It also includes a cumulative summary of all safety and security incidents and complaints for the last twelve month period. Breakdown of incidents for 1st quarter 2017 (January to March) Security

Description Explosives involved

Outcome/Possible causes

Explosives Discrepancy (5 incidents) 1 x case of explosives found to be missing after transportation.

Shaped charges

Analysis of delivery found paperwork error to be cause.

Fully intact and sealed pallet of explosives found to be one unit short.

Boosters Investigation ongoing. All indications point to an issue at the packing factory.

Whilst unloading a pallet load of explosives it was identified that the pallet was one bag short.

Ammonium nitrate fuel oil

Investigation ongoing. Findings indicate pallet delivered 1 bag short.

Box of explosives incorrectly delivered.

Detonators Investigation completed. Confirmed that extra product delivered.

Explosives unaccounted for during routine stocktake.

Blasting explosives

Investigation ongoing.

Unsecured explosives (6 incidents) Live explosives left on blast bench overnight after shot crew evacuation.

Booster Site investigation completed.

Explosives magazine keys left overnight in shotfirer’s vehicle.

Other Site investigation completed. Magazine reconciliation showed no loss or theft.

During shot reconciliation a discrepancy of 1 detonator was identified.

Detonator Operations ceased immediately. Search located the unit in discarded packaging destined for onsite waste.

Explosive cartridge found on floor of underground light vehicle after blasting.

Blasting explosives

Investigation completed. Follow up inspection to be conducted to review blasting process and security of explosives.

Main gate of explosives storage facility found open.

Security sensitive ammonium nitrate

Audit of facility security found no issues. Determined that lock had become loose and sprung open. All locks replaced.

Explosives requiring storage stored in 2 x unsecured areas overnight.

Ammonium nitrate emulsion

Investigation ongoing.

Flyrock (1 incident) Flyrock impacted crib hut 200 metres from blast. No injuries.

Building was on limit of equipment exclusion zone. Site investigation into cause of flyrock.

Fume event (2 incidents) Level 5 fume event generated. Fume contained within fume management zone.

Nil exposures. Nil unplanned evacuations.

Other (1 incident) Non-electric lead in line ignited spilt oil from a broken down drill rig.

Detonators Drill and Blast engineer responsible for ordering was unaware of magazine licence capacity. A review of licence capacity has now been included prior to ordering and again at confirmation of delivery date.

Suspicious activity (2 incidents) Unidentified airborne drone observed entering airspace above explosives storage facility.

Other Investigation completed. Review of procedures for further instances.

Damage to fence of reload compound discovered.

Security sensitive ammonium nitrate

No evidence of security breach or suspicious activity. Damage thought to be caused by wildlife.

Unauthorised possession of explosives (1 incident) 3 x containers of explosives transported without authorisation.

Ammonium nitrate prill

Site have implemented inspections of deliveries entering and exiting site as an interim measure until investigation has been finalised.

Unintended initiation (2 incidents) Premature detonation due to fault in electronic firing system.

Electrical firing system

Testing firing device. During continuity test current functioned explosives.

Ignition of gas after coal shot. During night shift inspection a small section of open flame observed.

Blasting explosives

Area could not be accessed. Flame was monitored and later self-extinguished.

Vehicle incident (4 incidents) Overheated Brake Booster. Smoke/Steam was observed coming from the brake booster.

Manufacturing residue only

Nearby water truck hosed down vehicle. Site emergency procedures activated and exclusion zone implemented until area cleared.

Mobile processing unit dragged an incline auger 10 metres after becoming caught on upper tank rails.

Security sensitive ammonium nitrate

No adverse results.

Hydraulic Oil Spill in Ammonium Nitrate Shed. Loader was loading into a kettle when trainer assessor for operator noticed a trail of hydraulic oil coming from under steps of loader. Oil spill measured 10 metres long.

Ammonium nitrate prill

Clean up completed without incident. Incident investigated.

Strap retaining a loaded bulka bag became loose and was caught under a trailer wheel, causing a puncture to bag.

Ammonium nitrate prill

Approximately 1 kg of product released from damaged bag. Leak contained and investigated.

CommunityOther (1 incident) Person injured by initiation of gunpowder while angle grinding.

Propellant powder

Person received second degree burns. Investigation completed.

The tail on a tie down strap has come loose while transporting explosives. As vehicle has turned, the loose strap has been caught under the rear wheel, causing the strap to overtighten, tearing a bag of product. Community Other (1 incident) Debris from fireworks display found in local public park.

Drill into/near explosives (1 incident) Whilst conducting ground support drilling in the roof of the drive the drill has intersected a bridged blast hole remnant containing emulsion explosive.

Detonators Incident to be reviewed in toolbox meeting.

Excavator operator moved a cable into the blast exclusion zone with potential to push loose rock to fall over the highwall onto loaded holes. Pre-strip supervisor removed barricading and drove onto loaded blast pattern during night shift to investigate a possible exposed edge above the dig wall due to the wall failing. Damage to explosives/packaging (1 incident) A bag of explosives product found to be leaking. All spilt product was cleaned up and disposed of. Drill into/near explosives (1 incident) During ground support installations it was noticed by the crew that a bolt hole had been drilled in close proximity (less than a metre) to a misfired hole. Flyrock (1 incident) Unplanned damage from a small blast occurred when blasting concrete in preparation for the installation of a belt head.
