Overview of the Premera Wellness Program

Research has confirmed that physical activity is the single most effective behavior to inspire wellness—including better productivity, improved efficiency, and heightened alertness and decision-making. This means that activity can help reduce overall healthcare costs.

This program moves beyond creating awareness about one’s health. It is designed to foster in your employees a healthful habit of being active. More physical activity usually leads to better health, a sense of well-being, and greater productivity. And it helps you stem the rising trend of healthcare costs to your business.

The Wellness Program from Premera Blue Cross Blue Shield of Alaska motivates your employees* to get moving and rewards them for it. And when you encourage your employees to participate, we reward your business with a discounted renewal rate as well.

The Premera Wellness Program is designed to help employers create a culture of worksite well-being. Premera provides a pre-packaged activity challenge that helps employers encourage employees to engage in physical activity.

COMPONENTS OF THE WELLNESS PROGRAMThe program package comes with tools and resources to help you administer the activity challenge. You will be equipped with ready-to-use emails, brochures, video, posters, and flyers to help communicate the program to your eligible employees.

We offer Fitbit** units that track physical activity in the form of steps, stairs, distance, and calories burned.

In addition, we offer an activity challenge platform through EveryMove,** which serves as the online tracking mechanism for employees participating in the challenge. On EveryMove, participants can earn points for reporting physical activity, invite friends, and check to see how their efforts are stacking up against their challenge goal.

EMPLOYER RESPONSIBILITYThe role of the employer is to inform and guide employees through the program benefit using the ready-made communication tools available. If you have questions, feedback, or comments, contact your Premera account manager or wellness client manager. Employers need only focus on encouraging employees to participate. Ready, set, go!

* Active employees and former employees enrolled on COBRA are eligible to participate if enrolled on the medical plan by month 4 of the plan year.

** EveryMove and Fitbit are independent companies that provide fitness tracking products and services. By creating EveryMove and FitBit accounts, participants agree to their respective privacy policies. Participant information on the EveryMove and FitBit portals will not be subject to participants’ health plan privacy policies or HIPAA. EveryMove and FitBit may share participant information. You can review their privacy policies here: everymove.org/privacy and fitbit.com/privacy. Premera uses participation data to administer the wellness programs, fulfill incentives, and for health plan administration. Premera does not share participation data with employers except anonymously to report participation levels, and when requested for tax purposes, to report which employees completed the program and earned the incentive.

Earn about 25 points a day to reach 750 points in a month.

A 25-point day might look like:

Play an instrument for 30 minutes + work out with weights for 30 minutes

Cook for 30 minutes + swim laps for 30 minutes

Garden for 1 hour + dance for 30 minutes

HERE IS HOW IT WORKS:1. One month prior to the challenge period, eligible employees will receive an offer from Premera for a free Fitbit Zip.

It’s theirs to keep regardless of whether or not they participate in and complete the EveryMove Activity Challenge.

2. To join the EveryMove Challenge, eligible employees must enter through a custom URL provided by Premera. They must also enter their unique employee ID code to verify their participation in the challenge and sync their Fitbit Zip.

3. Armed with their Fitbit Zip and registered on EveryMove, eligible employees can track their physical activity online or through the EveryMove mobile app during the two-month challenge period.

4. Start earning points! Employees can earn points for almost any kind of healthy move—from jogging, to gardening, to playing with the kids.

5. Eligible employees who achieve 750 points in at least one calendar month during the two-calendar-month challenge period will earn a Visa® gift card.* Plus they get to keep their Fitbit Zip.

Biometric Screening An alternative to the EveryMove Activity Challenge

Eligible employees who can’t or prefer not to participate in the EveryMove Activity Challenge can earn the reward by completing a biometric screening with their doctor.

Biometric screening results are valid only when the employee’s doctor faxes the biometric screening form to 855-351-6378 within the two-month challenge period.

Administration: Managing the Premera Wellness Program

Member communications are included within the emails for distribution by the employer.

Here is a more detailed description of how the EveryMove Activity Challenge works.

1. Two months before the beginning of the challenge period, you will receive an email welcoming you and your eligible employees to the program. Your email includes:

• A program guide

• An email to send to your eligible employees inviting them to participate in the program

ADMINISTERING THE EVERYMOVE ACTIVITY CHALLENGE2. One month before the beginning

of the challenge period, you will receive an email to distribute to your employees to remind them about the upcoming challenge and the instructions on how to order their Fitbit.

• Employees must use the URL provided in the email to access the Fitbit website.

