in Walmart purchases on your Anthem Rewards Card! Complete the Health Needs Screening (HNS), and Anthem Blue Cross and Blue Shield will give you $10 on your Anthem Rewards Card! To earn your rewards, you must complete the HNS within 90 days of joining Anthem.

You can complete the HNS and earn your reward in one of three ways:

1. Take your Anthem Rewards Card to a Pursuant Health kiosk in a Walmart pharmacy (see right).

3. Call 1-866-408-6131 (Hoosier Healthwise, HIP) or 1-844-284-1797 (Hoosier Care Connect). A representative will help you.

To take the HNS and earn your reward at a Walmart pharmacy kiosk:

➊ Scan the barcode on your Anthem Rewards Card at the kiosk.

➋ Complete the HNS. It only takes about 5-10 minutes.

➌ Earn $10 for completing the screening. The money will be loaded right to your card.

➍ Start shopping at Walmart! And keep this card to possibly earn more rewards for being healthy!
