Introduction

Welcome to the East Valley Institute of Technology (EVIT) Adult Education Center’s Practical Nursing Program! Students enrolled in the EVIT Practical Nursing Program have selected an intense and demanding nursing program. Students will be held to the high standards and expectations of the EVIT academic heritage of excellence. The administration, instructors, staff, and support personnel welcome you to this opportunity and wish you great success in achieving your goals. Please do not hesitate to ask for assistance if you have a question or concern.

The information contained in this handbook is intended to accompany, augment, and compliment the information outlined in the EVIT Adult Education Handbook and Catalog. It is imperative that both are understood in their entirety. Policies that apply specifically to the Practical Nursing Program (PNP) may differ from general Adult Education policies. In such cases, the policies detailed for the Practical Nursing Handbook will take precedence.

The EVIT PNP has compiled the information contained within this handbook in order to provide access to important policies and procedures for the Program. The Practical Nursing student is responsible for knowing and abiding by these policies and procedures. The student will be expected to retain a copy of the handbook throughout the program. All students will formally acknowledge the receipt of this compilation by returning the completed and signed verification of receipt form.

Nursing Program Philosophy & Overview

PurposeThe purpose of the EVIT Practical Nursing Program is to prepare students to become a Licensed Practical Nurse by providing a comprehensive course of study that combines theory with clinical practice. The curriculum is designed to provide qualified individuals an opportunity to acquire the knowledge, attitude and skills that will enable them to become safe and competent practitioners of practical nursing.

MissionEVIT’s mission is to provide a productive, technically trained workforce that meets the market driven needs of business and industry. In addition to the EVIT mission, the EVIT Practical Nursing Program has a mission to provide competent, employable nurses that meet the health care needs of the community in a variety of settings.

Program Goals

Upon completion of the EVIT Practical Nursing Program, the certificate holder will be prepared

to:

1. Demonstrate professional behaviors according to the legal and ethical standards of the practical nursing scope of practice and standards of care.

2. Incorporate effective communications with clients, families and the healthcare team by use of informatics, interpersonal and therapeutic communications skills.

3. Participate as part of an interdisciplinary team in the data collection, planning, implementation, and evaluation of safe effective care for healthcare consumers across the lifespan.

4. Demonstrate a compassionate approach to safe, appropriate care for each client, according to their needs and the practical nursing standards of care.

5. Prioritize care for clients and delegate appropriate aspects of that care to unlicensed assistive personnel.

6. Utilize training and job skills necessary to obtain and maintain employment in a competitive market.

Program Quality and Evaluation

Quality instruction, service, and student outcomes are very important to EVIT Practical Nursing Program. We strive to constantly improve and maintain the highest possible quality program available to the community. In order to achieve this goal, the nursing program requires high expectations from faculty, staff, and students. Throughout the course of this training students will have opportunities to provide input related to the policies and procedures of the program via confidential survey. Recommendations from these surveys are carefully analyzed by the administration, discussed in faculty meeting, and forwarded to the instructors for implementation where appropriate. Further discussion regarding student input and evaluation may be found in Article III. STUDENT LEADERSHIP.

Nursing Instructors, Staff, and AdministratorThe EVIT PNP values the skills, knowledge, and abilities of all nursing instructors, staff, and Nursing Program Administrator. PNP nursing instructors and staff may work full time or part time to provide instructional and support services. The Nursing Program Administrator works full-time overseeing the operations of the program, providing support to the instructors, staff and students, and maintaining the quality and integrity of the program.

1. All nursing instructors and the Program Administrator have an unrestricted nursing license in the state of Arizona.

2. The Nursing Program Administrator has a minimum of a graduate degree in nursing with a major in nursing and a minimum of three years’ experience as an RN providing direct patient care and a minimum of one academic year full-time experience teaching in or administering a nursing education program leading to licensure.

3. Instructors for the PNP have a minimum of a baccalaureate degree with a major in nursing and a minimum of two years’ experience as an RN providing direct patient care.

4. Each clinical instructor has functioned in a clinical area prior to being assigned to teach in a clinical assignment.

5. Nursing instructors, the director, and staff are required to continuously complete professional development activities.

Program Evaluation

The EVIT Practical Nursing Program undergoes regular evaluation by the accrediting agencies listed in the Adult Education Handbook and Catalog. The program and curriculum are faculty developed and meet the requirements of the AZ Board of Nursing. The Board of Nursing has oversight of the Program and conducts periodic evaluations to ensure quality education. Additionally, EVIT PNP values input from students and uses student input form confidential surveys in order to assess and evaluate program improvement needs including curriculum and content delivery.

Program Oversight

Program RegulationThe EVIT Practical Nursing Program follows the Arizona Rules for Practical Nursing education. These statues also regulate the scope and practice of the entire nursing field. Details can be found at http://www.azbn.gov/NursePracticeAct.asp. The Arizona State Board of Nursing (ASBN) and the Arizona Nurse Practice Act as they relate to the professional nurse will be discussed throughout the program.

Student Nurse RegulationsAll nursing students should refer to Arizona Nurse Practice Act for information regarding reasons for denial of licensure to practice as a practical nurse in Arizona and reasons for disciplinary action by the Arizona State Board of Nursing. The ASBN requires that any nurse with a license in Arizona report any arrests, charges, or potential violations of the Arizona Nurse Practice Act to the ASBN within ten days of the incident in question. Students are required to inform the nursing program director, as well. Students who report an incident to the instructor, and/or director will be advised on the impact, or potential impact, on the student’s training and financial aid. All such discussions will be held in confidence.

Felony ConvictionsApplicants for a nursing license or certification who have been convicted of a felony must show documentation that five or more years have elapsed from the date of the absolute discharge of a sentence for the felony or undesignated offense for the ASBN to consider the student to be a candidate for licensure and for EVIT PNP to consider the applicant for admission to the Program. Applicants with felony convictions, who have fewer than five (5) years since the date of absolute discharge of a sentence, are not eligible for a nursing license. Individuals charged or convicted of misdemeanors will be reviewed by the ASBN on a case-by-case basis. Questions regarding licensure should be referred to the Arizona State Board of Nursing.

Admissions

PrerequisitesIn order to be considered for admission, applicants are required to complete the TEAS exam (60% overall score is preferred), participate in advisement with both the financial advisor and the Practical Nursing Program Administrator, and have completed the following:

1. Nursing Assistant—Prior to July 1, 2016, Applicant must hold current Arizona State certification as a CNA in good standing OR provide proof of successful completion of nursing assistant program and Board of Nursing CNA examination with certificate application in place prior to start of classes (verification is required). If applying after July1, 2016 or if CNA certification expires on or about July 1, 2016, Applicant must hold a current Arizona LNA license.

2. High school graduation – An applicant must show evidence of high school graduation or the equivalent in country of origin.

3. Math – Applicant must have at least two years of high school math or one year of basic college math or higher taken within the last 5 years with a grade of C or better AND an acceptable TEAS math score.

4. Biology/Chemistry – Applicant must have one year of high school biology or chemistry one semester of college life sciences biology or chemistry taken within the past 5 years with a grade of C or better.

Background ChecksAll students desiring to be admitted to the Program are required to provide a DPS Level 1 Fingerprint Clearance Card showing the completion of a background check. It is the student’s responsibility to maintain a current DPS card during the entire time he/she is in the program.

Proof of Legal ResidenceAccording to the ASBN requirements, all applicants must prove US citizenship or legal residence to receive a license to practice nursing in Arizona. (See www.azbn.gov) for a list of acceptable documents.

Practical Nursing Advanced Placement

There is no advanced placement offered for the EVIT Practical Nursing Program.

Transferability of credit

The EVIT PNP is a clock hour program leading to permission to sit for the NCLEX-PN licensing exam. A transcript of the course work and sequence will be provided to the student upon determination of program completion. Syllabi for each individual course of instruction are

available on request. The PNP does not guarantee the transferability of any coursework as this is the sole decision of the accepting institution.

The Practical Nursing ProgramEvidence-based Education

The EVIT Practical Nursing Program is a clock-hour program. Success is determined by a student’s ability to demonstrate the knowledge, skills and abilities to perform competently as a practical nurse. This grouping of skills and knowledge are referred to as competencies. The competencies are based on evidence from actual nursing practice. Learning activities are organized into courses. PN students progress through courses in sequence through the demonstration of competence in performing nursing skills, passing exams, and performing satisfactorily in nursing clinical assignments. Attendance is recorded as mandated by federal financial aid requirements.

Instructional Methods/Technology

PN Instructors employ a variety of teaching and learning strategies during the course of the Program. These include but are not limited to PowerPoint supported lecture, small group discussions, case scenarios, demonstrations, and computerized review. Computer based online ATI instruction and review courses are available for use by students both at school and on their personal technological devices 24/7. Several charting methodologies are introduced to the students in the classroom and during nursing lab, then practiced in various settings. Information Technologies Support staff are available to assist faculty and students as needed during all Program hours. Computers with Internet connection are available to students in the PN Resource Center during all program hours; the Computer Lab is reserved weekly and as needed for the PN class.