• There they will be prompted to enter a promo code. This is the last four digits of their Premera health plan ID

number (not including person code/suffix), followed by their date of birth (MMDDYYYY), and their first and last initials (as shown on their Premera ID card) in the following format: 1234MMDDYYYYFL. For more detailed instructions, see promo code (Fitbit) or employee ID (EveryMove) in the FAQ.

• Each eligible employee will be able to order only one Fitbit device and can choose any of the available colors.

• We encourage employees to order the Fitbit right away to make sure they get it in time to start the activity challenge on the first of the following month. For delivery to Alaska addresses, delivery takes about 7–10 business days after the order is received.

• After employees receive their Fitbit, they should follow the instructions provided with their device to register and sync it on fitbit.com.

3. On the day the challenge period starts, you will receive an email with the custom URL employees must use to join the EveryMove Activity Challenge.

• Employees must access the challenge by using this custom URL to ensure their points are tracked under the program and they receive their incentive after reaching the goal.

• After they access this custom URL, employees must select Sign Up With Email and follow the onboarding prompts. For details, see EveryMove experience in this guide.

• Employees must enter their Employee ID (same as the Fitbit promo code, see FAQ) when prompted by EveryMove.

• During the onboarding process participants must sync their Fitbit device to their new EveryMove account. This will enable them to earn points via activity tracked on their Fitbit for their incentive.

4. After they register on EveryMove and sync their Fitbit device, participants can track their physical activity online or through the EveryMove mobile app during the two-month challenge period.

• Participants receive points for activities tracked on their Fitbit device. To ensure they receive points, they should wear their device at all times.

• Participants can also manually enter their activities into EveryMove by using the Log Activity button in the top right corner of the main newsfeed page.

5. Participants who achieve 750 points in at least one of the two calendar months in their challenge period will earn a Visa® gift card. Plus all participants get to keep their Fitbit Zip.

• Goal achievement will be assessed after each month of the challenge. Those who earn the incentive should expect to receive it within 8 weeks after reaching the goal.

• Only one Visa gift card will be awarded per eligible employee.

EMPLOYER PREMIUM CREDIT

For new groups: An employer premium credit is applied to your first year’s overall medical premium at your next renewal, based on your business agreeing to encourage your employees to participate in the wellness program.

For renewing groups: Groups with a participation rate of at least 50 percent will receive an employer premium credit on the overall medical premium at their next renewal.

Groups with can boost their employer premium credit by completing an Alaska Wellness Academy module by the end date of the engagement period.

For the exact amount of the employer premium credit, talk to your Premera account manager.

ADMINISTERING THE BIOMETRIC SCREENING ALTERNATIVEEmployees who can’t or prefer not to participate in the EveryMove Activity Challenge can earn the reward by completing a biometric screening with their doctor.

• Participants will need to print the biometric screening form.

• They should take this form to their appointment and have their doctor complete the form.

• To receive credit, both the provider’s and the participant’s signatures are required on the form.

• Biometric screening results are valid only when the biometric screening form is submitted within the two-month challenge period.

Employees are eligible to participate if enrolled on the medical plan by month 4 of the plan year.

* Benefits earned by employees participating in the Premera Wellness Program may need to be reported for tax purposes. After your participation in this wellness program, we will send you a list of the employees who participated and who received a debit card and/or FitBit® through the program. This information is provided to you for tax purposes only. Please consult your tax advisor to determine impacts to you and your organization.

FITBIT EXPERIENCE1. Participant orders Fitbit by using

the unique Fitbit URL provided in the employee email.

2. Participant enters Fitbit promo code by entering code comprising the last 4 digits of Premera health plan ID + birth date (MMDDYYYY) + first and last initials as shown on the Premera ID card. See Promo code (Fitbit) or employee ID (EveryMove) in the FAQ.

3. Fitbit verifies by checking the submitted promo code against the promo code eligibility file and then checking that this participant has not already ordered a Fitbit.

4. If verified by Fitbit, participant selects a device: Participant chooses one of the available colors, then clicks Add to Cart.

5. Participant enters address by entering the mailing address they want the device sent to, then clicking Next.

6. Participant verifies and completes order by reviewing the information and clicking Place Order.

7. Participant receives confirmation via email, including a number they can use to track their order.

8. Fitbit ships ordered device via U.S. Postal Service Standard by 5–7 business days after order date.

9. Participant receives Fitbit in 7-10 business days after shipment.

• To participate in this program, the participant MUST use the custom URL provided to order the Fitbit.

EVERYMOVE EXPERIENCE

1. Participant receives information from employer including a custom URL to join EveryMove Activity Challenge.

2. Participant must access this URL and select Sign Up With Email on home page.

3. Participant will be taken to screen to create an account

where they need to enter their name, preferred email, password, and date of birth, and then click Sign Up.

