Job Description Job Title Project Worker Operational Responsibiliti es (Senior Management roles only) None Working Hours 37 hours per week Contract Fixed Term – 31 st March 2020 Reports to Project Manager / Head of Energy Services Budget Accountability None ENERGY ADVISOR Role Overview This is a key role for Severn Wye set partly within the Onward Care Team working in a hospital environment to support the successful discharge of those patients who are medically stable and able to return home. The emphasis is to ensure that every opportunity is taken to access the Warm Homes Fund on behalf of patients and that the uptake is high. Patients will need hand-holding through the process and successful candidates will need to be able to manage a number of key communication channels liaising with housing services, the patient’s family and contractors to ensure the process is as easy and as seamless for the client as possible. Key Responsibilities Provide information and advice to patients about accessing the Warm Homes Fund Helping patients understand the eligibility criteria and make application for the Warm Homes Fund Liaising with other hospital staff and support staff within the Onward Care Team to ensure the neds of the patient are met Liaising with housing services and family members to arrange access to the patients property for survey if they are successful in their application and where this isn’t possible prior to the patient discharge that this activity is undertaken when the patient has returned home Page 1 of 3

Develop relationships with district, borough and county housing services staff as well as with housing services staff within Registered Social Landlords

Have a proven track record of self-management and be an independent self-starter Work with the Warm & Well Project Manager and Partnership Coordinator as well as other

Severn Wye staff to ensure links between the service and the wider stakeholder community and health stakeholder group

To provide reports on activity and feedback to the management teams at Severn Wye as well as wider external partners like the Warm Homes Fund, the Clinical Commissioning Group and the Warm & Well Steering Group

GENERAL DUTIES Liaise with Onward Care Team to identify suitable clients Provide support and guidance to vulnerable clients using sensitivity and a customer first

approach to ensure clients understand the benefits of the service Work externally with health and care professionals working in the community to build up a

network of trusted partners Communicate effectively and work collaboratively with other members of the Severn Wye

team working in the same areas Take a pro-active approach to developing the service using initiative and determination to

implant a new service within an already busy and demanding environment Follow all organisational policies and procedures, and assists in ensuring that these are

applied consistently across the organisation and the external partners with which they are working

Required Knowledge, Skills and Experience

Essential:Qualifications or Experience in

A relevant discipline e.g. community, health, housing and energy or similar.

Experience working in the community, health, housing or energy sector Experience of building and maintaining productive relationships with internal and external

stakeholders and partners Supporting the development and implementation of changes in peoples living standards or

lifestyle Leading consultative customer exercises and balancing the priorities of all parties to reach

the optimum outcome (patient, family, housing officers and contractors) Ability to manage expectations and tensions including having difficult or challenging

conversations Ability to make rational and logical decisions in a timely manner and communicate them

clearly Computer literate: Windows, Word, Excel, PowerPoint, e-mail, internet, database. Driving licence and own car to use

Desirable:

Good understanding of housing issues, including types of housing and key legislation Experience of working in the NHS and/or working with vulnerable people Good general knowledge of energy infrastructure in UK Experience of working in, or in support of, local or other statutory authorities
