others, to secure an investment of $100 million for primary care workforce programs in California. This monumental appropriation included in the budget will support and expand primary care residency programs and teaching health centers and help recruit providers to practice in medically underserved areas.

In addition to this budget win, we experienced a highly successful year legislatively. CaliforniaHealth+ Advocates sponsored seven bills this year, four of which made it out of their house of origin and three of which made it to the governor’s desk. This achievement was realized in large part due to the work of our staff, our lobbyists and our members – working together to better our communities and build a healthier California overall.

2016 has been a highly successful year for our organization, our members, and those we serve. We have embarked on the development of our next strategic plan, which will guide our organization into 2020 and the years beyond. Throughout the summer, CPCA held regional meetings throughout the state to gain insight from health center staff on what the organization can to do to support them into the years ahead. Through this process we are developing a vision that is bigger and bolder than anything we have ever done. I’d like to thank all of those who were able to participate in the development of this innovative vision for our future – we couldn’t have done it without you.

I am so proud to see how far this organization has come, and am encouraged by the positive impact it will have in the coming years. We are still a young organization, with much to prove, but working together, like we did this year, we can accomplish great things – working together towards a bolder, brighter tomorrow.

Welcome to the California Primary Care Association (CPCA) 2016 Annual Conference. This year’s

conference theme, Together Towards Tomorrow, couldn’t be more appropriate as we celebrate the accomplishments of the past year and look forward to the opportunities in our future. Since their founding, health center leaders have marched for equality, fought for social justice, and have been leading community activists. It’s always been clear to health centers that if they want to deliver on the

promise of health, it was up to them to advocate for the health of their patients and the health of their communities.

Health centers embraced that notion over twenty years ago when they formed the California Primary Care Association. In doing so, they took their advocacy efforts to another level. It was a transformative decision that created a single unified statewide voice for health centers. CPCA has flourished and is a stronger voice for health centers today than it has ever been before. Our successes have been notable, but our work was not done, and it was time for us to again transform the way we advocate.

In January, the CPCA Board of Directors took a bold step in authorizing the creation of a new 501(c)(4) organization to support the organization’s statewide advocacy efforts. Community health centers have a proud and storied history of advocacy. CaliforniaHealth+ Advocates builds on that legacy and takes a bold step into the future with an organization whose sole purpose is to advocate for community health centers and help them advance their mission through lobbying, advocacy, and meaningful engagement with elected officials and candidates for office. So far this year, we have enjoyed great success through the work of CaliforniaHealth+ Advocates.

It was clear from our members that workforce was a top priority this year and CaliforniaHealth+ Advocates developed a multi-pronged strategy to bring about much needed solutions. That is why the organization joined in collaboration with the California Medical Association and the California Academy of Family Physicians, among

CELEBRATING OUR PAST AND OUR FUTURE Dear Friends and Colleagues,

It has been my pleasure to serve as the California Primary Care Association’s 2015-16 Board Chair and it is my honor to welcome

you all to the 2016 Annual Conference. I am so excited to have this year’s conference in Southern California, offering health center staff an opportunity to attend and learn about best practices and solutions to issues they face on the front lines each day. CPCA’s Annual Conference is the organization’s premier event providing attendees with fresh educational content,

abundant networking opportunities, and access to cutting-edge expertise. This year’s event will not disappoint with renowned keynote speakers, an even bigger tradeshow than prior years, and new, exciting features!

This year’s theme, Together Towards Tomorrow, really resonated with me as we work on the workforce challenges we are all facing. We are fortunate that our state continues to expand Medi-Cal eligibility and that our continued success in educating and enrolling patients into coverage has been remarkable. But, finding the right providers to meet the demands of our growing patient population is not easy and retaining them once they’ve started is no small feat either. In fact, CPCA’s recent Horizon 2030 report estimates that we will need an additional 8,243 additional primary care physicians by 2030 – more than one-third of our current workforce!

While this may seem daunting, it is not undoable. We are already well on our way to making progress in the area of workforce, starting with a significant investment by the Governor and Legislature in the state’s primary care workforce in this year’s budget. While this funding is earmarked for loan repayment, residency programs, and teaching health centers, it’s a step in the right direction. We are now that much closer to

achieving the workforce goals we set out to accomplish at the beginning of the year. And we were able to achieve this success by working together toward a common goal.

Imagine what we can do in the coming years working together on workforce issues! Our neighborhoods are full of bright, caring, and diverse individuals who, if provided the opportunity, are eager to serve their communities and would play a critical role in health care. I see it in my own community, and I’m sure that many of you do as well. We also know that sixty percent of physicians from the National Health Service Corps remained in underserved communities ten years after their service obligation was completed. Let’s build those connections with a health care workforce pipeline in every school in California where we support programs that engage students and get them thinking about health care careers that allow them to give back.