Clinical Requirements

Each student must submit appropriate proof of immunizations or immune status before entering the first course in which there is a clinical component. Students must hold and maintaining “CPR for the healthcare professional” certification. Some clinical facilities require the completion of online clinical orientation modules. Any fees associated with clinical requirements are the sole responsibility of the student. If the requirements are not kept current, the student will be removed from the clinical assignment until the requirements are submitted to his/her program file. This may delay the student’s progress through the training program.

Role of the Instructor

Instructors serve as facilitators to learning. They provide the information, mentoring, and direction needed for students to access the information they are required to learn in order to pass the program and the state licensing exam. Instructors cannot force a student to learn or utilize the resources provided—that is the responsibility of the student.

Each student is encouraged to make arrangements to meet with the course instructor whenever classroom issues arise. You may have different instructors with each course. Each student will have the opportunity to meet with the instructor during the course for an Individual Progress Report. This is an opportunity for students to discuss issues and concerns. Students are encouraged to make appointments to see instructors any time there is a need.

Instructors do not work for the clinical agencies, in clinical sites they are still educators who are at the site to supervise the students as they practice giving nursing care in the clinical setting. Students may not give gifts to the instructors. To avoid the appearance of bribing, an instructor may only accept a token gift from an entire class of students.Instructor assistance may include by not be limited to the following:

1. Study plan and outlining assistance2. Review procedures3. Test taking strategies4. Assistance with technology5. Clarification of course or unit objectives6. Clarification of assignment guidelines7. Academic counseling and evaluation8. Guidance for group projects and presentations

Program Sequence

The following is a sequence of courses and their associated hours. Class schedules will be

affected by holidays occurring during courses. Classroom and clinical components run

concurrently for selected courses. The Program is one calendar year in length and is comprised

of eight courses in three blocks of instruction as follows:

Guidelines, Policies, and Standards

Article I. STUDENT CODE OF CONDUCT

Section I.01 Student Responsibilities- Students in the nursing program are expected to assume the following responsibilities:

(a) Maintain client and fellow student confidentiality. (b) Maintain ethical integrity when completing tests, reports, homework, and projects. The

work must reflect only the student’s work unless it is properly cited or the instructor specifically indicates otherwise.

(c) Know and follow all written or verbal instructions given by instructors. (d) Contact instructors to ask questions and get clarifications about assignments, test grades,

and program progress. (e) Report any errors or “near miss” in client/patient care to a nursing instructor and/or the

appropriate staff member. (f) Report to the nursing clinical instructor any observations of other students or staff

performing unsafe patient care. (g) Follow the chain of command to report problems, complaints, or violations. (h) Sign in and out as directed for all learning experiences.

(i) Adhere to clinical dress codes. (j) Maintain a professional demeanor in dress, hygiene, language and behavior while in the

classroom or at the clinical site.(k) Remain current on all financial obligations to the school.

Section I.02 Standards: The nursing department promotes a high standard of ethics and integrity for students to follow. The nursing profession carries much responsibility for another person’s life and well-being, and licensed nurses are expected to adhere to a high level of integrity. No violation of the Student Code of Conduct will be tolerated. Doing so may result in disciplinary action or dismissal from EVIT Practical Nursing Program. The following violations are captured in the PNP Student Responsibilities and are of particular importance to the program:

(a) Cheating- Intentionally using or attempting to use unauthorized materials, information, or study aids in any academic exercise. This includes but is not limited to:

(i) Copying from another student’s paper(ii) Knowingly using, buying, selling, offering, transporting, or soliciting any of the

contents of a test(iii) Taking a test for another student or permitting someone else to take a test for you (iv) Bribing or attempting to bribe, or intimidating or attempting to intimidate another

person or instructor to obtain a passing or better grade on a test, paper, assignment, or course grade

(v) Intentional misconstruing or misreporting of facts or incidents relating to any event occurring during the program

(b) Plagiarism- Defined as the representation of work of others as your own. This includes the use of another’s words, facts, ideas, or information without acknowledgement of the source. It may occur in a variety of ways:

(i) Quoting without giving proper credit(ii) Expanding someone else’s work without giving proper credit

(iii) Adopting an actual document, including an electronic document(iv) Directly using someone’s ideas, rather than words, without giving proper credit

(c) Compromising Test Materials- Includes unauthorized removal of testing materials from any area, writing or copying test questions, or verbally or electronically sharing test questions with any other person.

(d) Facilitating Academic Dishonesty- Intentionally or knowingly helping or attempting to help another to violate any provision of this code. This includes collusion and fabrication.

(e) Misrepresentation/Fraud- This includes:(i) Using false records, false identification papers, and any portion of the uniform,

unauthorized I.D. cards or methods of I.D. to influence others concerning your status

(ii) Accessing official documents, services, departmental records or materials, or patient information without specific permission via electronic or other means including: attendance sign-in sheets (signing in or out for another person, asking another student or staff member to sign you in or out, leaving the campus for extended periods without signing out or without instructor permission, or failing to sign in or out appropriately)

(f) Violating the Practical Nursing Scope of Practice- This includes operating outside the level of knowledge and skills permitted by the student’s current level in the nursing program and as defined by the Arizona State Board of Nursing.

(g) Disruption of the learning process, infliction of bodily or mental harm, use of vulgarity or profanity, harassment- Intentional behaviors aimed at other students, instructors, staff, or patients or staff in either the didactic or clinical setting.

(h) Defacement, destruction or theft of property-Using school equipment and supplies in an unsafe or inappropriate manner, or taking supplies off school grounds without having permission to do so.

(i) HIPAA or FERPA Violations- This includes failure to maintain confidentiality on school grounds and/or in the clinical setting.

Article II. VIOLATIONS AND DISCIPLINARY ACTION

Section II.01 Code of Conduct Violations- If the student is found to have violated any rules of the Code of Conduct, any one or a combination of the following penalties may be imposed. This list of disciplinary actions is not intended to be exhaustive or all-inclusive.

(a) Student may receive a zero on the work involved. (b) Student may receive a zero for the course. (c) Student may receive a formal reprimand. (d) Student may be suspended from the nursing program. (e) Student may be dismissed from the nursing program. (f) Student may not be eligible to re-enter the nursing program. (g) Students may be removed from a clinical location or all clinical locations.

Section II.02 Behaviors That May Result in Dismissal and/or Ineligible to Re-enter- Nursing students are expected to consistently meet program standards and guidelines. In addition to violations of the Code of Conduct, the following behaviors may be grounds for dismissal from the nursing program, without a probationary period:

(a) Falsifying attendance records (b) Reporting to class or clinical assignments under the influence of alcohol or other drugs,

or with any evidence of impaired performance related to chemical abuse(c) Theft or vandalism of school or others’ personal property(d) Behavior violation resulting in disruption of the learning environment(e) Conviction of a felony(f) Violation of the ASBN Scope of Practice

(g) Failure to meet tuition or other school financial obligations(h) Safety violation(i) Repeated no call/no show absences or tardiness in either the clinical or classroom

environments

Section II.03 Nursing students may be immediately withdrawn from the practical nurse program based on the inability to place a student in a clinical facility resulting from his/her conduct in the clinical facility.

Section II.04 The Discipline Process- Violations that endanger patients, other students, or the integrity of the program may dictate immediate action beyond warning or probation. Students have a right to file a grievance at any point in the process. The following is the discipline process:

(a) The first Code of Conduct violation will result in a verbal warning, given within three (3) school days of the occurrence, which will be documented and put in the student’s file.

(b) A second Code of Conduct violation will result in a written warning, given within three (3) school days of the occurrence, with the student being placed on probationary status (Behavior Probation). This status will remain in effect for the duration of the program. The written warning will state the specific requirements the student must meet to remain in the program. A copy will go in the student’s file, and the student will receive a signed copy.

(c) A third Code of Conduct violation will result in a formal disciplinary process, to be initiated no more than ten (10) days from the date of third Code of Conduct violation, and possible dismissal from the program.

(d) With each Code of Conduct violation the student will be informed they have violated the Code of Conduct, informed of the specific consequences, and given an opportunity to discuss the situation with the Instructor, and/or Nursing Program Administrator, as is appropriate to the situation and within the timeframes outlined above. All disciplinary activities will be documented. All disciplinary activities will remain confidential, and shared only with staff on a need-to-know basis.

Section II.05 Grievance Process- During the course of training, individual issues and concerns may arise that require formal resolution. EVIT PNP has a grievance procedure in place to allow the swift resolution of areas of concern.

(a) Individual issues and concerns should be first brought to the Instructor’s attention, within ten (10) days of the occurrence, and discussed at the earliest possible opportunity.

(b) If no resolution is reached within ten (10) days of the student/Instructor conference, the student and/or Instructor may request a meeting with the Nursing Program Administrator to discuss the issue. These meetings may be held individually or together and must take place within ten (10) days of request to meet with the Administrator.

