1. Introduction

Alpine Construction LLC (Alpine) completed the construction activities for the Nassau Lake Dam (NLD) rehabilitation on May 01, 2009. This Final Inspection Report has been prepared by Kyle F. Swierski, PE, LLC (KFS) to document the NLD site conditions observed on May 01, 2009. This Final Inspection Report represents the culmination of the Construction Progress Reports, prepared and distributed by Alpine in the regularly scheduled NLD progress meetings. The KFS final inspection confirms that the construction for the NLD rehabilitation project has been completed in accordance with the documents approved by the NYSDEC.

2. Site Visit Attendees

3. KFS Final Inspection Notes o Final Inspection Site Photographs taken by KFS on May 01, 2009 are

provided in Appendix A.

o During the KFS final inspection, Alpine was completing the seeding operations for the restoration of the construction easement area and performing final cleanup & demobilization.

o KFS observed the field construction for the NLD rehabilitation to be

complete, including the punchlist items identified during the previous substantial completion inspection on March 12, 2009. There were no outstanding work items noted by KFS, or other site visit attendees. The NLD Final Completion Certificate prepared by KFS is provided in Appendix B.

o Alpine, on behalf of GE, delivered the following to the NLPIA:

o Two identical gate operator handwheels (one is a spare) o Two spare signs each of Type A, B & C (design drawing 008 sheet 3)

o Alpine, as a courtesy, also provided the NLPIA with two timber bulkheads

which Alpine installed in the gate chamber inlets for dewatering. Should the gate chamber require dewatering in the future, NLPIA's contractor should verify the adequacy of the timber bulkheads.
