutes provide the issuance of a free Class D license to persons who are 65 years of age or older or who will turn 65 in the calendar year of their license. The applicant must be 65 to have a free identification card issuance. The system will automatically calculate these free licenses.
2. No person who has been honorably discharged from active service in any branch of the Armed Forces of the United States or Oklahoma National Guard and who has been certified by the United States Department of Veterans Affairs, its successor, or the Armed Forces of the United States to be a disabled veteran in receipt of compensation at the one-hundred-percent rate for a permanent disability sustained through military action or accident resulting from disease contracted while in such active service shall be charged a fee for the issuance or renewal of an Oklahoma driver license.
These veterans shall receive an original, renewal or replacement driver license or identification card at no charge, upon presentation of the required documents:
a. Proof of 100% status from the U.S. Department of Veterans Affairs, or
b. A tax exempt card from the Oklahoma Tax Commission showing exemption from state tax based on 100% status.
Agents must call their help desk to issue these licenses at no charge.
If the applicant has a CDL license they must first go to a Driver license examiner and obtain a DL10 form.
3. Agents are authorized to issue a free license or identification card to applicants whose card has suffered mechanical or material failure (e.g. laminate peeling).

All free replacements that are due to mechanical error or those that are DPS authorized must be without changes. If there are changes of any kind, the appropriate fee will be required.
Out of State Transfers
Agents may transfer a license from out of state, provided the applicant has previously had an Oklahoma driver license. Applicants who hold an out of state CDL license must return to an Examiner.
If the applicant’s Oklahoma license has been expired more than 1 year, the applicant must provide the agent with a certified copy of their birth certificate. If the agent has not been birth certificate trained, the applicant must see a Driver License Examiner.
If the applicant’s Oklahoma license has been expired more than 3 years, the applicant must see a Driver License Examiner.
The agent MUST call Driver License Services for assistance in transferring the out of state license back to Oklahoma, even if the system doesn’t prompt them to.
Original Identification Card Issuance
Tag agents may issue original identifications cards to applicants who:
1. Who show a valid, unexpired Oklahoma driver license.
See TAG AGENT MENUS AND ISSUANCE PROCEDURES for the procedure to issue these identification cards.

PHOTOGRAPHS “A photo which, in the opinion of the Commissioner or his designee, distracts from the identification purpose will not be allowed to appear on an Oklahoma driver license or identification card.” Photos muct be taken looking straight ahead from the top of the head to the neckline.
Any object, costume or gesture that is not necessary for identification will not be permitted.
A head covering is permitted if the head covering does not:
a. Obscure or obstruct a full front view of the face from hairline to chin, or
b. Display any:
1) Logo 2) Insignia, Symbol or Regalia 3) Word or Words 4) Letter, Number, Character or any combination thereof 5) Graphic Design, other than the overall pattern of the fabric or material, or
c. Cast a shadow on the face of the person
Objects that were not designed for the purpose of a head covering will not be allowed.
Prescription or non-prescription glasses are not permitted.
If the agent takes a picture which does not meet the guidelines, the picture may be retaken while the agent is still on the photo screen during issuance.
Pictures which meet the guidelines may not be retaken because the applicant does not like it.

START UP AND LOGIN PROCEDURES The computer tower should left in a powered on state at all times. The battery backup is there to allow a clean shutdown if there is a power failure. Do not attempt to process applicants when power has failed. Once the equipment is installed do not move power cords or plug any additional items in the battery backup. Opening procedures: Make sure power is turned on for the monitor, printer and camera.

g) Enter okdpscdl and press the <enter> Key Once you are on the mainframe you can left mouse click on the lower right of the task bar to go to the desktop.
Pressing the ESC key while on the mainframe will clear the screen and move the cursor to the upper left corner of the screen which is the command input area.
f) Double Click on the MTUSA Icon. This will display the Capture Software Logon Screen. Enter Your Active Directory login and password. Make sure the Domain shows okddls. Left mouse click on the OK button. If you have enrolled your finger image you can place your finger on the reader to login. This will complete the login process for an Motor License Agent . Use the alt-Tab to move between programs or left mouse the desired icon on the bottom task bar.

The menu the system directs agents to in order to complete issuance of an identification card. This menu is brought up automatically. The agent does not need to start transactions with TAGI. TAGU
The menu used to begin transactions when the driver license or identification number is not known. This menu will not be used for transactions with DL10s as all DL10s list the driver license or identification number. Record can be found using the name and date of birth.
TAGR
The menu used to print daily and semi-monthly reports.
TAGS
The menu used to view and print daily lists of applicants who have been issued a driver license or identification card.
MVRP
The menu used to print Motor Vehicle Reports. (See Section 9.MOTOR VEHICLE REPORTS for MVRP procedure.)
Tag agents should begin all transactions using TAGA if the driver license or identification number is known. TAGA will automatically direct the agent to the needed menus to complete issuance of the driver license or identification number.
NOTE: The procedures throughout this section provide screen shots of different steps the agent will use for issuance of driver licenses and identification cards. The colors and fonts may differ from your screen.
If at any point during the following steps you press Enter and receive prompts and/or question marks on the screen, follow the prompts to correct any mistakes or empty fields.