(c) At any point the student may file a formal written complaint with the Nursing Program Administrator. Formal written complaints will be addressed by the Administrator within ten (10) days of receipt. The following information must be in the written complaint:

(i) Name of the student(ii) A formal request for a grievance hearing

(iii) A statement of the date, time, location, and the person(s) involved in the event(s) giving rise to the grievance

(iv) A statement of complaint or what occurred(v) A statement regarding attempts made to resolve the problem at an informal level

(vi) The desired outcome

Article III. STUDENT LEADERSHIP

Section III.01 Philosophy- EVIT and the PNP value input from students and use this input to assess and plan changes to improve curriculum and content delivery and to develop policies and procedures for the Program. Throughout the process of formal and informal peer group leadership, nursing students are mentored in the skills they will use as productive and effective members of the contemporary healthcare team. Class officers, elected by their peers, may function autonomously under general school guidelines in the management of student-led classroom and community projects. Each Cohort will have its own class officers and conduct their own class business.

Section III.02 Class Officers- At the beginning of each Block students may nominate self or others for the various class officer positions. Terms of office are for one Block. Elections are student led and occur once per Block unless necessitated by early departure or vacancy of elected position. Voting is by hand-marked secret ballot. Tabulation of votes is conducted and documented by an election committee of no more than three students, under the supervision of the Instructor as needed. Class officer roles are listed below.

(a) President- Determines timing of class meeting cooperatively with Instructor, creates class meeting agenda, conducts class meetings.

(b) Vice President- Assists President in all activities as needed and fulfills role of President when President is unable to be present for any reason.

(c) Treasurer- Accepts funds and keeps written account of transactions involving class monies. Counts all monies incoming and outgoing with at least one other class officer, documenting total and date of transaction with signatures. Provides a report of funds at class meetings.

(d) Secretary- Keeps accurate and detailed meeting minutes, records monies in and out along with Treasurer. Completes required forms and paperwork necessary to conduct class business and promote project management.

(e) Activities Coordinator- Assists in the promotion and development of social activities such as class parties, fund raisers, lunches, community service projects, and holiday decorations.

(f) Class Representative- Meets with Instructor as needed to communicate class concerns and plans and to gain approval or advice for plans. Meets at least quarterly with Nursing Program Administrator to discuss class issues, coordinate implementation of improvement objectives, and keep the lines of communication open between students and faculty.

Section III.03 Student-led Class Meetings- Monthly class meetings will be held and conducted by the class President or Vice President. Meetings are run according to the agenda created by the class officers as determined by plans or issues from previous class meetings.

Section III.04 Comment Drop Box- Each classroom shall have a comment box in which students may anonymously place their written suggestions for course improvement. Comments therein will be reviewed by faculty and administration on a quarterly basis and feedback given to each specific class.

Section III.05 Program Evaluation- At the conclusion of each course and clinical assignment students are given the opportunity to complete an evaluation of various aspects of their educational experience. At the conclusion of the PNP students are given the opportunity to evaluate the program in its entirety. Written and verbal comments and suggestions for program improvement are welcomed throughout the course of the program as well.

Section III.06 Quarterly Leadership Meeting with Administration- Every quarter the Nursing Program Administrator will invite each Cohort’s Class Representative for a private meeting to discuss class issues, coordinate implementation of improvement objectives, and keep the lines of communication open between students and faculty. Minutes of the meeting will be taken at the time of the meeting and signed by both the Class Representative and the Nursing Program Administrator. Minutes shall include an action plan section if applicable. Meeting minutes including an action plan will be formally reviewed with the Adult Education Director within 10 business days of the quarterly leadership meeting to discuss the mechanism of response and create a timetable for implementation if applicable. Topics of discussion at the quarterly leadership meeting may include but are not limited to the following:

(a) Information, issues, and requests presented from the classroom comment drop box.(b) Information, issues, and requests presented during class meetings. (c) Suggestions specific to modification of academic policies and procedures.

Section IV.01 For students returning to the program after a Leave of Absence or after having formally exited the program, readmission is not automatic. All readmissions to the nursing program are subject to space availability and administrative approval. Students will have one opportunity to be readmitted to the nursing program. Students seeking readmission for any reason must submit a readmission application to the Nursing Program Administrator or designee. An absence of 6 months or more from the program may necessitate readmission at the beginning of the next cohort.

Section IV.02 Readmission Guidelines Related to Substance Abuse- Students withdrawn from the EVIT PNP for reasons related to substance abuse may reapply to the program after a successful required period of treatment of at least one year and must do the following:

(a) Submit a letter requesting readmission to the Practical Nursing Program.(b) Include documentation from a therapist specializing in addiction behaviors indicating

status of abuse, addiction, or recovery and/or documented rehabilitation related to the alcohol/drug illness.

(c) Include documentation of compliance of a treatment program as identified by the therapist including a statement that the student will be able to function effectively and provide safe and therapeutic care for clients in a clinical setting.

(d) Repeat drug screen for alcohol/drugs immediately prior to readmission.(e) All drug screening fees will be the responsibility of the student.(f) Submit full tuition for that year.

Section IV.03 If a student, after being re-admitted to the nursing program, has positive results on an alcohol/drug screen, the student will be exited from the program and barred from readmission.

Article V. ACADEMIC STANDARDS

Section V.01 Educational Progress in Training- A progressive sequencing of classroom and clinical instruction has been designed in the course so that the practical nursing student can form necessary links of theoretical knowledge, clinical reasoning, and practice. Students must pass the classroom (didactic) theory and clinical component of each course. A passing didactic score is 78% minimum grade point average. A grade point average below 78% is a failing grade. Clinical experience is graded on a Pass/No Pass basis. Failing to pass either component of the course, didactic or clinical, will cause the student to fail the entire course and become ineligible to continue on to the next course. The failed course must then be retaken in its entirety and passed in order to remain in the program. In addition to the material presented in this section, students must adhere to specific academic guidelines and information presented in each course syllabus.

Section V.02 Student Work(a) Written assignments are due on the day and at the time stated by the Instructor. A 10%

reduction in score will be taken for each regular school day the assignment is turned in late, up to a maximum of four days. Upon the fifth day an assignment is late a score of zero “0” will be entered in the gradebook.

(b) Under extreme hardship circumstances, such as death in the family, the Instructor may elect to allow a reasonable grace period for late work. In order to be considered for this concession the student shall meet privately with the Instructor to discuss the unique situation, providing documentation of its validity if requested.

(c) Written assignments shall be given 10-15% of the academic weight allotted to a unit test. Individual research projects may be weighted no more than 40% of a unit test score.

(d) From time to time students may be assigned graded or ungraded group projects. It is expected that each member of the group will perform to the best of his/her ability in completing the assignment. Furthermore, each group member will share the task load in an equitable manner as determined by the student group. Group projects may be weighted up to 40% of a unit test score. The grade earned by the group will be applied equally to each member of the group in the official gradebook.

Section V.03 Instructional Course Testing and Assessment- Every effort will be made to ensure that each student has an equal opportunity to complete testing in a standardized and uninterrupted environment. It is the responsibility of the student to learn the material included in each course of instruction and to meet the learning objectives presented in class. Detailed study guides will not be given to direct student attention in preparation for unit tests or final exams.

(a) Students must be present at the beginning of the testing period to be allowed to participate in testing. There will be no entry to the classroom once testing has begun.

(b) If a student is absent from the class during testing the student will be required to take an alternate make-up test of the Instructor’s choosing that contains material from the same set of competencies as the original test but with different test items. The make-up test must be taken after class on the first available school day following the student’s absence. It is the student’s responsibility to make arrangements with the Instructor for testing following regular classroom hours. Taking a make-up test may be done once in a course. A second missed original test will result in a zero “0” score recorded in the gradebook for the missed test.

(c) Unit tests are taken utilizing a ScanTron answer sheet. Students must bring two #2 pencils with erasers to the test for use in selecting their responses on the ScanTron sheet. Scratch paper will be supplied by the Instructor when needed for math calculations. Students should have their own simple calculator available for calculations. Cell phones and other electronic devices may not be used as calculators while testing.

(d) Quiz documents may be written on directly using either a blue or black pen or pencil.(e) Students observed to be either giving or receiving assistance from other students or

utilizing materials not part of the testing process will receive a zero “0” score for the test. The incident will be recorded in the students’ permanent files and reported to the Program Administrator.

(f) Students must remain seated and in the testing area until they have completed their test responses. If permitted by the Instructor, students who have completed their test may turn in their test and answer document and leave the testing area quietly. Students who leave the testing area for any reason may not re-enter the area until the conclusion of the testing process as indicated by the Instructor.

(g) Answer documents turned in by the student are considered to be complete once surrendered to the Instructor and may not be modified by the student prior to or after scoring. No credit will be given for test items left blank, incorrectly filled in ScanTron items, or more than intended marks for a single test item. It is the student’s responsibility to correctly fill in the ScanTron answer document during the allotted testing period.

(h) Students who are disruptive during testing will be immediately dismissed from the testing area and receive a zero “0” grade on the test.

(i) The Instructor has the option to specify a time limit for testing or impose such procedures as will best facilitate the testing environment.

(j) Accommodations for testing can only be made for students with written and approved disability documentation on file.

