Introduction This is our draft Sustainability and Transformation Plan (STP) for Cornwall and the Isles of Scilly

The Sustainability and Transformation Plan (STP) is a plan for how health and social care will work together over the next 5 years.

The plan has been written by health and social care organisations in Cornwall and the Isles of Scilly.

We also need to involve all the voluntary and private organisations, and the people who use the services.

The NHS say our plans must:

1. Improve the health and wellbeing of the local people

2. Improve the quality of local health and care services

3. Make the services good value for money so that we can keep running them for a long time

In Cornwall and the Isles of Scilly, all of the health and care organisations are committed to achieving these three aims.

We want to know what you think about these draft plans.

Please tell us before Friday 20th January.

We will take your views into account when we make the final plans.

Making our plan The proposals have been made by local health and care organisations including:

• NHS Kernow Clinical Commissioning Group

• Cornwall Council

• Cornwall Partnership NHS Foundation Trust

• The Council of the Isles of Scilly

• Kernow Community Interest Company

• Royal Cornwall Hospitals NHS Trust

• NHS England

Keeping healthy Many people have an unhealthy lifestyle. The way they live causes ill health, disability and early death.

The main problems are:

• Drinking too much alcohol

• Smoking

• Not keeping fit

• Eating the wrong food and getting overweight

• Loneliness (feeling alone and not having company)

People would live healthier lives and need the doctor less if they changed some of these things.

£1 of every £5 that the NHS spends is because of these lifestyle problems.

We have also become used to going to hospitals rather than getting treatment at home.

Getting treatment at home means:

• People can be more in control of their treatment

• People can remain in control of their lives

• The NHS saves money

There are other things that can affect people’s health and wellbeing like:

• Housing

• Employment

• Education

Why should we change? We need to change the way we give people health and social care because:

• Our current health and social care services are not meeting people’s needs

• Some services are good, but some are not. We want to make all the services good

• There are some services that are doing the same thing. This wastes money. Our changes will save money

• These plans will help people to care for themselves which will stop people getting ill

The changes We have 6 ways that we want to improve our health and social care services:

1. Helping people to live healthier lives

2. Getting Health Services and Social Care services to work together better

3. Focus on urgent and emergency care

4. Managing people with special or long lasting health problems

5. Improving how we work

6. Changing the way the system works

1. Helping people to live healthier lives

People told us they want:

• Help to live healthier lives

• Help to keep fit

• Better use of technology

• Support in the community

Why we need to change In Cornwall and the Isles of Scilly we need to:

• Reduce the number of people who are overweight

• Reduce the number of people that smoke

• Stop older people feeling lonely

• Support people living in poverty and can’t afford to stay warm at home

• Support people with mental health problems to stay safe and well

People need to do more to look after themselves.

By spending at least £20 million over the next 5 years we believe that we can save £17 million from the budget by 2020/2021.

We think we need to focus on these things:• Working with pregnant mums who

are smoking

• Doing something about the number of children who are overweight

• Increasing the number of children getting their injections

• Working with schools to increase the amount of exercise that children get

• Protect children from violence at home

• Better support for people with mental health problems and long term conditions.

• More affordable housing and insulated homes to keep people warm and well.

• Better use of technology and self-care to help people help themselves.

• Teach people to use the chemist for small health problems like colds, so that GPs can spend more time on serious illness.

How will we know it is working? We will know we are doing well when we see:

• Healthier babies

• Fewer pregnant women smoking

• Fewer households in poverty

• Fewer overweight children aged 10 or 11

• Fewer people going to hospital with problems caused by smoking or alcohol

2. Getting Health Servicesand Social Care services to work together better

People told us they want:

• To get health and care services quickly and easily

• Specialist services to work together under one roof in community hubs

A community hub is a local place where you can get assessments and treatment from different health care professionals all in the same place.

• Health services to work together in an organised way

• Improved housing, particularly for older people.

Why we need to change There are too many older people in hospital who don’t need to be there

This problem will get worse because we will have more older people needing more help from the NHS.

We want to improve the way we look after older people and other vulnerable people and find a way to save money.

We think we need to focus on these things:• Spend more money on community

services so that fewer people need to go to hospital

• Work with carers and volunteers in the community

• Pharmacists and nurses to do more so local doctors (GPs) focus on more serious work.

• Change some community hospitals into community hubs -maybe on fewer sites.

• Work to catch health problems early - before they get worse

• Manage long lasting health problems using technology

• Provide more housing with care and support for independent living

If our community services work together better we think we can save £24 million by 2020/2021.

3. Focus on Urgent and Emergency Care

People told us they want:

• Reliable services that are always good

• Services that are easy to contact

• Enough staff to support people when they need it

• Clear information on what services are available, where and when.

• Improvements in NHS 111 to address overuse of the Emergency Department.

Why we need to change More people are using urgent and emergency care than ever before.

Sometimes, this isn’t the best place for people to go with their health issues.

The problem is:

• People don’t understand what health services do what

• Some services aren’t open at the right times

We think we need to focus on these things:• Better NHS 111 and out of hours

service

• Help people understand what different services can help

• More Urgent Care Centres in the right places

• Maybe close some Minor Injury Units so we can save money and make better urgent care centres

• Change the way we work at Emergency Departments – so people are seen by the right professional quicker.

Royal Cornwall Hospital in Truro and Derriford Hospital in Plymouth will continue to treat the majority of emergency and trauma patients in our region.

We think these changes can save £2 million by 2020/2012

4. Managing people withspecial or long lastinghealth problems

People told us they want:

• More focus on preventing ill health and supporting people to control their own lives

• Better use of technology to manage health conditions and share information

Why we need to change There are more people with long term conditions like diabetes, heart disease and cancer in our area than other parts of the UK.

Technology can help people monitor their conditions so that they don’t need to go to specialist clinics as often. We think we need to focus on these things:• Review our specialised services

and treatment for long lasting conditions

• Offer therapy to people with certain long lasting conditions. This might stop people needing operations, or staying in hospital for a long time.

• Better use of technology to monitor and treat conditions such as diabetes.

• Offer mental health care closer to home

We think these plans would save £22 million from the budget by 2020/2021

5. Improving how we work People told us they want:

• Health and social care services to share information more

• Health and social care services to be able to see people’s health records

• Information about how money is spent

• To reduce the money spent on management and unnecessary services.

Why we need to change Working together, with better technology to support staff, we will improve services and save money

We think these changes will save £7 million from the budget by 2020/21.

We think we need to focus on these things:

• Reduce the support teams

• Share office staff across different services

• Get better at buying goods and services

• Work together to employ and support new staff

• Look at our buildings and offices to make the best use of what we have

6. Changing the way system works

People told us they want:

• Involve patients in planning services

• Cut the number of times you are passed on to another health worker.

• Services that work together

Why we need to change We need to make sure all the the services we fund, work together.

We think our plans can save over £150 million in the budget by 2020/21.

Most of these savings will be from changing the way we work together as organisations.

We think we need to focus on: Getting all health services to look for new ways to work together and the way they organise to provide excellent services.

We will know our plans areworking well if we see: • Less patients having to go to

different organisations.

• Cheaper care packages

• Less delays in getting people hospital beds

• Less money spent on medicines

• Fewer people going into hospital