We must also make sure that we have a seat at the table – locally, statewide, and nationally – in building programs that will provide our needed pipeline. We need to bring together those within our communities that can help make our workforce aspirations a reality. We’ve made this issue a priority and we must see it through. We cannot do it alone, we must work together – because together we can accomplish great things.

The Role of California’s Health Centers as We Move Together Towards Tomorrow Sandra R. Hernández, MD, President and CEO, California Health Care Foundation The California Health Care Foundation (CHCF) has been a dedicated partner and champion of the health center movement over the years. Dr. Sandra Hernández will share her vision of what role community clinics and health centers can and should play in the overall health care system to

help all of California move together towards tomorrow and healthier communities. Dr. Hernández is an assistant clinical professor at the University of California, San Francisco, School of Medicine and maintained a clinical practice at San Francisco General Hospital in the AIDS clinic from 1984 to 2016. She is a graduate of Yale University, the Tufts School of Medicine, and the certificate program for senior executives in state and local government at Harvard University’s John F. Kennedy School of Government.

Caring for the Whole Person: A Vision for the Delivery System ROOM 201

There is a growing recognition of the role of comprehensive oral and behavioral health care in meeting the Triple Aim of better outcomes, better patient experience, and lower cost of care. The state and federal governments have begun making enormous investments in both oral and behavioral health care, from the expansion of Medi-Cal behavioral health benefits, to the new Drug Medi-Cal Organized Delivery System Waiver, to the new State Oral Health Plan and the Dental Transformation Initiative in the 1115 Waiver. This session explores possible solutions for integrating care from the top down - breaking down silos between payors and attempting to make the health delivery system integrate care as a reflection of the integration being done at the provider level.

Health Homes for Patients with Complex Needs: What’s Coming and How to Prepare

The Department of Health Care Services (DHCS) will be rolling out Health Homes for Patients with Complex needs (HHPCN) in California counties in 2017 and 2018. For the past 4 years, Partnership HealthPlan of California has built up a prototype for this program, called the Intensive OutPatient Care Management (IOPCM), with 13 sites now participating. This presentation will give an update on the current structure of this prototype, from a health center and health plan perspective, drawing out best practices for sites preparing for implementing HHPCN. A representative from DHCS will give details about the HHPCN program, scheduled to go live in January, 2017.

The Primary Care Workforce Crisis: Funding Solutions to Drive Progress ROOM 201

In early 2016, CPCA released a sobering analysis of our primary care workforce needs while detailing key opportunities to meet the workforce needs of tomorrow. The report estimates that California will need 8,243 additional primary care physicians by 2030 and provides a stark reminder that the primary care workforce shortage has reached a critical point and will continue to devolve if California doesn’t take immediate action. Payment Reform is Coming: Position Yourself for Successful Rate Negotiations ROOM 203

California is moving forward with implementing SB 147 (Hernandez), the Alternative Payment Methodology pilot legislation, and while not all health centers in California will be part of the pilot, all health centers in California should be paying attention to the pilot. All health centers should also be looking at their data and managed care plan arrangements now so that they can make the strategic decisions that will position them for the best launching point once the APM becomes an opportunity for all health centers. This session will review payment reform nationally, the California APM construct, and then describe trends found in a recent APM data modeling exercise. Health centers will leave this session with a to-do list for looking at their data and talking to their managed care plans.

The evidence is clear that at a population level, social, environmental, and behavioral factors dwarf health care access and quality in terms of shaping health outcomes. There are a number of significant initiatives nationally, in California, within integrated health systems, and locally exploring how best to address social determinants of health (SDH) such as housing and economic opportunity as part of health system transformation. There is a robust dialogue underway about what it takes to do this well including necessary capacity- building, data infrastructure, and financing mechanisms. This session will review the current landscape of work on SDH, the potential leadership role for health centers given their history of connection to communities, and the value proposition and competitive advantage of incorporating a substantive focus on SDH into planning and operations.

William “Bill” HobsonWilliam D. (Bill) Hobson is being honored with a CPCA Clinic Legacy Award for his distinguished career as a community health advocate and leader for more than 45 years. While now working as a private consultant, he most recently served as the President and CEO of the Watts Healthcare Corporation (WHCC) in Los Angeles, CA from 2003 to 2015, overseeing a staff of 300 and a budget of approximately $30 million. Prior to WHCC, he held senior management positions in the

Health Resources and Services Administration (HRSA), including serving as the Deputy Associate Administrator of the Bureau of Primary Health Care (BPHC) in the US Department of Health and Human Services, and subsequently as Acting Director of BPHC for two years.

