Uses of Eltroxin Tablets

Eltroxin tablets are used as a hormone replacement in patients of any age or state (including pregnancy) with hypothyroidism of any etiology except transient hypothyroidism as well as for simple (nonendemic) goiter and chronic lymphocytic (Hashimoto's) thyroiditis. Eltroxin (Levothyroxine Sodium) is also used as a pituitary TSH suppressant in the treatment or prevention of various types of euthyroid goiters.

Eltroxin (Levothyroxine Sodium tablets ) has to be stored at controlled room temperature i.e. from 20C to 25C (68F to 77F) with excursions permitted between 15 to 30C (59 to 86F). Store away from light and moisture.Keep this as well as any other medicines away from children and pets.

Eltroxin Tablets must be swallowed whole, and taken with a full glass of water. Levothyroxine tablets should not be split and must preferably be taken on an empty stomach.

The recommended initial dosage for adults is 50 to 100 micrograms daily and adjusted at four or six week intervals by 50 micrograms until attainment of clinical and biochemical euthyroidism.

For infants with congenital hypothyroidism a suitable starting dose is 25 micrograms Levothyroxine Sodium given as 50 micrograms every other day is advisable. This may be slowly increased by increments of 25 micrograms every two to four weeks until optimal response is achieved

The same dosing regimen applies to juvenile myxoedema, except that the starting dose for children older than one year may be 2.5 to 5 micrograms/kg/day. The calculated daily dose equivalent should be rounded to the nearest 25 micrograms to determine the actual prescribed dose.

Eltroxin (Levothyroxine Sodium tablets ) should not be used for the treatment of obesity or for weight loss. In euthyroid patients, Eltroxin doses within the range of daily hormonal requirements are NOT effective for weight loss.

Contraindications

Eltroxin is contraindicated in patients with a hypersensitivity to Levothyroxine Sodium or any other component of this medication. Levothyroxine tablets are contraindicated in patients with untreated subclinical (suppressed serum TSH level with normal T and T levels) or overt thyrotoxicosis of any etiology as well as in patients with acute myocardial infarction. Levothyroxine is also contraindicated in patients with uncorrected primary or secondary adrenal insufficiency. Also contraindicated in cases of acute myocardial infarction uncomplicated by hypothyroidism.

Cardiovascular: Chest pain, tachycardia, cardiac arrhythmias, palpitations, angina, myocardial ischaemia, myocardial infarction, heart failure, death.

Nervous system: Irritability, anxiety, nervousness, excitability, restlessness, tremors, headache, poor concentration, emotional lability, sleep disturbance, insomnia, mania, psychosis, psychotic depression, seizures, petit mal status epilepticus, pseudotumour cerebri (especially in children).

Gastrointestinal system: Diarrhoea, vomiting, malabsorption.

Skin: Warmth, erythema, telangectiasia, hyperhydrosis, alopecia, hyperpigmentation.

The following Eltroxin side effects are indicative of excessive dosage and usually disappear on reduction of dosage or withdrawal of treatment for a few days.

Respiratory system: Increased minute ventilation, tachypnoea, and shortness of breath.

Neuromuscular system: Myopathy, lid lag, muscle weakness and cramps.

Reproductive system: Amenorrhoea, menstrual irregularities, decreased libido, gynaecomastia (in male).

Metabolic: Fever, glucose intolerance, weight loss, premature craniosynostosis (in children) TRH suppression, heat intolerance, sweating, flushing.

The following Eltroxin side effects are indicative of excessive dosage and usually disappear on reduction of dosage or withdrawal of treatment for a few days.

Eltroxin Overdose

Symptoms of Eltroxin overdose can include the following: chest pain (angina), tachycardia or arrythmia, muscle cramps, headache, restlessness, flushing, sweating, diarrhoea, tremor, insomnia and hyperpyrexia. These signs can take upto 5 days to appear. Atrial fibrillation may develop. Convulsions occurred in one child. There may be increased toxicity in those with pre-existing heart disease.

Give oral activated charcoal if more than 10 mg of Eltroxin has been ingested by an adult or more than 5 mg by a child, within 1 hour. If more than 10 mg has been ingested by an adult or more than 5 mg by a child, take blood 6 hours to 12 hours after ingestion for measurement of the free thyroxine concentration. Patients with normal free thyroxine concentrations do not require follow up.

Eltroxin During Pregnancy

Eltroxin (Levothyroxine Sodium tablets) has been classified by the US FDA as Pregnancy Category A. Controlled studies in pregnant women treated with oral Levothyroxine Sodium (Eltroxin tablets) to maintain a euthyroid state DO NOT demonstrate a risk to the foetus in the 1st trimester and there is no evidence of a risk in later trimesters. Hence the possibility of foetal harm remains remote.