4. User follows onboarding prompts on screen, including prompts to verify their employer using their employee ID (same as the Fitbit promo code, see FAQ), and to sync their Fitbit or other device.

5. Participant lands in main user newsfeed page.

Participant is ready to start tracking activity via their device and/or manual entry and can view their progress toward the reward in the Reward screen.

A We want to give employers tools and resources to create a culture of worksite well-being. Because activity is the single most effective method to improve workforce productivity, efficiency, and well-being, this could have a major impact in lowering healthcare costs.

PARTICIPATION IN THE PREMERA WELLNESS PROGRAM

Q Can spouses or dependents participate in the Premera Wellness Program, or is it just for employees?

A No. Only employees are eligible to participate if enrolled on the medical plan by month 4 of the plan year.

Q How much will the wellness program cost to the employee?

A The wellness program is built into the medical plan. Therefore, there is no additional cost to the employee.

Q Will premiums increase for employees who do not participate in or complete the Premera Wellness Program?

A No, individual employee premiums are not negatively impacted by the outcome of the wellness program.

Q What is the employer premium credit?

VISA® GIFT CARD

Q What do employees have to do to receive the Visa gift card?

A They must earn 750 points in at least one calendar month of the two-calendar-month challenge period or complete a biometric screening with their doctor and fax the completed form to the number on the form.

Q How is the employee reward paid?

A The reward is in the form of a Visa gift card. The cards will arrive in a white envelope via United States Postal Service.

Q How and when do those who earn the required points get the Visa gift card?

A Premera assesses goal achievement in the activity challenge monthly. Those who achieve their goal receive the reward up to eight weeks after they reach the 750-point goal.

Q Do the Visa gift cards expire?

A Visa gift cards are valid for six months, beginning on the first day of the month following the month they are issued. The expiration dates are listed on the cards.

Q Can a lost Visa gift card be replaced?

A Yes, in certain cases they can be reissued:

• Visa gift cards can be reissued up to 60 days after expiration (60-day grace period).

• Cards past the 60-day grace period cannot be reissued.

• Reissued Visa gift cards are sent via first class mail within six weeks after the reissue request has been submitted to Premera.

Q Whom should employees contact for questions about their Visa gift card?

A Contacts:

• Cardholders can access information, such as transaction history and balance information, online or by calling the toll-free number printed on the Visa gift card 24/7/365.

• Cardholders who have not received their card should contact Premera customer service at 800-508-4722.

PROMO CODE (FITBIT) OR EMPLOYEE ID (EVERYMOVE)

Q What is the promo code (Fitbit) / employee ID (EveryMove)?

A Fitbit calls the number participants need to enter a “promo code.” EveryMove calls the same number an “Employee ID.” It’s the same number. Here’s how participants determine their promo code (Fitbit) or employee ID (EveryMove):

1. Enter the last four digits of the identification number from the Premera health plan ID card.

Example: If the plan number is 123456789, enter 6789.

2. Add eligible employee birthdate in the format MMDDYYYY.

Example: If birthday is May 30, 1984, enter 05301984.

3. Finally, eligible employee adds the initials of their first and last name as it appears on their Premera ID card. Example: If the eligible employee’s name is Mary Smith, enter MS.

In the example, the promo code or employee ID is 678905301984MS

HOW TO USE THE FITBIT ZIP

Q How do eligible employees get the free Fitbit Zip?

A One month before the beginning of the two-month challenge period, employers get a reminder email from Premera to distribute to employees. The email contains a link to the initial page on the Fitbit website, and instructions on how to order. After employees enter their promo code when prompted on the Fitbit website, they choose the color of Fitbit Zip they want and then place their order.

Q Does the eligible employee have to get a Fitbit Zip to participate in the EveryMove Activity Challenge? Can the eligible employee use any type of fitness tracker they may already own?

A Eligible employees are not required to get a Fitbit to participate. Physical activities can also be entered manually on the EveryMove platform and/or by using a different compatible device.

To verify which devices are compatible with EveryMove, participants can select “see other apps and devices” in the connect apps onboarding prompt or by going through “more” and then “apps and devices” once signed in to EveryMove.

The tracker the member decides to use needs to be synced with the EveryMove challenge. This is done under “apps and devices” after the tracker is selected.

Q How long does it take to get the Fitbit after ordering?

A The Fitbits are available to be ordered one month before the challenge period. (For dates, see Timing under Administration: Managing the Premera Wellness Program.) For delivery to Alaska addresses, delivery takes about 7–10 business days after the order is received. To receive the Fitbit in time to get started on the activity challenge the first of the following month, we suggest ordering the Fitbit as soon as possible.

