They collaborate in interactive case-based learning sessions to help primary care providers develop advanced skills and best practices. The Kidney Disease ECHO will help primary care providers improve: • Chronic kidney disease (CKD) detection. • Co-management of CKD with diabetes and hypertension through best practices. • Understanding of cardiovascular morbidity and mortality. • Management of complex CKD patients. EMPOWERING CARE TEAMS TO IMPROVE UNDERSTANDING, AWARENESS AND ENGAGEMENT Prediction, Prevention, Protection • No cost continuing education for health care professionals • Collaboration, support and ongoing learning with specialist physicians and other experts • Patients get better care in their community • No cost to participating sites or individuals HOW DOES IT WORK? Participants will join an interactive online video conference twice per month. They will share and discuss clinical cases related to such topics as: diagnosis, prediction and prevention of cardiovascular risk; diabetes management; non-pharmaceutical treatment of hypertension; medication management of CKD; low sodium diets; empowering patients through self-management; management of edema and anemia; co-management of CKD patients; and when to refer to a nephrologist.