(k) Assessment and evaluation tools utilized throughout the program include but are not limited to formal written tests, oral and written quizzes, projects, homework assignments, attendance and participation, case studies, skills demonstration, observation and documentation of interactions, self-assessment, web-based NCLEX-PN prep assessments.

Section V.04 Post-Testing Period- As soon as possible following the testing period the answer documents will be scored and results given individually to students.

(a) The Instructor will review the test and answers with the class as a whole. Correct answers and rationale will be provided for each test item along with references. Students may also review their test and answers individually with the Instructor if desired.

(b) Individual student test scores, assessment scores, and grades are kept confidential unless the students themselves disclose the score.

(c) Students experiencing difficulty in attaining the required minimum score of 78% on assessments will be assisted to develop successful study and test taking skills and counseled to avail themselves of group study opportunities.

Section V.05 Medication Dosage Calculation- Accurate calculation of medication dosage is a fundamental nursing skill. Basic math skills, dosage calculations, and medication scenarios will be presented on exams throughout the Practical Nursing course. Mastery of dosage calculation is a prerequisite for continuing on from the Introduction to Nursing Studies course to the Medical/Surgical Nursing I course in which the student nurse will be assigned to administer medication to clients at the clinical facility. To demonstrate mastery prior to medication administration a Dosage Calculation Exam must be passed with 100% accuracy by the conclusion of the Introduction to Nursing Studies course. Coursework and practice exams will be given in preparation for this Exam. Each subsequent clinical rotation may have a mandatory Dosage Calculation Exam competency test to determine student preparation for client care.

Section V.06 Grading- The Arizona State Board of Nursing standard is that 80% of nursing school graduates pass the NCLEX-PN the first time within 12 months of program completion. Academic policies and procedures of the EVIT Practical Nursing Program are designed to assist students to meet this goal, including passing each course of study with a minimum grade point of 78% and maintaining high enrollment and re-admission standards.

(a) A summary of student progress is maintained and available at all times in the electronic gradebook. A printed progress report will be given to each student at the midpoint and conclusion of each course of instruction. Students may request a printed grade report at any time in between the scheduled progress report printing.

(b) At course midpoint, end, and at the end of each clinical rotation, students will be given the opportunity during class time to discuss their progress with the Instructor. Students may request private conferences outside of class time as well. Students will be given feedback and evaluation of performance in the clinical, skills lab, and classroom settings.

(c) Instructors will initiate a private conference with a student whenever the student’s grade point average falls below passing (78%). At this time the Instructor will assist the student to identify methods to improve study habits and test more successfully.

(d) Total point weighting of didactic instructional course components:1) Homework, Projects, Assignments: 10-15% of total course points possible2) Quizzes and Unit Tests: 55-60% of total course points possible3) Course Final Exam: 25-30% of total course points possible

Section V.07 Academic probation- A verbal warning will be given to students when a pattern of low scores indicates risk for failing a course. Students who maintain a failing grade (below 78%) at the midpoint of an instructional course will be placed on academic probation. Students with a passing grade at midpoint but a failing grade at the conclusion of the course will be placed on academic probation.

(a) The student will meet with the Instructor in order to complete an academic probation contract. The terms of probation, plan for academic improvement, and consequences for lack of improvement will be documented on the probation contract. The probation contract will be signed and dated by the student and appropriate faculty member. A copy of the document will be offered to the student at that time.

(b) At this meeting the student and Instructor will explore ways to meet the terms of the contract, improve academic achievement, and successfully complete the course.

(c) Students placed on academic probation at midpoint: If at the end of the course the student’s grade point average is still below 78% a failing grade will be given for the course. This will result in review of the probation contract with the program administrator which may result in dismissal from the program.

(d) Students placed on academic probation at the conclusion of the course: Students placed on academic probation at the conclusion of a course will remain on probation into the next instructional course. If at the midpoint of the next course the student has maintained a failing grade, the student will be considered for dismissal from the program. Review of the probation contract with the program administrator will take place, which may result in dismissal from the program.

(e) A student who withdraws or is dismissed from the program for academic reasons will be assigned a grade of Y (withdrawn, failing) in the gradebook.

Section V.08 Appeal of Dismissal for Academic Failure- A student recommended for dismissal from the Practical Nursing Program due to academic failure may make a formal appeal. Only one appeal of any kind will be considered per student during the entirety of the Program regardless of entrance status.

(a) Within 48 hours of receiving notice of intent to dismiss, the student shall be required to compose and submit a formal written appeal of dismissal directly to the Nursing Program Administrator. Verbal communications will not be considered as an appeal. The appeal must include circumstances of the academic failure, the reason why the student believes he/she should be considered for appeal, and the steps he/she will take to improve academic performance.

(b) Following receipt of appeal the Nursing Program Administrator will gather pertinent information, consider the counsel of faculty as needed, and deliver an answer to the student within ten business days or less. The appeal will either be granted, allowing the student to remain enrolled in the Program, or denied, causing the student to be immediately dismissed from the Program. During the time the appeal is under consideration the student shall remain in class unless other violations, such as safety or behavior, would make attendance unadvisable.

(c) Students granted appeal for academic failure will remain on academic probation until midpoint of the next instructional course (for appeals granted at the conclusion of a course) or until the conclusion of the course (for appeals granted at midpoint in a course). If the student’s grade point average is still below 78% at the end of the appeal period as stated on the contract the student will be dismissed from the program. The student, if qualified for re-entry to the program, will be required to repeat the originally failed instructional course before progressing on in the program.

Section V.09 Repeating a Course Due to Failing Grade- Students who receive a failing grade in a course and are dismissed from the program having no student conduct issues will have the option of repeating the course when space allows. Only one course may be repeated and re-entry is on a space available basis only. An absence of 6 months or more from the program may necessitate readmission at the beginning of the next cohort. The student must meet with the course Instructor and Nursing Administrator at the time of dismissal in order to identify ways to improve academic performance and areas where further study is needed. Re-admission to the program is not guaranteed nor is it automatic. The student must contact the Enrollment Advisor and fill out required forms in order to be considered for re-entry. At that time a meeting will be set up with the course Instructor and Nursing Administrator to determine the student’s suitability for re-entry. Failure of more than one course of instruction or having a failing grade at the midpoint of a repeated course will result in the student being permanently exited from the program.

Section 5.10 Assessment Technologies Institute® (ATI) The comprehensive nursing education, assessment, and remediation program offered by ATI is utilized throughout most courses in the Practical Nursing Program. This assessment driven review program is designed to enhance student NCLEX-PN success. Syllabi and dated course outlines for courses in which ATI is utilized will contain specific information regarding assignments, tests, and associated points. Towards the end of the Program students will take the ATI PN Comprehensive Predictor exam via the online environment. The purpose of this exam is to indicate the student’s “Predicted

Probability of Passing NCLEX-PN® on the First Attempt.” Each of the aforementioned standardized assessments are valuable tools to assist the Practical Nursing student to identify areas of knowledge strength as well as areas in which active learning and remediation would be beneficial in preparation for the NCLEX-PN®. Article VI. ATTENDANCE STANDARDS

Educational Hours of Attendance- Hourly schedules for classroom and clinical instruction will be detailed in each specific course syllabus. Hours for clinical will vary with the clinical site and experience. Some weekend hours may be required for both clinical and for preparation for clinical. The nursing office is open at 7:30 a.m. and closes at 4:00 p.m. Monday through Friday. Some instructors may utilize different days or hours during the week and this may change during the course.

The EVIT Practical Nursing program is a clock-hour program. Regular on-time performance and active participation in the learning environment are necessary to prepare the practical nursing student for licensure and subsequent placement in the workforce. Therefore, students are expected to attend 100% of regularly scheduled didactic and clinical experiences. Because unforeseen personal circumstances may arise students are required to attend a minimum of 90% of didactic, clinical, and skills/simulation lab hours for each course and 90% of the cumulative program hours.

If a student will miss educational time due to illness or extreme personal emergency, he or she must notify the Instructor prior to the commencement of the learning experience. Students should consult the specific course syllabus for detailed information regarding Instructor contact numbers, times, and locations of educational activities for that course as these may vary with each training course.

Section VI.01 Didactic Attendance(a) Classroom attendance is imperative in order for students to fully understand the

theoretical basis of safe and effective nursing practice and to form the link between theoretical knowledge, clinical reasoning, and nursing practice.

(b) Nursing students are required to sign in and out of the classroom using the designated attendance log. Sign-in is required during the first five minutes of class time. Furthermore, attendance will be taken when class resumes following each daily break. The classroom attendance log sheet is used to complete the official attendance log for each course.

(c) Tardiness is disruptive to the learning environment. Time missed from class due to tardiness or early departure is recorded as time absent. Upon the second didactic tardy in a course the student will be placed on attendance probation. It is the responsibility of the student to meet with the Instructor following class after the tardy in order to complete a probation contract. A third tardy will result in review of the probation contract with the Nursing Program Administrator, which may result in dismissal from the program.

