A Service Improvement Project: Moving From Intraoperative Intravenous to Preoperative Oral Simple Analgesics in Day Surgery Kim Wild - ST7, Wasim Huda - Consultant, Shilpa Pangam - Consultant, West Suffolk Hospital Background Day surgery guideline standard: “Prophylactic oral analgesics with long-acting NSAIDs should be given to all patients if not contraindicated” 1.Practice at the West Suffolk Hospital (WSH) is to administer intravenous (IV) analgesia intraoperatively. Oral simple analgesics have been proven to be effective for acute pain postoperative pain. 2. Converting to preoperative oral analgesics would: save money and reduce our carbon footprint (fewer disposables), whilst still providing effective analgesia. We: 1. Audited analgesic practice for 50 DSU laparoscopic cholecystectomies (8/2014 - 11/2014). 2. Reviewed the evidence base. 3. Questioned three hospitals that currently administer prophylactic oral analgesics. Key Findings 1. Only 2/50 (4%) received preoperative oral paracetamol 1g. All other analgesics were given IV intraoperatively 2. 48 (96%) received IV paracetamol 1g 3. 47 (94%) received IV NSAIDs. 4. 71% of IV analgesics were administered within 15 minutes of the start of anaesthesia. Potential Cost Savings: 1. The WSH day surgery spent ~£1547 on paracetamol and ~£7144 on NSAIDs over one year (03/2014 - 02/2015). 2. If oral preoperative paracetamol and long-acting ibuprofen had been used instead, ~£485 would have been spent, i.e. a 94% reduction in expenditure. Recommendations The findings were presented at an anaesthetic clinical governance meeting, and a recommendation was made to introduce the widespread use of oral preoperative analgesia (paracetamol and ibuprofen retard) in DSU. Actions 1. A successful business case was presented to obtain ibuprofen retard on formulary. 2. The DSU drug chart was revised to include preprinted prescriptions in the “once only drugs” section of: ibuprofen retard, and paracetamol. 3. A guideline to introduce preoperative analgesic practice was written. References 1. Verma R, Alladi R, Jackson I, et al. Day case and short stay surgery: 2, Anaesthesia 2011; 66: pages 417-434 2. Annual Meeting Newmarket

2016

The East Anglian Association of Anaesthetists Core Topics & Harold Youngman Prize 2016 will take place on Thursday 10th and Friday 11th November. The meeting venue will remain the Conference Facilities at Tattersalls, Newmarket. The educational programme for 2016 has been designed around the feedback received at the 2015 meeting, particularly in relation to format and content. On the first afternoon of the meeting there will be a new focused clinical update session addressing core areas of CPD for the general anaesthetist such as obstetrics, an update on hip fracture anaesthesia (SNAP2) and anaesthesia for amputation. The meeting organisers are Drs Claire Williams and Familia Alagarsamy, both from Addenbrooke’s Hospital.

In 2017, the Royal college of Anaesthetists will celebrate 25 years since gaining its Royal Charter. The East Anglian Association of Anaesthetists meeting has competitively won an RCoA Silver Jubilee Bursary for the 2017 meeting and this promises to be a special event. Quite how the Bursary will be spent to support the meeting is currently under review. The President of the RCoA, Dr Liam Brennan, will open the meeting.
