PHASE 1

Begins on the first day of menstrual bleeding, and this usually lasts three to five days.

Following menstruation, a woman normally experiences some days without any signs of fertility.

Time of infertility, and its length will vary with each individual woman, and even from cycle to cycle.

PHASE 2

Begins as soon as a woman’s daily observations detect the onset of signs of fertility

This fertile time lasts until several days after the time of ovulation (when an egg is released from the ovary).

In a normal, healthy woman, Phase II will typically last approximately 8 to 12 days.

PHASE 3

The post-ovulation time

Time of infertility, and typically accounts for the last one-third of a healthy woman’s cycle.

This part of the cycle is generally constant in length from cycle to cycle.

HORMONES

The three phases of the female cycle are the result of hormones.

Hormones are: chemical messengers produced in one part of the body transported by the bloodstream to

another part to bring about some physiological

activity It is the interaction of a number of hormones that produces these three different phases of the female cycle.

4 KEY HORMONES

4 key hormones enable the female reproductive system to function properly.

Two are produced by the pituitary gland, a small gland at the base of the brain:

1. Follicle Stimulating Hormone (FSH)2. Luteinizing Hormone (LH)

Two are produced within the female reproductive organs:

1. Estrogen2. Progesterone

FSH Follicle Stimulating Hormone (FSH) is released by the pituitary gland during Phase l of a woman's menstrual cycle.

This hormone affects the ovaries and initiates the transition process from Phase I to Phase II.

FSH stimulates the immature eggs within the follicles (or egg containers) in the ovaries to develop into mature eggs.

Usually, only one of the eggs fully matures and is ultimately released. (In rare cases, a second follicle will mature and release a second egg, but always within 24 hours of the first.)

It is FSH, then, that initiates the changes in follicles in the ovaries that will eventually result in ovulation.

As the follicle grows and develops, it produces a second key hormone – estrogen.

FSH CONT’D

ESTROGEN

As estrogen increases it causes changes in a woman's body: 1. The glands that lie within the cervix

produce mucus that is essential for sperm survival

2. The cervical os – the opening of the cervix – opens slightly and softens in order to allow sperm to enter

ESTROGEN CONT’D

3. The lining of the uterus – the endometrium – builds up with blood and tissue so that if a pregnancy occurs, the newly conceived child will have an adequate endometrial lining in which to implant

4. A feedback message is sent to the pituitary gland to cause a surge in another hormone from the brain – Luteinizing Hormone (LH)

LUTEINIZING HORMONE

Luteinizing Hormone (LH) acts as a chemical messenger, traveling from the pituitary gland to the ovary to signal the release of the mature egg.

Once the egg is released from the ovary, the follicle (now an empty container) turns into a structure known as the corpus luteum.

The corpus luteum has a new and very important function: in addition to continuing to produce estrogen, it also now produces progesterone.

PROGESTERONE

Progesterone has a number of effects on a woman's reproductive system:

1. Causes the cervix to close and harden

2. Continues to enrich blood and tissue in the endometrial lining

3. Causes the mucus that is being produced by the cervical glands to thicken, dry up, and/or disappear

PROGESTERONE CONT’D

4.Affects a woman's basal body temperature (her waking, resting temperature), which rises and remains elevated for about two weeks until the next menstruation

5. Sends feedback to the pituitary gland to suppress any further ovulations in that cycle

If a pregnancy has not occurred, then the progesterone level remains high until the beginning of the next cycle, when it drops and the brain receives a signal to start a new cycle.

HORMONE COMBINATION

A remarkable orchestration of four hormones – two from the brain (FSH and LH), and two from the ovaries (estrogen and progesterone) –

Enables the female menstrual cycle to function for decades from the time of puberty (onset of menstruation) to menopause (when the female menstrual cycles end).

GRAPH EXPLAINED It is easy to see that estrogen is the more dominant hormone of the two before ovulation; whereas, progesterone (blue line) is more dominant after ovulation. ‘

It is important to note that, although one hormone is dominant in each half of the cycle, both hormones are still present throughout the cycle.

A woman's body continually produces some of each

In fact, in the second half of the cycle there is still a relatively significant amount of estrogen being produced; but in comparison, progesterone production surpasses it, and hence, is the dominant hormone at that time.

LH GRAPH EXPLAINED This graph shows the levels of the pituitary hormones, FSH and LH, during the cycle.

Both of these hormones are at their highest levels during the early and middle parts of the cycle, with FSH (red line) at a higher level than LH very early in the cycle.

Both reach their peak amounts about mid-cycle in a typical 28-day menstrual cycle. In shorter or longer cycles. the peak amounts of FSH and LH would occur approximately two weeks before the onset of the next period.

As ovulation approaches and the levels of estrogen increase, the influence of estrogen produced by the ovary signals the brain to release higher doses of LH (orange line).

It is the surge of LH around mid-cycle that triggers the release of the egg from the follicle in the ovary, or ovulation.

After this point, both FSH and LH play a less prominent role.

CONCEPTION Conception or fertilization occurs when one sperm penetrates one egg-providing a complete set of genetic material (one-half from the mother and one half from the father) to the newly conceived life.

At this stage, the new life is called a blast cyst as it begins to divide and grow.

During this early division process the blastocyst travels down the fallopian tube, and in approximately six to nine days it develops some thread like projections on its surface the enable it to attach to the lining of the uterus.

This attachment process is called implantation, and the new life enters the embryonic stage of development for the next eight week.

After eight weeks, the embryo enters another stage and is called a fetus.

CONCEPTION CONT’D Remember that life begins at the moment of conception.

The terms blastocyst, embryo and fetus describe stages of development before birth, just as infant, toddler and teen describe stages of development after birth.) 

Within eight days after conception, the early developing placental tissue produces Human Chorionic Gonadotropin (hCG) hormone.

This hormone stimulates the corpus luteum to continue producing progesterone for the first 10-12 weeks of pregnancy. Then, the placenta takes over and produces progesterone and the corpus luteum degenerates.