Q What should a person do if they’re having issues with their FitBit?

A Please contact Fitbit for help with delivery, troubleshooting, and servicing the devices.

Q Is it safe to swim or shower with the Fitbit tracker?

A No, do not swim or shower with the tracker.

Q What if the participant loses the Fitbit? What if it breaks?

A Each participant will be offered only one Fitbit device. Participants are responsible for replacing lost or broken Fitbit devices. For help with lost or broken devices, contact Customer Support at Fitbit.

EVERYMOVE ACTIVITY CHALLENGE

Q How are points earned on EveryMove?

A Points are automatically earned from the steps recorded on the participant’s Fitbit, if their Fitbit is synced to EveryMove. Additionally, participants can earn points for almost any kind of healthy move—from jogging, to gardening, to playing with the kids.

To add activities:

• Enter healthy activities on the EveryMove home page.

• Walk a certain number of steps per day, as measured by linking a device such as a

FitBit or another pedometer or tracking device.

• Check in at a healthy place like the gym or a 5k event via Facebook or Foursquare.

• Invite friends to EveryMove and get active in the community. Participants can earn points by “spreading the love” when friends are active.

Q How do participants earn 750 points in the EveryMove Activity Challenge?

A Those who do the doctor recommended 30 minutes of activity 3 times a week can easily get 750 points in a month.

For example:

• Running 3 times a week for 30 minutes would earn 860 points in a month.

• Yoga 3 times a week for an hour would earn 840 points in a month.

• Biking 3 times a week for an hour would earn 1,040 points in a month.

• Lap swimming 2 times a week for an hour would earn 784 points.

Even participants who don’t regularly do this sort of exercise can get 750 points.

For example, doing the following every week would also earn 750 points in a month:

• Run once a week for 30 minutes, do 1 hour of housework, play with children 1.5 hours, and take 5,000 steps 5 days/week

• Bike twice a week and do 2.5 hours of yard work during the month

• Lap swim once a week, do yoga once a week, and an hour of housework

In other words, those who regularly do something active should be well on the way to 750 points a month. They can also get extra points for active days and points for doing things with another EveryMove friend, which all add up.

Q What if the participant does not reach 750 points in the first month?

A There are two months in the challenge period. Those who miss the first month can try again the second month.

Q Do points roll over to the next month?

A No. Each participant starts from zero points at the start of the second month.

Q Does every activity earn points?

A Not quite every activity. There are a few cases where a logged or synced activity on EveryMove doesn’t award any points.

If EveryMove does not award points for an activity, it’s probably because:

• We’ve seen it before. This usually happens if participants use multiple apps or devices to track their workouts—logging a bike ride on both Strava and MapMyRide, for example. In these cases, only one instance of the activity will generally score points.

• It’s a distant memory. EveryMove awards points for activities only if they were completed and recorded within the last three days (72 hours.)

• You’re just getting started! A single push-up or a walk to the water cooler won’t meet the minimum threshold for granting points. And because many devices roll up activity across longer periods of time, those minimums can be a little higher. For instance, before points start rolling in, the user needs to record more than 5,000 steps a day on a pedometer connection.

• Not all places are created equal. EveryMove awards points for location check-ins only if they’re on the EveryMove list of health or fitness minded places. Parks, gyms, and pools are good bets, but casinos and airports won’t give points on EveryMove.

Q How many points is an activity worth?

A EveryMove’s points system reflects both the intensity of an activity and the amount of time spent doing it. So riding a bike for an hour earns more points than walking for an hour, but fewer points than an hour-long boot camp. Double the time doesn’t always mean double the points, though, so a 14-mile run isn’t necessarily worth twice as many points as a 7-mile one.

Why not? Well, it’s complicated, but basically studies show that “more pain, more gain” isn’t necessarily true when it comes to health. In fact, regular exercise of moderate intensity is great for the heart and lowers risk for many diseases a lot more than, say, bench pressing 350 pounds or running a 5-minute mile. And even heart-healthy activities like jogging and biking have a point of diminishing return where additional effort doesn’t have much of a payoff.

Q How often should program participants sync their Fitbit with EveryMove?

A Daily syncs are recommended by Fitbit.

Q How do users know how many points have been earned?

A EveryMove automatically calculates, tracks, and displays how many points a member has earned. Points come from activity automatically uploaded from a synced Fitbit and from the self-reported activity on EveryMove. If you’re not seeing your points, make sure you have verified your employer.

Q What is an Active Day bonus?

