Scope Testing & Commisioning include Hydro Test, Visual Test, Radiography and Pree Welding Heat Treatment (PWHT). Routinely actvity, check, review and validate the discipline scope of work against the appropriate Contract Requisition to ensure the Contractors correctly interpret and comply with the latest revision AFC drawings, quantities, and specifications. Ensure all quality control checks and tests are instigated, with records maintained and available as required for underground metallic and non-metallic checks, pre and post test checks, hydrostatic testing, flushing and drying, safety relief valve installation etc., all in accordance with contractors’ agreed ITP, method statements and project specifications.

Identify the hand over sequence on a system by system, area by area basis, all scheduled to produce logically planned work completions for commissioning and start-up requirements. In conjunction with the Commissioning Manager ensure pre-commissioning requirements are undertaken in accordance with project specifications and procedures, and coordinated with ongoing construction activities.

Scope Maintenance includes routine and non routine. Maintenance Routine is carried out every day of the examination Utilities against damage, corrosive (tickness), data retrieval performance, lubrication, sand blasting, painting and coating. Maintenance Non-routine is emergancy works related to the production, implementation should be timely with completion as soon as possible, so that requires a shut down. Isolation Plan Instead of installation work, all hot work and welding activities of construction/fabrication will be performed outside CGS-5 area (shop fabrication). Individual equipment shut down is required during installation and shall follow LOTO and isolation plan drawing as attached.

Inspection PlanTo ensure that the construction work is safety and efficiently carried out, and to ensure good coordination with CPI Operations, CPI will contractors. For tank inspection, the contractor shall perfome wall thickness measurement by UT for roof and shell, by mfl for bottom, settelment, roundness, recommendation drawing, etc as per API 653 and MIGAS certification requirement. repair should be completed prior to execution.- This repair shall involve facility owner, and shall keep good communication among project executor, project

coordinator and facility owner,to make sure that the job will be executed in timely and safety manner.- The contractor will prepare the manpower requirement for the scope mentioned above, and also provide

tools and equipment.- Please note that the scope of repair will be refer to inspection result. The scope in thia document is

developed based on internal inspection and experience from the previous Tank Cleaning Project.Scope of tank cleaning is to remove oily waste, sludge, and clean the tank to the level that an internal inspection using MFL (Magnetic Flux Leakage) can be performed. This requires that no sludge, oil, slippery surface, or other product residue left on the bottom plat of the tank. For tank cleaning, it shall consider the following:

- Less exposure to portential risk, especially for tank cleaning crew.- No man entry and work in a closed loop system hall be pursued. When it is not avaible, a tight procedure for

safe tank entry must be applied, including safety for hazardous materials exposure.- Disposal of hardous waste, consider using SFI facility. Temporary containment before sending to SFI will use

existing API pit or slurry tank.The scope of repair will be based on the inspection result that may consist of: Repair bottom plates, including patching, and/or bottom plates replacement. The fix scope shall refer to internal

inspection result which will be conducted after tank cleaning completes. Repair roof plates, including patching, and or/roof plates replacement. The fix scope shall refer to internal

inspection result which will be conducted after tank cleaning completes. Repair shell plates, including patching, and shell replacement. The fix scope shall refer to internal inspection

result which will be conducted after tank cleaning completes. Internal coating for roof, bottom plates and shell. - External coating for tank shell and roof.

Storage tanks repair will involve some uncertainties, and the scope will be developed based on inspection findings which may include:

- Rafter replacement, - Tank Columns,- Tank Internal, e.g.,spreader box, - Tank appurtenance,- Sampling box repair (separated from tank shell), - Water leg box repair - Safety in Design items.- Scaffolding installation (internal and or/ external) based on the repair and coating work requirement- Non Destructive Examination (NDE)- Overflow line installation(separated MOC)- Emergency vent installation(separated MOC)- Additional sampling point installation (separated MOC)- Platform fabrication and installation (separated MOC)

Complete repair scope will be developed after tank inspection completed and the recommendation based on the tank inspection result received. Certification completion

After completion of tank repair, the tank shall be certified as per Migas regulation. The cetified shall be done by current “Perusahaan Jasa Inspeksi Teknis (PJIT)”appointed as Certification Company under contract no. 7337-OK. The document that shall be provided for repaired tank certification:- Out of service tank inspection report including thickness measurement, settlement, grounding, roundness,

plumbness, etc.- Material certificate - WPS/PQR- Welding test calibration document and specification. - Welder certificate- Welding test procedures,report, and inspector certificate.

CGS-3 Waste Water Line Pipe Support Modification, Recommended Action: Modify the existing pipe support at node 500 (16” existing waste water line pipe support) by bracing it to new

concrete pipe footing. Previously proposed to install new pipe supports (pilling), however there is safety concern from IM (Construction Division) about the hazard of pilling in location which are surrounded by many facilities.Instll new pipe guide into the existing pipe support. Please do soil syrvey by digging the soil to investigate the pipe support foundation condition.

Install one pipe guide into node 160 – 14” existing waste waterline pipe support, Install one anchor into node 360 – 14” existing waste water line pipe support, Install one pipe guide into node 400 – 14”existing waste water line pipe support, Install one pipe guide into node 450-14”existing waste water line pipe support, Install one pipe guide into node 530-16” existing waste water line pipe support, Construction scope work will consist of- Concrete foundation of fabrication and installation, - Fabrication and installation of pipe supports,- Installation of several pipe guides and anchor into existing pipe supports.- Nondestructive examination (X-ray, UT, etc) and hydrostatic testing of the line are needed to be conducted

(currently the line has been shut down,by the time the consruction completed, there is possibility the waste water line leaks due to long periods of shut down.

Safety regulation :- Wear standard Personal Protective Equipment (PPE).- All personnel should minimize their exposure to the open launcher barrel.- Never assume that a launcher barrel is depressurized, as pressure can accumulate over time.- Never attempt to open the barrel unless it is certain that it is at atmospheric pressure.- Always check that the launcher barrel is completely depressurized before draining to the drain/vent K.O.

vessel.- Use a trolley to assist on the insertion or removal of the pig where possible.- Follow manual handling requirement to avoid injuries.