(d) Early departure from the classroom for any reason is disruptive to the learning environment and requires sign-out on the attendance log. Early departure is defined as leaving the classroom during instructional time or leaving campus prior to class dismissal. Personal belongings should not be left in the classroom when the student signs out. Upon the second early departure during a course the student will be placed on attendance probation. It is the responsibility of the student to meet with the Instructor on the next didactic day after the early departure in order to complete a probation contract. A third early departure will result in review of the probation contract with the Nursing Program Administrator, which may result in dismissal from the program. The Instructor may use discretion in re-admitting a student to the classroom for extreme circumstances, such as an acute medical issue.

(e) Upon the second didactic absence in a course, the student will be placed on attendance probation. It is the responsibility of the student to meet with the Instructor on the next didactic day after the absence in order to complete a probation contract. A third absence will result in review of the probation contract with the Program Administrator, which may result in dismissal from the program.

(f) It is a violation of the student code of conduct to sign-in or out for another student or to solicit another student to falsify information on the sign-in sheet (this includes signing in and then leaving training without signing out). This is also a violation of federal law for students receiving federal financial aid.

(g) Breaks on test days will vary with the testing schedule in mind. Students will not be admitted after testing has begun for any reason.

Section VI.02 Clinical Attendance- Students are expected to attend 100% of clinical hours. Students are required to maintain 90% attendance for the clinical component of each course. This includes both facility clinical rotation and simulation lab. Clinical attendance below 90% will result in failure of the course with the requirement of repeating the course in its entirety.

(a) Students who come to clinical acutely ill will be sent home, and that day will count as an absence. There are no exceptions.

(b) Nursing students are required to sign in and out of clinical using the designated attendance log. The clinical attendance log sheet is used to complete the official attendance log for each course.

(c) There will be no admittance to clinical following report (transfer of care).(d) Early departure from clinical without the recommendation of the clinical instructor is

considered to be abandonment of patient care and is strictly prohibited. Students who voluntarily leave clinical early for any reason will be placed on clinical attendance probation.

(e) Upon the first clinical tardy the student will be placed on attendance probation. It is the responsibility of the student to meet with the course Instructor on the next didactic day after the tardy in order to complete a probation contract. Further clinical tardiness will result in review of the probation contract with the Program Administrator, which may result in dismissal from the program.

(f) Upon the first clinical absence the student will be placed on attendance probation. It is the responsibility of the student to meet with the course Instructor on the next didactic day after the absence in order to complete a probation contract. Further clinical absence will result in review of the probation contract with the Program Administrator, which may result in dismissal from the program.

(g) Students who arrive to the clinical site in violation of the dress code may be sent home, and that day will count as an absence.

(h) The clinical Instructor will notify the staff in the Nursing Program office of clinical attendance.

(i) When a student is absent from a clinical assignment he/she may not sign in on the campus to receive credit for the day unless directed to do so by his/her Instructor.

(j) One no-call/no-show at clinical will result in the student being placed on probation. A second no-call/no-show may result in the student being exited from the program

Section VI.03 Attendance Probation- Students failing to meet the Practical Nursing Program standards for attendance will be placed on written attendance probation. Attendance violation events and standards are defined in Sections 6.01-6.02 above, including but not limited to absences, tardiness, early departure, and no-call/no-show.

(a) It is the responsibility of the student to meet with the course Instructor on the next didactic day after the attendance violation event in order to complete a probation contract. The probation contract will be signed and dated by the student and appropriate faculty member. A copy of the document will be offered to the student at that time.

(b) Further attendance violations will result in review of the probation contract with the Program Administrator, which may result in dismissal from the program.

Section VI.04 Dismissal for Attendance Violation- If it is determined by the faculty and administration that the student is unable to meet course objectives and/or attendance standards within a course due to didactic and/or clinical attendance violations the student will be counseled to withdraw from the course. If a pattern of attendance violation develops from course to course during the Practical Nursing Program the student will be recommended to administration for dismissal from the program.

(a) A student who withdraws or is dismissed from the program for attendance reasons will be assigned a grade of W (withdrawn, passing) in the gradebook.

(b) Students who have been withdrawn or dismissed from the program must follow the guidelines for readmission found in Section 6.08 and 4.01 of this Handbook.

Section VI.05 Appeal of Dismissal for Attendance Violation- A student recommended for dismissal from the Practical Nursing Program due to attendance violation may make a formal appeal. (c) The student, if qualified for re-entry to the program, will be required to repeat the instructional course from which they were withdrawn or dismissed before progressing on in the program. An absence of 6 months or more from the program may necessitate readmission at the beginning of the next cohort.

Section VI.06 Leave of Absence (LOA)- Students who will miss more than one week of training at a time are allowed to take one LOA during their training, for a maximum of 180 days. Per federal financial aid guidelines, students may not take an LOA strictly for academic reasons (poor grades).

(a) Students who develop attendance deficits as a result of health or personal issues which cannot be remedied in a reasonable timeframe by the student may be encouraged to take an LOA. Some reasons for LOA include but are not limited to: 1) Medical. 2) Personal. 3) Military Duty. 4) Extended Jury Duty. 5) Attendance is interrupted due to unavailable class schedule when repeating a course.

(b) A medical LOA requires a release from a licensed health provider in order to return to the EVIT Practical Nursing Program course of training.(c) The date the student will be able to return from the LOA will be determined by the availability of space in the program. Students must return to the beginning of the course they did not complete. (d) Students who are considering an LOA must get approval from the Instructor and the

Nursing Program Administrator. All parties shall meet with the student to discuss the student’s progress, financial aid concerns, and possible resolutions to the student’s issues.

(e) In order to be an approved LOA an EVIT LOA form must be completed and signed by all parties indicated on the form.

(f) Prior to re-entry into training the student must meet with the enrollment advisor to discuss the timing and conditions of re-entry. The class Instructor and administration will then

meet to consider class availability. If appropriate, an EVIT LOA Clearance form will be given to the student containing instructions on returning to class.

Section VI.08 Program Dismissal or Withdrawal from Training- Dismissal/withdrawal from the Practical Nursing Program may be initiated by either the Nursing Program Administrator or the student. Students leaving the program must meet with the Enrollment Advisor and the Nursing Program Administrator to determine if the student is eligible to re-enter the program and repeat the course from which he/she has withdrawn, and to complete all required forms. When all financial obligations to the school have been satisfied the student may receive a transcript for all instructional courses completed.

Section VI.09 Holidays/Scheduled Days Off- The Practical Nursing Program follows the EVIT published customary federal and state holiday schedule. Additional scheduled days off may occur. Students will be notified of any changes to the schedule as soon as they become available. Students who need to take additional religious holidays should submit a request to the classroom Instructor.

Article VII. CLINICAL AND LAB POLICIES

Section VII.01 Skills Lab- The skills lab exists to give students an opportunity to practice and perfect hands-on nursing skills before performing these skills on real patients. The Practical Nurse Program provides this opportunity to ensure a safe and humane patient care in the clinical setting. Students will also be evaluated on ethical and professional behavior. Students in the skills lab must demonstrate a respectful and professional approach to the skills equipment and body models, fellow students, and instructors.

Section VII.02 Skills Lab Rules- The following rules will be enforced for the skills lab:

(a) All nursing students are required to demonstrate competency performing nursing skills in each skills lab to receive a grade of Pass. Students who do not demonstrate skills with proficiency, as assessed by the skills instructor, will be scheduled to return to the skills lab until he/she demonstrates competency or is determined to have earned a failing score.

(b) Students who do not complete the skills assignments will not be allowed into the clinical setting.

(c) Any student may request to schedule extra practice time in the skills lab after scheduled class time.

(d) No student may perform, nor allow to have performed on them, any invasive procedure. This includes, but is not limited to, needle and IV sticks and placing any type of catheter internally.

(e) No student may access the simulation and skills equipment without an instructor present. (f) Rules for using the simulation and skills equipment are posted in the lab. Students who

do not follow these rules may be asked to leave the skills lab and face disciplinary action. (g) The above is true for all high-tech interactive/virtual equipment and software. (h) The skills lab is not an outpatient clinic. Students may not use the skills lab or PNP

medical equipment to take vital signs, or perform assessments or procedures on anyone outside the Practical Nursing Program, unless specifically arranged by an instructor.

Section VII.03 Clinical Requirements- Clinical gives students an opportunity to practice nursing care with direct supervision of the clinical instructor. This helps the student gain a feel for real world nursing experiences, and provides an opportunity for students to put into practice the skills, knowledge and critical thinking they have developed in the classroom. Working with real patients and medical facility staff takes a commitment from the student to practice within their scope, keeping safety at the forefront of all actions. The following rules must be followed at the clinical site:

(a) Clinical objectives are listed in the syllabus distributed by each clinical instructor. Clinical evaluations are based on the clinical objectives, and will be different from one instructor or clinical site to another.

(b) Students will be expected to arrive at the clinical site prepared to care for their assigned patients. To accomplish this, in many cases, students will need to gather information on the patient the day prior to clinical for the week. See individual syllabus for details.

(c) Students will wear program name tag and be professionally dressed at any time they are at the clinical site, whether in uniform scrubs for clinical shift, or to gather patient data prior to clinical. No shorts, tank tops/halter tops, or other unprofessional attire.