A An Active Day bonus is a points-oriented pat on the back for really breaking a sweat. There are two ways to earn an Active Day Bonus. The first is within the day. For example, biking to work and swimming a mile and walking the dog on the same day could score an Active Day bonus for that day. It’s possible to earn these bonuses every day depending on how much the person does.

Second, those who have more than one Active Day a week get even more bonus points for staying consistent. This type of Active Day bonus can be earned up to three times a week.

Q What is Spread the Love?

A For every Active Day, participants get 15 bonus points to share with three friends. Spread the Love points don’t come out of the participant’s total. Participants can use them to show an EveryMove friend they’re doing a great job or better yet, nudge someone who has fallen off the fitness wagon.

Q For the EveryMove Activity Challenge, how does the participant prove completion?

A Completion is tracked in the EveryMove platform based on whether they reach the activity points goal in one of the designated months for their renewal date. The EM admin portal will obtain a report that

Q How can participants sign up for the EveryMove activity challenge (or sign in after their account is set up) if they don’t sign up through their employer’s custom URL/landing page?

A Participants who do not sign up / sign in through the custom landing page still need to select and verify their employer by using the Membership page on their profile:

1. In the top navigation space on the main news feed page, select Memberships under More.

2. Expand the employer section by clicking the check mark.

3. Type in the employer and select OK.

4. To verify employer, use the data points determined on the eligibility file, and then select Verify employment.

ALTERNATIVE BIOMETRIC SCREENING

Q How do participants complete their biometric screening?

A Participants who can’t or prefer not to take the EveryMove Activity Challenge can earn the Visa gift card by completing a biometric screening with their doctor:

1. Download and print the biometric screening form.

2. Take the form to the appointment where screenings are performed, including body mass index (BMI), heart rate, blood glucose, and cholesterol tests.

3. Both participant and provider must sign the form.

4. Participant or provider must fax the signed, completed form to the number on the form within the two-month challenge period.

Q Will the biometric screening be covered by the health plan?

A The biometric screening will be covered as a preventive exam by the plan as long as the physician tests only what is asked for on the form. Any additional blood tests conducted by the physician may result in an expense to the member.

Q What if the participant had blood work completed by their doctor earlier in the plan year?

A Biometric screening results and blood work results from the current plan year will be accepted as long as they are submitted on the correct form during the two-month challenge period.

Q What is Premera’s privacy policy?

A Premera uses employees’ completion data to administer the wellness program and fulfill incentives. Premera does not share personal data with employers except anonymously to report participation levels, and when requested for tax purposes, to report which employees completed the program and earned the incentive.

Q What kind of information will a group receive?

A Groups will receive information in aggregate form. Information will not disclose, nor will it be reasonably likely to disclose, the identity of specific individuals, except as is necessary to administer the plan.

Q What type of information does EveryMove and Fitbit share with others?

A EveryMove recognizes that a user’s fitness habits are personal information that they want to be able to control. To help in this effort, EveryMove has made it so only the user’s friends can see their photos, posts, and comments.

A user can always opt out of leaderboards or sharing their info with employers, insurers, or any other organization—but that may cost the user eligibility for some

rewards. Lastly, EveryMove guarantees that the information a user provides will never, ever affect their premiums or ability to partake in other promotions. See the EveryMove privacy policy for more details.

By using EveryMove or Fitbit, a user agrees to their privacy policies (EveryMove privacy policy, Fitbit privacy policy). The user’s information on EveryMove or Fitbit will not be subject to their health plan’s privacy policies. EveryMove and Fitbit may share their users’ information.

Q Who all will see the EveryMove user’s points and rankings?

A Other participants in the activity challenge will see the user’s first name and last initial as well as the user’s points. Those on the user’s friends list will see the user’s full name and points.

Q How can participants in the EveryMove Activity Challenge protect the privacy of their information?

A The user’s privacy is paramount. So EveryMove keeps the user 100 percent in control of the user’s own personal information. The user decides what information to share and with whom, and what to keep private. And of course, the user can change privacy settings any time.

A The Premera Wellness Program is designed to be compliant with HIPAA non-discrimination requirements. The program is voluntary and employers cannot require their employees to participate or ask if they did so.

Q Does the program comply with the Americans with Disabilities Act (ADA)?

A The program is designed to comply with ADA requirements. For example:

• The program does not discriminate on the basis of disability in regard to incentive and other terms.

• Through the biometric screening option, the program makes reasonable accommodation for employees for whom it is medically unreasonable to participate in an activity-based program.

• The program provides all employees equal access to benefits.

• The gift card incentive does not exceed the maximum allowable incentive.