Among his many roles in community health, he also served as the Executive Director for Central Seattle Community Health Centers in Seattle, WA from 1977-1994 and as Human Rights Commissioner for the City of Seattle from 1978-1993. Appointed by the Governor of Washington State, he served as a member of the Washington State Medicaid Advisory Committee from 1985-1992 and from 1993-2000 he maintained an appointment as a Clinical Associate Professor in the University of Washington School of Public Health and Community Medicine’s Department of Health Services.

Mr. Hobson has also been active with the National Association of Community Health Centers (NACHC) representing Region X while in Seattle, and representing Region IX during his tenure at WHCC, representing the region in 2008-2009, and again in 2011, 2013, and 2015. Mr. Hobson has also been active within CPCA, serving as Legislative Committee Chair from 2011-2013 and as Speaker on the Executive Committee during his tenure on the CPCA Board of Directors.

Mr. Hobson received a Master of Science Degree in Environmental Health from the University of Cincinnati and a B.A. in Biology from Earlham College.

2016 CLINIC LEGACY AWARDSBeatrice BostickBeatrice Bostick is being honored with a CPCA Clinic Legacy Award in recognition for her distinguished career in helping to serve culturally diverse and low income populations in Northern California. She has more than 30 years of experience working with community health clinics, with more than 18 years of management experience in her role as CEO of Community Health Clinic Ole (now Ole Health) in Napa and most recently Alliance Medical Center in

Healdsburg, California. Both organizations are Federally Qualified Health Centers (FQHCs) that serve a predominantly farmworker population. After a long and successful career in community health, Ms. Bostick retired earlier this year.

During her tenure, she guided Alliance through great change within the health care system due to the implementation of the Affordable Care Act, outreach and enrollment efforts, and increased patient access. Her work with Clinic Ole included expanding services, growing the service area, and increasing patient access.

Ms. Bostick specialized in financial recovery and service expansion efforts. She received her B.A. in Art History and an MA in Acupuncture and Chinese Medicine. She also studied Egyptology and Archaeology at Wayne State University, hoping to become an explorer or archaeologist. After graduating, she spent more than two years doing health-related work in Togo, West Africa, which inspired her to continue working in healthcare when she returned stateside. She is currently enjoying retirement and lives in Taos, New Mexico, an area known for its artistic community. In her spare time, Beatrice also enjoys travel and writing fiction.

PUBLIC HEALTH LEADERSHIP AWARDAssemblymember Richard Bloom Assemblymember Richard Bloom (D-50th District) is being recognized for his commitment to improving population health and decreasing the disease burden of California’s most vulnerable communities. He was first elected to the 50th District of the California State Assembly in 2012, representing the communities of Agoura Hills, Bel Air, Beverly Hills, Brentwood, Hollywood, Malibu, Pacific Palisades, Santa Monica, Topanga, West

Hollywood and West Los Angeles. The Assemblymember authored Assembly Bill 2782, the Healthy California Fund which called for a dedicated revenue source that addresses the root causes of epidemic levels of preventable diseases caused by sugary drinks, including diabetes. While the bill was ultimately pulled from this year’s legislative calendar, this important issue remains active with support from a growing coalition of those who support the effort.

In just his first few years, Asm. Bloom has also been involved in legislative efforts to combat public health issues, including the prescription drug overdose epidemic, address homelessness, improve services for veterans, expand and protect the rights of the LGBT community, foster local redevelopment and create jobs. He serves on the Assembly Committees on Appropriations, Budget, Business and Professions, Higher Education, and Transportation. For the last three years, he has also served on the Legislative Budget Conference Committee which is a small, select group of legislators tasked with negotiating the state budget with the Governor.

Born in Philadelphia, Asm. Bloom grew up in Altadena and West Los Angeles. He attend the University of California (Berkeley and Los Angeles) and Loyola School of Law and holds a B.A. in Communications and Public Policy, as well as a Juris Doctor degree. He practiced law for nearly 30 years and worked as the Executive Director for a non-profit that assisted low income and homeless clients.

Mary Maddux-González, MD, MPHMary Maddux-González, MD is being recognized with a Clinic Legacy award for her commitment to public health and the community health center movement. Dr. Maddux-González recently retired from the Redwood Community Health Coalition (RCHC), where she was the Chief Medical Officer, a position she has held since 2011. RCHC is a network of 17 community health centers, with over 40 sites in Marin, Napa, Sonoma and Yolo counties serving more than 240,000 individuals each

year – more than 1 out of every 5 people in these communities. She also served as RCHC’s interim CEO for a short time, leading the organization through a transitional period. Prior to joining RCHC, Dr. Maddux- González served as Sonoma County’s Public Health Officer and division director for more than a decade. She also worked at Alliance Medical Center in Healdsburg and at Sonoma County’s public health clinics from 1991-2000.