(d) Students will not perform invasive procedures or dispense medications to patients without the presence of a nursing instructor or as directed by the nursing instructor.

(e) Students may not perform invasive procedures on each other in the clinical setting. This includes initiating IVs, phlebotomy, glucose monitoring, etc. Students who violate this will be terminated from the training program.

(f) Failing to pass the clinical component of a course will result in the student being required to repeat and pass the course in its entirety before progressing in the Program. See Section 5.01

(g) Any incident occurring in a clinical area resulting in an incident report will require documentation and signatures by the clinical instructor and the nursing student. Students will abide by the Code of Conduct and the PNP rules any time they are functioning in the student nurse role. This includes clinical and any off-campus site students go to for the purpose of meeting objectives of the program.

(h) By Arizona State Board of Nursing rules, a nursing clinical instructor may have no more than ten students on the nursing unit.

(i) EVIT PNP has a limited number of nursing clinical instructors and assigned clinical sites available for student instruction. There may be times when a clinical assignment must be changed with very short notice due to unexpected clinical situations. Each student must give his/her instructor current telephone contact information to assist the instructor to notify students of a change in assignments. It is the student’s responsibility to inform the nursing office and the instructor when his/her telephone number changes.

(j) Previous work or school experiences may not be used to substitute for a clinical. Nursing students are required to demonstrate competence in meeting all clinical objectives. The clinical instructor has the responsibility to determine if the student has passed the course or must repeat the course for failing to meet the clinical objectives.

(k) It is a violation of the Student Code of Conduct for the nursing student to refuse a patient assignment, to leave the clinical site without notifying the instructor, and/or to argue with or confront the nursing instructor or members of the nursing or medical staff.

(l) If the clinical instructor requests that a student leave a clinical assignment, the student must leave. Prior to returning to the assignment the student will meet with the clinical instructor and the Nursing Program Administrator to develop a plan for the student to return to the clinical assignment.

(m) If a student is not allowed by a clinical facility to practice as a student in that facility, the student may not continue enrollment in the nursing program.

(n) Nursing students may not visit a clinical facility in uniform or with their EVIT PNP nursing name badge when he/she is not completing a clinical assignment. The nursing program discourages students visiting patients when the student is “off duty.”

(o) Cell phones must be turned off or to vibrate only, and put away. If a personal emergency necessitates that the student be available by phone, the student shall notify the instructor at the beginning of the shift. The emergency call must be taken away from the patient care area and the instructor notified of the call.

Section VII.04 Clinical Evaluation- Clinical evaluations are based upon clinical objectives and guidelines. Nursing instructors have the responsibility of evaluating a student’s clinical performance. Nursing clinical instructors will complete clinical evaluations for students enrolled in his/her clinical assignment. Students will receive a copy of the clinical evaluation at the end of the course and as needed.

Section VII.05 Clinical Remediation- To successfully complete a clinical assignment, nursing students are required to demonstrate competence as outlined in the syllabus for that clinical. If a student demonstrates unsatisfactory or unsafe clinical performance, he/she will be referred for mandatory remediation. Mandatory remediation may include:

(a) Completing mandatory skills practice in the nursing skills laboratory with an instructor(b) Evaluation of clinical performance by another clinical instructor(c) Clinical probation(d) Withdrawal from the program if remediation attempts fail

Section VII.06 Clinical Probation- Clinical probation is an agreement between the student and clinical instructor that the student will meet agreed upon guidelines within a given time period.

(a) This can include but is not limited to: (i) Mandated time practicing in the skills lab

(ii) A chance to return to the clinical site with the instructor for further patient care opportunities.

(iii) Completion of an assignment of the instructor’s choosing. (b) Students may be placed on clinical probation for the following reasons:

(i) One or more incidents or near misses(ii) Failure to meet the competencies of the clinical

(iii) Attendance falling below 90% in clinical rotation

Section VII.07 Clinical Appearance Requirements- All nursing students are required to adhere to the uniform standards of the EVIT PNP at the clinical site, without exception. The cost of the uniform is not included in the tuition. Please refer to the clinical environment dress code (Article IX).

Article X. HEALTHCARE ENVIRONMENT REQUIREMENTS

Section X.01 This nursing program and the nursing profession are demanding. Students pursuing a career in nursing must consider the physical, mental, and emotional requirements to complete the program. Some of these requirements include the ability to:

(a) Be comfortable with the sights, sounds, and smells encountered in the health care environment.

(b) Have the ability to lift at least 25 pounds and be able to bend and stoop. (c) Respond positively to mental and emotional stressors related to providing nursing care to

patients. (d) Respond positively when interacting and communicating with clients and members of the

nursing staff and the health care team.

Article XI. TRANSPORTATION

Section XI.01 Students are responsible for arranging their own transportation to and from EVIT PNP and all clinical assignments. Bus schedules are available online at http://www.valleymetro.org/. The nursing program will not be able to make clinical assignments based on a student’s individual transportation needs.

Article XII. HEALTH CONDITIONS

Section XII.01 Pregnancy(a) The condition of pregnancy will not prevent a student from enrollment, admission to, or

continuation in the nursing program. (b) It will, however, be necessary for a pregnant student to submit a letter from her licensed

health care provider, stating the she may participate in the activities of the nursing program, including clinical requirements.

(c) Students who do not provide this information will not be scheduled for a clinical assignment.

(d) Following delivery, a student must submit a written release from her licensed health care provider stating that she may return to participate in all training activities, including clinical assignments. The release must state any restrictions.

Section XII.02 Illnesses(a) If a student has a temporary condition that inhibits or restricts class or clinical attendance,

or participation in clinical activities, he/she may be asked to supply a written explanation from a licensed health care provider.

(b) Following hospitalization, serious illnesses, or any other similar condition, a medical release from a licensed health care provider will be required before the student will be allowed to return to the classroom and/or clinical assignments. The medical release must state any restrictions. In order for accommodations to be provided, the student must request it ahead of time.

Section XII.03 Disability Services/Accommodations- EVIT Practical Nursing Program complies with the American with Disabilities Act (ADA) of 1990 and section 504 of the rehabilitation act of 1973 as amended, as well as other applicable federal and state laws and regulations that prohibit discrimination on the basis of disability. A student with a disability must be able to provide documentation of the disability, in the form of a letter from a medical doctor, psychiatrist, or psychologist to the EVIT Disability Coordinator for proper accommodation plans.

Article XIII. DRUG TESTING

Section XIII.01 Pre-Clinical Drug Testing (a) In accordance with the industry standards for nursing, a random drug screen is required at

the student’s expense prior to start of clinical experience. Payment will be made to the Lab at time of service.

(b) Written verification by the physician of any prescription drugs the student is currently taking should be submitted to the testing personnel and/or EVIT PNP Administrator if requested due to positive drug screen.

(c) Only students with negative results on the drug screen will be allowed to remain in the Practical Nursing Program. Students that test positive for illegal substances, non-prescribed legal substances, or substances deemed clinically unsafe for the clinical setting will not be permitted to continue in the program.

(d) Once notified of the date and time of the drug testing, students have 24 hours to complete the drug screen request. Students absent on the day of drug screen notification will be notified by the Nursing Program Administrator or Instructor and must comply within 24 hours.

Section XIII.02 “For Cause” Drug Testing:(a) When a faculty/clinical instructor perceives the odor of alcohol or observes behaviors

such as, but not limited to, slurred speech, unsteady gait, or confusion, and these behaviors cause the faculty or clinical instructor to suspect the student is impaired by a substance, the following steps are taken:

(i) The instructor will remove the student from the campus or clinical area and notify the appropriate supervising personnel. Corroboration of the student behavior will be requested from the supervisor or other qualified medical personnel for validation and for the incident reporting process.

(ii) The student’s listed emergency contact will be called to pick up the student for transport. Students impaired by substances will not be allowed to drive themselves.

(iii) Contact information for the designated drug testing facilities will be provided to all students. If requested to leave a campus or clinical site for cause testing, students have 2 hours to appear at the drug testing facility for testing at their own expense. The student will use their EVIT PNP picture ID for identification at the drug testing facility.

(iv) The Nursing Program Administrator will be informed of the student’s actions and procedures taken by the faculty.

(b) Following for cause drug testing, the student must meet with the Program Administrator before being given clearance to return to class or clinical. If the results of the drug test are positive, the student will be exited from the program. Recommendations for counseling intervention will be offered. If the results of the test are negative for drugs, alcohol, or other illegal substances, or for non-prescribed legal substances, the student will meet and discuss the circumstances surrounding the impaired behavior. Based on the information provided in this meeting, and further medical evaluations if warranted, the Program Administrator and/or the clinical instructor will make a decision regarding the student’s return to the campus and/or clinical setting.

(c) If a student refuses “for cause” testing(i) The instructor will remove the student from the campus or clinical setting pending a full

investigation.(ii) The student must meet with the Program Administrator prior to attending further

clinical or classroom experiences.

(iii) Failure to comply with any aspect of this policy will result in withdrawal from the program.

