Service Suppliers

Strongly encourage that the individual providing the information be knowledgeable of OSHA SafetyProcesses and Requirements Current active contract in place to provide services Potential desire to bid on and/or provide services with Entergy

Registration is required prior to submitting any new bids or signing new contracts

Receive Invitation Submit Registration Fee Update Company and Contact Information through registration process Select a NAICS code

Upload Third-Party Certification Form or Self-Certification Form (if qualified)

Self-Certification Form is a temporary placeholder valid for one year while waiting for third-partycertification to be completed.

CVM Reviews documentation and updates supplier status as Certified Entergy qualifies diverse materials supplier

Diverse Services Process Receive Invitation Submit Registration Fee Update Company and Contact Information through registration process Upload Third-Party Certification Form or Self-Certification Form (if qualified)

Supplier Process Checklist:

Required Documentation for Upload into CVM SystemCurrent Third-Party Diverse Certification (diverse suppliers only)

For qualified Suppliers, Diverse Self-Certification Form is acceptable on an annual basis If providing a Diverse Self-Certification, a Third-Party Diverse certification is strongly encouraged

OSHA 300A Summary and 300 Logs for previous Past Three Years (services suppliers only) If not required by law to provide OSHA Reports, OSHA 300A Waiver is required

EMR(Experience Modification Rate) for previous three years (services suppliers only) EMR information provided and documentation must match or application will be rejected. For non-qualifiers, provide letter from insurance company with reason why company does not qualify

for EMR rating.If incorrect information is entered into the system and the account becomes locked, contact EntergysProcurement Analyst.Current Certificate of Liability Insurance (services suppliers only) required only prior to signing a contracInsurance is required immediately if already under contract and doing business with Entergy.

Vendors will provide the highest between contract agreements insurance requirements or the minimumrequirements below General Liability minimum $1M Worker's Compensation minimum $1M Auto Insurance minimum $1M Umbrella $4M or difference between $5M and General Limit Liability Amount Form must show as Certificate Holder:Entergy and its Affiliated & Associated Companies

information.