CAPTURE PROGRAM OPERATION Once the applicant is processed on the main frame the information is transferred to the MTUSA Capture program. This is a tab type screen to step you through the image capture procedure.
• Make sure the upload server and Print Manger buttons show a green checkmark. • Select applicant is active tab.
• To select the applicant double click the left mouse button with the mouse pointer over the name.

• Review the information displayed. If all information is good and the correct applicant matches the photo click on the Next Button.
The Red X located in the top name field will execute a VOID operation if clicked before a card is printed.

MOTOR VEHICLE REPORTS (MVRs) Agents process motor vehicle reports and pay the required fees to the Oklahoma Tax Commission. Daily and semi-monthly reports will reflect the number of reports printed and the amount of money due to the Oklahoma Tax Commission.
Currently, the public’s fee for an MVR is $25 at a tag agency. Any licensee over 65 years of age requesting an MVR will not be charged. If someone other than the licensee requests an MVR for someone over 65, the requester will be charged the regular fee.
MVRs also serve as clearance letters. A clearance letter may be requested by another licensing jurisdiction when a licensee is suspended in Oklahoma or is used in lieu of a surrendered license. The clearance statement is at the end of the MVR, provided the licensee is not under suspension.
Before an MVR can be issued, a records request form and/or consent to release records form must be filled out. The agent shall verify that they are using the most up-to-date form by locating it on www.dps.state.ok.us/mla. A sample of the MVR request form can be found in Section .FORMS. All request forms must be completely and properly filled out, including the agent’s name and agency number. If the agency information is not completed on the form, agents may not get credit for those MVRs.
MVRs which are run in error or do not print to equipment failures qualify for a refund to the tag agency. The agent must send in the regular fees to the Oklahoma Tax Commission. A notarized letter stating what happened and that there was “no fraud intended” shall be forwarded with the semi-monthly report to Driver License Services. This letter will be forwarded to the Oklahoma Tax Commission for reimbursement to the agent.
To generate an MVR, follow the following steps.

1. In the mainframe program, type MVRP and press Enter. The following format will appear.
2. Enter the name of the person requesting the MVR in the format [LASTNAME,FIRSTNAME MI] (This may not always be the person on the file.)

DRIVER LICENSE REPORTS Prompt reporting of driver license activity is imperative in order to keep the records of the Department current. Reports shall be forwarded to the Driver License Services Division in accordance with the rules and regulations of the Oklahoma Tax Commission or as prescribed by law. Currently, reports shall be made every 15 days, the first through the fifteenth and the sixteenth through the last day of the month. These reports shall be mailed in no later than ten (10) working days after the cut-off date of the report period. Preparation of reports is very important. They must be assembled in the proper order for handling and interpretation by Driver License Services.
a. Corrections needed should be made within seven (7) calendar days of the daily report. This information
is transferred to the Oklahoma Tax Commission by tape every seven (7) days and corrections cannot be made by Driver License Services after transfer has been made.
b. Semi-monthly reports should to be forwarded to:

Care should be taken when preparing your report for shipment. Make sure that the packaging will be substantial enough not to break open while in transit. The following guidelines will help you prepare your semi-monthly reports to assist Driver License Services in processing the many reports which are sent in. Be aware that DL10’s sent into Driver License Services will be scanned into the digital system. Therefore, extra care should be taken to be sure that DL10’s are kept in good order; going against the given guidelines when preparing your report can compromise the integrity of the documents.

PREPARING YOUR SEMI-MONTHLY REPORT 1. Assure all DL10’s accepted within the report period are surrendered. Sort all DL10’s by category, NOT DAY. Use
the following categories:
a. ALL DL10’s which include an out of state license b. Original, Renewal, Replacement CDL (Class A, B, C) c. Original Class D and ID cards d. Renewal, Replacement Class D and ID cards
2. DEFACE ALL LICENSES AND ID CARDS by clipping the bottom left corner or punching a hole in each card.
3. Keep all surrendered out of state and CDL licenses with their corresponding DL10.
a. Write the Oklahoma file number on the back of all out of state licenses. Please use a permanent black marker.
b. Attach all cards to the bottom left hand corner of the DL10.
i. Use ONE staple to attach the license to the DL10. DO NOT USE heavy staples. ii. DO NOT USE TAPE of any kind. iii. If your stapler will not go through the card, use ONE paper clip.
c. Cards which are not from out of state or are not CDL should not have been surrendered. Remember to return these cards back to the applicant after being defaced.
4. Bundle all voided and sample cards together.
a. On the back of each voided card, write the reason you voided it. Please use a permanent black marker.
5. Bundle surrendered out of state licenses which do not have a DL10 together.
a. Write the Oklahoma license number on the back of the out of state license. Please use a permanent black marker.
6. MVR requests included in the report should be on the most recent form found at www.dps.state.ok.us/mla.
a. Request forms should be turned in by themselves with nothing attached – including copies of the applicant’s license or receipts. b. Assure the bottom of each form is filled in correctly with the name of the person who issued the MVR and the agent number. c. Keep MVR requests separate from other forms, preferably in a separate envelope within your report.
7. If you are birth certificate certified, include photocopies of the accepted birth certificates.
a. Do not send in photocopies of marriage licenses or court orders. b. Write your agency number and the applicant’s license or identification card number on each photographed birth certificate.
8. Include one (1) copy of your semi-monthly report. Void reports are not necessary in this report.
9. Package your report carefully.
a. Keep DL10’s FLAT if possible. If not possible, fold DL10’s in half ONCE with the print facing out. DO NOT ROLL DL10’s. b. Keep all paperwork facing the same direction. c. Use a sturdy box or envelope that will not fall apart in the mail.