Article XIV. PROGRAM RESOURCES

Section XIV.01 Textbooks & Reference Materials (a) Obtaining required textbooks is the responsibility of the student.(b) Students will receive a list of the required textbooks when they are accepted into the

program. Students are encouraged to take extra precaution to protect textbooks, training supplies and personal belongings.

Section XIV.02 Related reference materials- Videos, magazines and other learning material are available in the EVIT PNP Library. Audiovisual programs and computer-assisted instruction may be used as instructional resources during the nursing program. These resources will be located in the nursing office, the resource center, or in the skills laboratory.

Section XIV.03 Simulation- Training may include working with low and medium fidelity equipment and mannequins incorporated in scenario based skills components.

(a) Students will utilize these resources, as guided by an instructor, in a manner that is appropriate, safe, and respectful to both the equipment, and staff/fellow students.

(b) Failure to abide by skills lab guidelines and EVIT PNP Student Code of Conduct will result in disciplinary measures in accordance with established policies.

Section XIV.04 Computers- Students will need to use computers throughout their training. It is the responsibility of the student to obtain and maintain access to a computer, printer, and related technology materials throughout the program. Students will demonstrate the use of the systems including but not limited to:

(a) Microsoft Word (b) Microsoft PowerPoint (c) Internet Explorer

Article XV. RECOMMENDATIONS FOR SUCCESS:

Section XV.01 Students must actively participate in the program to be successful.

Section XV.02 It is recommended that students: (a) Work no more than 20 hours per week (b) Maintain 90% attendance (c) Attend all nursing classes, labs, community, and clinical assignments. (d) Communicate with assigned instructors when experiencing difficulties or with questions.(e) Work cooperatively with members of the nursing class cohort. (f) Spend no less than 40 hours per week (including the training time) completing reading

assignments and additional study/learning activities.

Article XVI. SAFETY

Section XVI.01 Students will be closely monitored and evaluated during clinical and scheduled skills labs for safe practice.

Section XVI.02 Reportable Incidents & Near-Miss:(a) Examples of incidents include patient injury as a result of a fall or as a result of moving

the patient during care, and any medication administration error. Any injury to the student which occurs in the classroom, skills lab, or clinical site is considered an incident.

(b) Examples of a near miss include failure to practice the “five rights” of drug administration, failure to calculate a drip rate correctly, and failure to notify the nurse or instructor of a change in the patient’s condition.

Section XVI.03 Incident Rules and Process(a) Students must avoid any actions or procedures that are outside the LPN scope of practice,

have not been approved by the instructor, or are performed in a manner inconsistent with standard nursing knowledge.

(b) Should an incident or near miss occur in the clinical setting, an incident form will be completed and placed in the student’s file, and discussed with the student. Safety issues will be evaluated, and the student may be required to complete extra training to be eligible to continue in the nursing program.

(c) All incidents must be reported to the nursing clinical instructor or the nursing classroom instructor at the time of the accident/incident. Appropriate forms must be completed and turned in to the EVIT PNP office. School security officials will be notified if there is an injury or accident on campus.

(d) The student will be responsible to pay the costs of emergency room or clinic visits, physician visits, and follow-up care as a result of injury or illness occurring in the clinical or classroom setting.

Section XVI.04 Exposure to Blood and Body Fluids- In the course of working in the healthcare environment, there is an increased risk of exposure to blood or body fluids. To decrease the chance of exposure, Standard Precautions should be taken with all clients at all times.

(a) Contaminated sharps shall not be bent, recapped, or removed. Shearing or breaking of contaminated needles is prohibited.

(b) Contaminated sharps must be placed in appropriate container as soon as possible.(c) Eating, drinking, smoking, applying cosmetics or lip balm, and handling contact lenses

are prohibited in the work area where there is a likelihood of occupational exposure.(d) Students must wear appropriate protective clothing/equipment when performing any

task(s) that may involve exposure to body fluids.(e) Any direct exposure to body fluids occurring while functioning as a nursing student must

be reported immediately to the Clinical Instructor.

(f) Students exposed to body fluids shall follow this protocol:(i) Wash the area immediately with a disinfectant agent; for eye splashes rinse the area

with clean water for a minimum of ten minutes.(ii) Report the incident to the Clinical Instructor.

(iii) Students should call their primary care provider. Students participating in the Practical Nursing program, will be financially responsible for any charges incurred resulting from medical treatment or services, while on campus or training at an off campus site. It is recommended that the student obtain their own medical insurance coverage. The EVIT PNP is not responsible for personal injury or loss of property on campus or training at off campus sites.

(iv) Students requiring medical attention while at the school site or at the clinical site should contact their primary care physician for instructions. The student will be responsible for any charges incurred.

(v) The Clinical Instructor and student will notify the agency department supervisor of the injury immediately.

(vi) The student will complete an agency incident report and follow agency protocol.(vii) The student will complete an official school student accident/incident report form.

(viii) The student will file a report of the incident with the Practical Nursing Program Administrator.

PROGRAM COMPLETION

The EVIT Practical Nursing Program is designed to prepare the student to obtain employment as an LPN. The Arizona State Board of Nursing regulates and approves AZ nursing licensure and certification programs. Neither employment nor passing of the licensing/certification exams nor licensure/certification from the State Board of Nursing is guaranteed by the EVIT Practical Nursing Program. Completion of the program is not a guarantee of eligibility to sit for state licensure examination in this or any state. PNP completers will be eligible to complete an application for taking the licensing/certification exams, and be eligible to complete an application to be licensed for practice as a LPN in the State of Arizona.

Nursing Completion CertificateSix (6) weeks before the student expects to graduate, the student should communicate directly with an Enrollment Advisor by submitting an Application for Program Certificate of Completion to determine if all the requirements for program completion have been met. The nursing program Administrator will review the student’s records and confirm the date of completion. In order to be considered as a program completer the student must have met all tuition, fees, attendance, and coursework requirements in full. Following confirmation of completion status, the EVIT PNP office will complete the process to prepare the student’s certificate and forward the appropriate forms to the Arizona State Board of Nursing. Program completers may obtain their EVIT PNP certificate of completion within five business days following their confirmed date of completion.

EmployabilityDuring the last course, Transition to Practice, training in the employment skills needed to implement his/her immediate plans for employment will be provided. This will include a review of the application process, interview techniques, updating the student’s resume and active job search activities.

State Testing and Licensure Application ProcessIt is the responsibility of the student to correctly submit the required forms and fees for NCLEX-PN® examination, Board of Nursing licensure application, fingerprinting, and any other forms necessary for licensure. During the final course of the Program Instructors will assist students to obtain and complete the application for the National Council Licensure Examination-Practical Nursing (NCLEX-PN®) and the application for licensure with the Arizona State Board of Nursing. Specific questions should be discussed with a Board of Nursing representative. National Council Licensure Examination-Practical Nursing (NCLEX-PN®) State Board Examinations for the PN license are offered through Pearson Vue and are taken by the Computer Adaptive Testing method (CAT).

Practical Nursing Licensure—ArizonaNursing students should submit completed application to the Arizona State Board of Nursing (ASBN) following the guidelines and instructions found in the application packet. These requirements are set by the Arizona State Board of Nursing and are provided to students as required by the ASBN. Licensure processes in other states may vary. This nursing program and the nursing profession in general are physically, mentally, and emotionally demanding. At minimum students must be physically capable of lifting patients, standing for several hours at a time, performing bending, carrying and pushing/pulling activities. Students should expect to lift patients and push/pull equipment with patients, ie hospital beds. Gross motor abilities, manual dexterity and perceptual/sensory ability sufficient to provide safe nursing care and monitoring of patients are necessary. Examples of nursing care include but are not limited to palpation of pulses, auscultation and evaluation of bowel and respiratory sounds, calculation and administration of parenteral medications, dealing with death and dying, demonstration of rational and professional behavior under stressful conditions, and participation in client education and conflict resolution activities. Students who have a chronic condition must maintain their prescribed treatment and be able to perform at minimum the essential functions outlined above.

RISKS INVOLVED IN PROGRAM: I understand that the clinical training environment for this Program in which I am enrolled through East Valley Institute of Technology contains exposures to risks inherent in activities of the Program such as but not limited to bodily injury, communicable and infectious diseases, and property damage.

HEALTH AND SAFETY: I have been advised to consult with a medical doctor regarding my personal medical needs. I state that there are no health-related reasons or problems that preclude or restrict my participation in this Program. I have obtained the required immunizations. I recognize that East Valley Institute of Technology is not obligated to attend to any of my medical or medication needs, and I assume all risk and responsibility therefore. ZERO TOLERANCE POLICYEast Valley Institute of Technology Nursing Program supports a Zero Tolerance Policy for the following behaviors:

Intentionally or recklessly causing physical harm to any person on the campus or at a clinical site, or intentionally or recklessly causing reasonable apprehension of such harm.

Unauthorized use or possession of any weapon or explosive device on the campus or at a clinical site.

Unauthorized use, distribution, or possession for purposes of distribution of any controlled substance or illegal drug on the campus or at a clinical site.

