Die Finger sind bei Kindern mit Apert-Syndrom distal knöchern eng verschmolzen (Abb. 1/2). Für die mittleren Finger bleibt nach den ersten Fingertrennungen wenig eigene Haut und kein Gewebe für die Fingerkuppenrekonstruktion. Nach der Distraktion mit dem Cube-Fix ist ein deutlicher Zugewinn an Haut und Nagelbett- breite erkennbar (Abb.4). Vier Jahre postop. sind alle Finger kräftig und die Endglieder gut geformt (Abb.5).

The cubes offer to be equipped with small K-wires. That is helpful for bone fractures of small children. The unstable open comminuted fracture of the middle phalanx in a 7 year-old boy is positioned in correct axial alignment and stabilized by the Cube-Fix.

Zur queren Weichteildistraktion wird der Distraktor aufgesetzt, eine Madenschraube im blauen Verbindungspin gelöst und mit einer Umdrehung distrahiert (entspricht einem Breitenzugewinn von 0,5 mm), dann Festdrehen der Madenschraube und Abnehmen des Distraktors. Das Distrahieren erfolgt an allen vier Verbindungsstangen. For soft tissue distraction the distractor is put on, a set screw of the blue connection pin is released, and distraction performed by one full turn (i.e.a gain in width of 0.5 mm). The set screw is tightened again and the distractor removed. Dis-traction has to be done at all 4 connecting rods.

Das Ziel, eine Verbreiterung von 20 mm zu erzielen, wurde nach vier Wochen erreicht.

EINSATZ UND ANWENDUNGSGEBIET Dieser Prospekt enthält Informationen zu externen Fixationssystemen zur Anwendung an der Hand, insbesondere an den Phalangen für die Behandlung von Frakturen mit Gelenkbeteiligung. Des Weiteren zur queren Knochen- und Weichteildistraktion bei komplexen Syndaktilien und Frakturen der Mittelhand-knochen und Phalangen.ALLGEMEINE WARNHINWEISE UND VORSICHTSMASSNAHMEN Bisher keine bekannt. Es muss jedoch darauf hingewiesen werden, dass der mehrfache Einsatz von Fixateur-Systemen allgemein zu einer Schwächung von Verbindungen im Fixateur-System führen kann. Hier sind insbesondere kugelige Verbindungen zu nennen, die eingeklemmt werden. Weiterhin muss auf die Problematik der Perforationsstelle hingewiesen werden. Über diese Hautverletzung können Keime in den Körper eindringen, die zu unterschiedlich starken Infektionen führen können. Ein derartiges Keimen wird durch unsach- gemäße Reinigung der Perforationsstelle begünstigt. Weiterhin ist bekannt, dass Nägel oder Schrauben aus Titanmaterial geringere Infektionen aufweisen, als Nägel und Schrauben aus einem medizinisch zugelassenen Stahl.

FIELD OF USEThis brochure contains information about external fixation systems for the hand, especially for the treatment of fractures with joint involvement of the phalanges. Also for the soft tissue and bone distraction in complex syndactylies and fractures of the metacarpal bones and phalanges.GENERAL WARNINGS AND PRECAUTIONS As for now, none. Although it has to be pointed out that, a multiple use of external fixation systems may lead to a weakening of the connections within the system. Spherical, clamped connections are pointed out especially. Furthermore problems according to the perforation area have to be pointed out. Bacteria may enter the body through this perforation, which can lead to infections. Infections may be triggered also by insufficient or false cleaning of the perforation area. Furthermore, it is known that pins, nails and screws made of titanium show a lower infection rate than their steel pendant.

Radiale KlumphandBei der radialen Klumphand liegen durch die Radius- aplasie die Handwurzel und Hand neben und unter dem Ellenende. Die stark verkürz-ten radialseitigen Strukturen verhindern die Einstellung der Handwurzelknochen vor das Ellenende. Ohne Distraktion ist eine Handgelenksaufrich-tung oft nur unter Resektion von Handwurzelknochen möglich. Die Resektion wiederum führt zu einer weiteren Verkürzung des kurzen Unter-arms und zu einer Druckbelastung mit Schädi-gung der distalen Ellenwachstumsfuge.

MitdemCFK-Minifixateurkönnendiekurzenradialseitigen Weichteile verlängert und die Handwurzelknochen spannungsfrei vor das Ellenende verlagert werden.

Radial club handIn radial club hands the carpus is transposed radially and palmarly to the distal ulna. The extremely shorte-ned radial structures hinder positioning of the carpus in front of the ulna. Straight-ening without resection of carpal bones is impossible. Their resection however leads to further shortening of the already shortened limb.

Pressure load and damage to the ulna physis increases.

Using the CFK-Mini Fixator the short radial tissues are lengthened and the carpal bones can be easily put on top of the ulna without tension.

Zwei Endgewindepins werden von radial in die Basis des 2. und 3. Mittelhandknochens einge-bracht. Sie stehen parallel zueinander und senk-recht zur Längsachse des 2. MHK. Zwei weitere Endgewindepins werden von ulnar in der Basis des 5. und 4. Mittelhandknochens verankert.Two end thread pins are inserted into the bases oft the 2nd and 3rd metacarpal. They are posi-tioned parallel to each other and vertical to the longitudinal axis of the metacarpal.Two more end thread pins are set into the bases of 5th and 4th metacarpal.

AndenPinswerdenDoppel-PinhalterundRingfixiert.Standard ist ein 5/8 Ring, optional kann ein Halbring ver-wendet werden. An diesem werden Gewindestangen mit gelenkigen Verbindungen montiert. Die Achse der Gelenke orientiert sich an der Gelenkebene zwischen Elle und Handwurzel. An den Endteilen wird der zweite 5/8 Ring montiert. Der Ring wird an einem von ulnar und senkrecht zurEllenachseeingebrachtenMittelgewindepinfixiert.Double pin locking screws and half ring (standard is 5/8 ring, half ring is optional) are fixed to the pins. Threaded rods are mounted to this ring. Every rod consists of a longer segment, connected with an end piece with thread end. The axis of these joints is oriented at the wrist level between ulna and carpus. The second 5/8 ring is mounted to these end pieces.The ring is fixed to the ulna by a mid-dle thread pin that is installed vertically to the ulna axis.

Zur Rotationssicherung wird ein weiterer Pin mit end-ständigem Gewinde von dorsal eingebracht. Er wird mit einem Pinhalter am proximalen Ring befestigt.For rotation stability a third end thread pin is installed dorsally. It is fixed to the ring by a pin locking screw.
