replacement

Hallux rigidus:
Adolescents & adults
affects 1 in 45 adults
> 50 yrs.
Hartrup & Johnson:Grade 1: mild to moderate osteophytes, joint space preservedGrade 11: moderate osteophytes, narrowing of joint space & subchondral sclerosisGrade 111: marked osteophytes, loss of joint space, plus minus subcondral cysts
Treatment
• Non-operative
• Cheilectomy
• Keller resection arthroplasty
• Moberg osteotomy
• Watermann osteotomy
• Silastic arthroplasty
• Joint replacement
• arthrodesis
Arthrodesis vs. joint replacement
• Arthrodesis• Strong 1st ray• For active lifestyle• Usual 80% + success
• Nonunion• Malunion• OA ipj
• Joint replacement• Range of movement• Footwear • ? Success rates
• ? Longevity• ? Sesamoid problems• ? Metatarsalgia
MOJE 1st metatarsophalangeal joint replacement
Zirconium ceramic prosthesis
Coated with Bioverit 1 – good biocompatibility & osteo-integration
44 year old dental technician: previous arthrodesis in excessive dorsiflexion; transfer metatarsalgia and arthritis of ip joint of great toe
Ipj fusion united
Regained good range of movement of 1st mtpj
Very pleased with result
Slight cockup deformity
Moje + Stainsby 2nd toe
Pleased with result – a little stiff initially because of lack of physiotherapy
46 year old theatre nurse; previous surgery elsewhere; presented with recurrent hallux valgus & painful 2nd mtpj
55 year old lady who had a Moje done elsewhere
Very important to get early range of movement exercises
Physiotherapy at three weeks
Prospective randomised controlled trial comparing arthrodesis to joint replacement
Patients aged 45 years or older
RA excluded
AOFAS scores
Gait analysis looking at 1st mtpj loading
To date: 24 patients entered
Results• Too early for trial results – initial results
satisfactory
• Overall results• 36 Mojes to date• 2 revisions of screwfit to pressfit• 1 case of early metatarsalgia – now resolving• No wound problems• No sesamoid problems

Wichtiger Anhang :
• Bitte lesen Sie die folgenden Informationen
Bis 2003 aufgetretene Probleme
• Gefahr des Einsinterns eines Implantates
• Implantatlockerungen