A Nursing Program student engaging in this misconduct is subject to immediate dismissal from nursing classes and disciplinary action as described in the Student Handbook.

HEALTH DECLARATION: It is essential that nursing students be able to perform a number of physical activities in the clinical portion of the program. At a minimum, students will be required to lift patients, stand for several hours at a time and perform bending activities. Students who have a chronic illness or condition must be maintained on current treatment and be able to implement direct patient care. The clinical nursing experience also places students under considerable mental and emotional stress as they undertake responsibilities and duties impacting patients’ lives. Students must be able to demonstrate rational and appropriate behavior under stressful conditions. Individuals should give careful consideration to the mental and physical demands of the program prior to making application.

All students entering the Practical Nursing Program must meet all Health and Safety requirements to maintain enrollment status for nursing courses. Students will meet these requirements by providing the Statement of Understanding, with all documentation attached including a copy of your Health Provider CPR card, and the signed Medical Clearance Form to the Nursing Department as directed.

DRUG SCREENING: All students are required to submit to a urine drug screening laboratory test. Only those students receiving negative drug screens, as reported by the Medical Review Officer (MRO), will be permitted to maintain enrollment in nursing courses.

WAIVER OF LICENSURE/CERTIFICATION GUARANTEE: Admission or graduation from the EVIT Practical Nursing Program does not guarantee obtaining a license or certificate to practice nursing. Licensure and certification requirements and the subsequent procedures are the exclusive right and responsibility of the Arizona State Board of Nursing. Students must satisfy the requirements of the Nurse Practice Act: Statutes, Rules and Regulations, independently of any college or school requirements for graduation.

Pursuant to A.R.S. § 32-1606(B)(17), an applicant for professional or practical nurse license by examination is not eligible for licensure if the applicant has any felony convictions and has not received an absolute discharge from the sentences for all felony convictions. The absolute discharge must be received five or more years before submitting this application. If you cannot prove that the absolute discharge date is five or more years, the Board cannot consider your application. All nurse and nursing assistant applicants for certification and licensure will be fingerprinted to permit the Department of Public Safety to obtain state and federal criminal history information. All applicants with a positive history are investigated. STUDENT INTERNET POLICY AND GUIDELINESUSE OF TECHNOLOGY RESOURCES

IN INSTRUCTIONEducational Purpose

The Network has been established for a limited educational purpose. The term “educational purpose” includes classroom activities, career development, and limited high-quality personal research.

The Network has not been established as a public access service or a public forum. The East Valley Institute of Technology District No. 401, hereafter referred to as the District, has the right to place reasonable restrictions on the materials students will access or post through the System. Students are also expected to follow the rules set forth in the District Student Code of Conduct rules and the law in their use of the Network.

Students shall not use the Network for commercial purposes. This means they shall not offer, provide, or purchase products or services through the Network.Students shall not use the Network for political lobbying. But, they may use the System to communicate with elected representatives and to express their opinion on political issues.

Student Internet Access

Secondary students may obtain an individual account with the approval of their parents/guardians and the School.

Students and their parents must sign an Account Agreement to be granted an individual account on the Network. This Agreement will be renewed on an annual basis. Parents/Guardians of secondary students may withdraw approval at any time.

Post-secondary students may obtain an individual account and must sign an Account Agreement to be granted an individual account on the Network. This Agreement will be renewed on an annual basis.

Unacceptable Uses

EVIT Media Users will not post any personal or class developed (web, video, photos, graphic design,

etc.) on the Internet.

Users will not post any media on the Internet that identifies EVIT, the EVIT logo or any other identifiable representations.

Users will consult with the Program Instructor and/or Department Chairperson to receive prior permission to post media.

Personal Safety

Users will not post personal contact information about themselves or other people. Personal contact information includes address, telephone, school address, work address, et cetera.

Users will not use chat rooms or messaging communications, i.e., AOL Instant Messaging, when it is not school related or authorized by the teachers.

Users will not agree to meet with someone they have met on-line.

Users will promptly disclose to their teacher or other school employee any message they receive that is inappropriate or makes them feel uncomfortable.

Illegal Activities Users will not attempt to gain unauthorized access to the Network or to any other

computer system through the Network, or go beyond their authorized access. This includes attempting to log in through another person’s account or accessing another person’s files, or engaging in “hacking”. These actions are illegal, even if only for the purpose of “browsing”.

Users will not make deliberate attempts to disrupt the computer system performance or destroy data by spreading computer viruses or by any other means. These actions are illegal.

Users will not use the Network to engage in any other illegal act, such as arranging for a drug sale or the purchase of alcohol, engaging in criminal gang activity, threatening the safety of others, et cetera.

System Security

Users are responsible for the use of their individual account and should take all-reasonable precautions to prevent others from being able to use their account. Under no conditions should a user provide their password to another person.

Users will immediately notify the IS Department (Support) if they have identified a possible security problem. However, users will not go looking for security problems, because this may be construed as an illegal attempt to gain access.

Users will avoid the inadvertent spread of computer viruses by following the standard virus protection procedures established in Policy IJNDB.

Users will not download software into District computers without expressed permission from the IS Director.

Inappropriate Language Restrictions against Inappropriate Language apply to public messages, private

messages, and materials posted on Web Pages.

Users will not use obscene, profane, lewd, vulgar, rude, inflammatory, threatening, or disrespectful language.

Users will not post information that, if acted upon, could cause damage or a danger of disruption.

Users will not engage in personal attacks, including prejudicial or discriminatory attacks.

Users will not harass another person. Harassment is persistently acting in a manner that distresses or annoys another person. If a user is told by a person to stop sending them messages, they must stop.

Users will not knowingly or recklessly post false or defamatory information about a person or organization.

Respect for Privacy

Users will not re-post a message that was sent to them privately without permission of the person who sent them the message.

Users will not post private information about another person.

Respecting Resources Limits

Users will use the System only for educational and professional or career development activities (no time limit), and limited, high-quality program related personal research.

Users will not download large files unless absolutely necessary. If necessary, users will download the file at a time when the System is not being heavily used. The file will be removed from the System at the earliest opportunity.

Users will not post chain letters or engage in “spamming”. Spamming is sending an annoying or unnecessary message to a large number of people.

Users will check their e-mail frequently and delete unwanted messages promptly.

Users will subscribe only to discussion group e-mails that are relevant to their education or career development as determined by the instructor.

Plagiarism and Copyright Infringement

Users will not plagiarize works that they find on the Internet. Plagiarism is taking the ideas or writings of others and presenting them as if they were original to the user.

Users will respect the rights of copyright owners. Copyright infringement occurs when an individual inappropriately reproduces a work that is protected by a copyright. If a work contains language that specifies acceptable use that work, the users shall follow the

expressed requirements. If the users are unsure whether or not they can use a work, they shall request permission from the copyright owner. If there are questions, ask a teacher.

Inappropriate Access to Material

Users will not use the District System to access material that is profane or obscene (pornography) that advocates illegal or dangerous acts, or that advocates violence or discrimination towards other people (hate literature).

If users inadvertently access such information as described in Policy IJNDB, then they shall immediately disclose the inadvertent access following District procedures established in that policy. This will protect users against allegations that the policy has been intentionally violated.

Parents of secondary students using the Network should instruct users if there are additional materials that they think would be inappropriate for their access. The District fully expects that students will follow their parents’ instructions in this matter.

Student RightsFree Speech

Users’ rights to free speech, as set forth in the District student conduct regulations, apply also to their communication on the Internet. The Network is considered a limited forum, and therefore, the District may restrict the users’ speech for valid educational reasons. The District will not restrict the users’ speech on the basis of a disagreement with the opinions being expressed.

Search and Seizure

Users should expect only limited privacy in the contents of their personal files on the District System and records of their on line activity.

Routine maintenance and monitoring of the Network may lead to discovery that a user may have violated Policy IJNDB, the District student disciplinary code, or the law.

An individual search will be conducted if there is reasonable suspicion that a user may have violated Policy IJNDB, the District student conduct regulations, or the law. The investigation will be reasonable and related to the suspected violation.

Secondary users’ parents have the right at any time to see the contents of their child’s/children’s e-mail files.

Due Process

In the event there is an allegation that a student has violated the District’s Policy IJNDB and its accompanying regulation, the student will be provided with a notice and opportunity to be heard in the manner set forth in the student disciplinary code.

Disciplinary actions will be tailored to meet specific concerns related to the violation and to assist the student in gaining the self-discipline necessary to behave appropriately on an electronic network. If the alleged violation also involves a violation of other provisions of the student disciplinary code, the violation will be handled in accordance with the applicable provision of the code. Additional restrictions may be placed on the use of the student’s Internet account.

Limitation of Liability

The District makes no guarantee that the functions or the services provided by or through the District System will be error-free or without defect. The District will not be responsible for any damage you may suffer, including but not limited to, loss of data or interruptions of service. The District is not responsible for the accuracy or quality of the information obtained through or stored on the System. The District will not be responsible for financial obligations arising through the unauthorized use of the System. Parents can be held financially responsible for any harm to the System as a result of intentional misuse.
