ART in Breast MilkDefining the Risk: Benefit Ratio
Catriona WaittSenior Lecturer in Clinical Pharmacology, University of Liverpool
Wellcome Postdoctoral Clinical Training Fellow
Symposium at 25th CROI, Boston“Breast Is Best” – but it’s a viral milkshake
5 March 2018
Risks and Benefits of ART whilst Breastfeeding
Risks• Infant exposure to drug• Risk of HIV-resistance if infant
becomes HIV+• Possible ongoing transmission risk• Risk of toxicities to breastfed infant
Benefits• MTCT rates of <1% even in
breastfeeding populations • Improved maternal health• Multiple wider benefits of
breastfeeding
Milk to Plasma Ratio: PIs
Milk to Plasma Ratio
Waitt et al. JAC 2015
PI reach very low concentrations in breast milkProbably due to high degree of protein binding

Conclusies
• “Nieuw” onderwerp: PK HIV medicatie in moedermelk
• Relatief veel HIV farmacologie, maar van beperkt aantal groepen
Hartelijke groeten uit Nijmegen!