10. Write your agency number and report period on the outside of the envelope or box to streamline the check-in process when your report arrives at Driver License Services.

Section 10.RETURN CHECK POLICY When a motor license agency is given a check for a driver license or identification card and the check does not clear the bank for any reason, the agency must try to collect the fee twice.
After the agency has made two attempts to collect the fee, and the licensee/card holder refuses or fails to comply, the agency must complete the Dishonored Check/ID Processing Form located on the following page or in Section 12.FORMS AND USEFUL INFORMATION. Each agency is responsible for maintaining their inventory of these forms.
To complete the return check process:
1. Complete section A of the Dishonored Check/ID Processing Form. All fields in section A must be filled in.
2. Attach one (1) photocopy of the returned check to the request.
3. Fax the photocopy of the returned check and the form with section A complete to Driver License Services at 405-425-2062 or mail to:

Upon receipt of the form and copy of the check, the Department will initiate a cancellation notice on driver licenses or a hold on identification card files.
The cancellation notice or hold will direct the applicant back to the tag agency that received the returned check.
4. Once the applicant returns to pay their fees, the tag agent shall collect a returned check fee of twenty five dollars ($25.00) for each returned check, plus the amount of the check. The agency requesting the cancellation is the only agency authorized to accept payment for the dishonored check.
5. Upon receipt of all fees due by the applicant, the agent shall complete section C of the saved Dishonored Check Processing Form. The completed form shall be given to the applicant to take to a Driver Compliance office or faxed to Driver License Services for further processing.
If the applicant chooses to take the form to Driver Compliance themselves, the agent shall notify them that their license will not be reinstated until they do so.
If the form is faxed to the Department, Driver License Services shall refer the case to Driver Compliance for reinstatement. Reinstatement can take 24-48 hours.
6. Upon completion of all steps, the agent may dispose of their copies of the form and check in a manner to protect the applicant from fraud.
NOTE: The cancellation form must include the customer’s driver license or identification card number if the agency accepts checks for driver licenses and identification cards. The agent shall write the driver license and identification card number on the check prior to issuing any cards as there will no longer be a way to retrieve the number after a check has been returned.

Section 11.HANDICAP PARKING PLACARDS The Department is authorized to issue a detachable Handicap Parking Placard to the qualified persons upon receiving a completed application form provided by this agency. These forms are made available at the following locations:

There are two types of placards issued:
1. Temporary: Issued to persons with temporary disabilities; this placard is issued for up to six (6) months and is valid through the last day of the expiration month on the placard. The period is determined by the Physician=s estimate of the termination of such disability. The placard shall be returned to the Department of Public Safety when expired.
2. 5 Year: Issued to persons with long-term disabilities if such disability is certified by the Physician; this placard will be issued for a period of five (5) years and must be renewed five (5) years from the last day of the month in which the placard was issued. Upon expiration, the placard shall be returned to the Department of Public Safety.
The person to whom the placard was issued may obtain a renewal placard by re-applying to the Department of Public Safety in the same manner as when the placard was originally issued.
In the event a placard, whether a five (5) year or a temporary, is lost, mutilated, destroyed or stolen the person to whom such placard was issued may apply in writing (complete the top portion of the application furnished by the Department of Public Safety) to this agency for a replacement placard.

FREQUENTLY ASKED QUESTIONS REGARDING PLACARDS AND PARKING Question: Can I obtain the Handicap Placard any place other than the Oklahoma Department of Public Safety in
Oklahoma City? Answer: No. However the applications are handled by mail or by walk in. The turn-around time is approximately twenty
(20) working days when received by mail.
Question: Is there a fee to obtain a parking placard? Answer: No, there is no longer a fee.
Question: How long does it take to process my application? Answer: About twenty (20) working days. You may bring the application to our office and we will process it while you
wait provided the information is complete and correct. However, we prefer that you mail the application, to avoid congestion and waiting time in our office.
Question: What are the business hours at the Department of Public Safety? Answer: Business hours are Monday through Friday, 8:00 a.m. to 4:30 p.m.
Question: I have lost my placard and don=t have the number, what do I need to do? Answer: Obtain an application form, fill out everything but the information supplied by the Physician and mail or bring it
to the Department of Public Safety. The other information is still on file. If it is time for renewal you must provide a new application with the Physician=s portion completed.
Question: Do I always need to hang the placard when I park? Answer: Yes. Remember that the parking placard is also required to obtain a special license plate through the Oklahoma
Tax Commission.
NOTE: Applicants cannot photocopy placards for use in other vehicles.