During her tenure at RCHC, Dr. Maddux-González served on various CPCA committees, workgroups and peer networks. She served as the Chair of CPCA’s Statewide Quality Improvement Committee (SQIC) from 2012-2014 and a Board Member of the Integrated Healthcare Association. Her particular area of interest is in improving the health of underserved communities through the alignment of public health and primary care strategies.

Dr. Maddux-González graduated from medical school at the National Autonomous University of Mexico (UNAM) in Mexico City. She also has a Diploma in Human Nutrition from the London School of Hygiene and Tropical Medicine of the University of London and a Master’s in Public Health from UC Berkeley. Dr. Maddux-Gonzalez completed her residency training at the UCSF-affiliated Santa Rosa Family Medicine residency and is Board Certified in Family Medicine.

Senator Ricardo LaraSenator Ricardo Lara (D-Bell Gardens) has been representing his District since November 2012. Raised in a blue collar immigrant household, the Senator knows first-hand the challenges facing working families and is committed to improving the quality of life for all Californians. Senator Lara is being honored as a CPCA Hero for his efforts in providing coverage for California’s undocumented population and leading the #Health4All movement in our state. He authored

Senate Bill 75, which provides expanded Medi-Cal coverage to undocumented children, and Senate Bill 10 – Expanding Health Care Coverage to All Californians, also known as #Health4All Waiver. Since federal law prohibits undocumented Californians from purchasing their own insurance through Covered California, SB 10 directs the state to apply for a 1332 waiver to allow Covered California to offer alternative insurance products through the exchange to undocumented individuals. The legislation was signed by Governor Jerry Brown earlier this year.

Senator Lara chairs the Appropriations Committee and serves as a member of the Energy, Utilities and Communications, Governance and Finance, Governmental Organization, and Banking and Financial Institutions committees. A champion for educational equity, civil rights and immigrant rights, he champions legislation that improves the quality of life for vulnerable communities. He has authored several first-in-the-nation laws that recognize and invest in immigrant communities such as the California DREAM Loan Program, which helps undocumented students apply for loans to complete their higher education goals.

In addition to public health issues, Senator Lara is also an environmental advocate – creating the California Clean Bus, Truck and Freight Program to encourage clean energy while investing in good jobs, and authored legislation to create a state plan to address short-lived climate pollutants, such as diesel soot, which are known to cause climate warming and negatively impact public health.

Upon winning the 2012 election, Lara became the first openly gay person of color elected to the Senate. The Senator earned his B.A. in Journalism and Spanish with a minor in Chicano Studies from San Diego State University.

2016 HERO AWARDSRay Baxter, Ph.D Ray Baxter, Ph.D, is being recognized for his longtime commitment to community clinics and health centers. Dr. Baxter recently retired from his role as Sr. Vice President for Community Benefit, Research and Health Policy at Kaiser Permanente. Dr. Baxter has more than 40 years’ experience managing public health, hospital, long-term care and mental health programs. As a member of Kaiser’s National Executive Team, Dr. Baxter led the organization’s activities to fulfill its social mission,

including care and coverage for low income people, community health initiatives, health equity, environmental stewardship and support for community-based organizations. He also led Kaiser Permanente’s work in research, health policy and diversity, and served as President of KP International. Under Dr. Baxter’s leadership, Kaiser Permanente increased its Community Benefit investments from $600 million to over $2 billion and became a national benchmark. His leadership of Kaiser’s Community Benefit program has led to continued and unprecedented support of community clinics, health centers, and the regional clinic associations.

Previously, Dr. Baxter headed the San Francisco Department of Public Health, the New York City Health and Hospitals Corporation, and the Lewin Group, a noted health policy firm. Dr. Baxter holds a doctorate from the Woodrow Wilson School of Public and International Affairs, Princeton University. He currently serves on the Advisory Board of the UC Berkeley School of Public Health and the Board of the CDC Foundation. He recently served on the Global Agenda Council on Health of the World Economic Forum and was a member of the Institute of Medicine’s Roundtable on Population Health Improvement and the National Academy of Medicine’s Leadership Consortium for Value and Science-Driven Health Care.

In 2001, the University of California, Berkeley, School of Public Health honored him as a Public Health Hero for his service in the AIDS epidemic in San Francisco. In 2006, he received the CDC Foundation Hero Award for addressing the health consequences of Hurricane Katrina in the Gulf Coast and for his longstanding commitment to improving the health of communities. In 2016, the San Francisco Business Times recognized Dr. Baxter with its first Legacy Award for Corporate Philanthropy.
