All first-time borrowers of a Federal Direct Stafford Loan must complete
entrance counseling prior to signing a promissory note or receiving funds.
A first-time borrower is a student who doesn’t have an outstanding Federal
Direct Stafford Loan or Federal Direct Unsubsidized Stafford Loan.
You’ll receive additional information when your award is finalized. Currently
enrolled students will accept awards on BlazerNET, UAB’s secure online
portal. New students who haven’t applied for a Blazer ID will accept awards
on Banner Self Service, our online tool to help you manage your personal
and academic information.
First-time borrowers can fulfill the entrance-counseling requirement online
at www.studentloans.gov . After you successfully complete the entrance
counseling, you will need to wait 48 hours before you can sign your master
promissory note, also at www.studentloans.gov .
If you’re approved for a Federal Perkins Loan, you’ll receive additional
information from Student Accounting Services after your award is finalized
and you’ve accepted the award.
Parents of dependent undergraduate students who want to apply for a
Federal Direct Parent Loan for Undergraduate Students (Federal
Direct PLUS) should complete the PLUS Loan Application at
www.studentloans.gov . You must also complete the Free Application for
Federal Student Aid (FAFSA).

2013-2014 Verification Requirements
1. If you are selected for verification by the U.S. Department of Education you
will be required to submit the following documents so UAB can verify the
information you supplied on the FAFSA. [You can determine if you have
been selected by reading your Student Aid Report (SAR) or contacting the
aid office.]
2. The preferred way to complete your verification requirement is to use the IRS
Data Retrieval Tool at www.fafsa.gov to transfer your 2012 tax return
information directly onto your FAFSA.
3. If you either cannot use or choose not to use the tool the Department of
Education no longer allows Schools to accept a signed copy of tax forms as
proof of income earned and taxes paid. The new Department of
Education regulations beginning in the 2013-14 school year and
subsequent years are as follows:

submit to Student Financial Aid yours and your parent’s Federal
Income Tax Return Transcript directly from the IRS from the
previous year. This Tax Transcript can be requested at www.irs.gov

These worksheets are available through BlazerNET. Dependent
students must make sure that the form is signed by them and a parent.
4. If you, your spouse, or your parents did not file a Federal Income Tax
Return and are required to file, you must submit each of the following:
a) a copy of your Application for Automatic Extension of Time To File
U.S. Individual Income Tax Return (IRS Form 4868), b) a copy of IRS
Form W-2 for each source of employment income received for the
previous tax year or, c) a signed statement by a self-employed
individual certifying the amount of the AGI and the income tax paid for
the previous year.
5. If you, your spouse (if married independent student),and your parents
(if an independent student) did not file a Federal Income Tax Return
and are not required to file, you must submit the applicable IRS Form
W-2 for each person for each source of employment income received
for the previous tax year. You also want to make sure it is so noted on
the Verification Worksheet.
6. After reviewing the documents stated above, often we find the need to
request additional information. We regret the need to do this, but
sometimes we must do so in order to satisfy the federal requirement to
eliminate discrepancies and to ensure the quality of application data.

If your eligible noncitizen status is not confirmed by Immigration & Naturalization (INS),
you must submit proof of eligible non-citizenship to the aid office. An Alien
Registration Card must be presented for review and a copy will be maintained for your
file.

When a decision has been made regarding your eligibility for financial aid, you’ll be
notified by mail or e-mail. If required, currently enrolled and new students will accept
award offers in BlazerNET.
When you register for classes, you create a financial obligation to the university. An
application for financial aid neither eliminates this responsibility nor extends the
deadlines established for payment—you’re still responsible for making sure your
tuition, fees, and other expenses are paid as required and on time, even if you’ve
applied for financial aid.

Financial Literacy
Economic times are tough. UAB is committed to providing students with up-to-date
advice and guidance on financial assistance options and budget-management tools that
can help ease your financial burdens. We’ll update these suggestions and links
regularly.
1. Apply for financial aid today. When financial conditions change, so may
eligibility for financial aid. You have to apply every year to be considered.
Complete your Free Application for Federal Student Aid now.
2. Apply for a scholarship.
Scholarships aren’t just for freshmen. You may apply for some
scholarships if you are a current student. Contact Heather McKnight,
Scholarship Coordinator in Financial Aid, at 205-934-8223 for
assistance or visit us online for more information.

3. Access low-cost loan programs.
The Federal PLUS Loan Program allows parents to borrow funds on
behalf of their student covering the difference between your total cost
of attendance and the amount of other financial assistance you
receive. The interest rate is 7.9 percent. To be eligible for a PLUS
loan, your parents must apply and consent to a credit review by the
U.S. Department of Education. For more information, visit the Office
of Financial Aid online or call 205-934-8223.
4. Get your personal finances in order. You can find information on financing
college, budgeting, managing your credit, and much more with our Green to
Gold basic money management.
5. Save money on
Books.
Why buy when you can rent? Barnes & Noble at UAB Bookstore now
offers a textbook rental program that can save you money on textbooks.
Transportation. You don't necessarily need a car on campus. Try a
loaner: Parking and Transportation Services offers the “UAB Connect
Program” in partnership with Hertz, providing access to cars for on-

with trolleys—hop on the Dart for just 25 cents a ride.
Entertainment. Looking for something fun to do? Students have
plenty of low-cost or free on-campus entertainment options available.

Food. Don't let good food go to waste. Fully utilize your student meal
plan or BlazerBucks.

Home. If you live off-campus alone, consider getting a roommate to
cut expenses.

All returns will be distributed to the student financial assistance programs in the order
below with the following exceptions. Title IV funds required to be returned by the student
will not be distributed to a Federal Direct Loan Program and no returns shall be
distributed to the Federal Work Study Program.

Return of Overpayment Policy Title IV financial aid recipients who withdraw from all classes, or who are administratively
withdrawn from all classes will be required to return a portion of the financial aid they
received. Calculations are done to determine the percent of financial aid earned and
unearned for the given term.
Title IV financial aid recipients who receive an overpayment resulting from changes in
enrollment, cost of attendance, general eligibility, and additional financial aid award(s)
will be required to return funds in the amount necessary to eliminate the overpayment.
The student will be notified if a return of Title IV funds is due. Failure to return Title IV
funds will result in the loss of eligibility for financial aid.

Federal Return of Title IV Funds Policy Title IV recipients who withdraw completely on or before 60% in time has elapsed during
the period of enrollment for which the student has been charged, will be required to
return a portion of Title IV funds. The return amount will be calculated using the number
of days the student attended in the given term and the total number of days in the given
term.

Complaint Policy
Informal Complaints
Before making written complaints, students are encouraged to seek resolutions by
discussing them informally with the relevant staff member who is most associated with
the matter. A staff member with whom a concern is raised by a student is expected to
deal with the matter in an open and professional manner and to take reasonable and
prompt action to try to resolve it informally. A student who is uncertain about how to seek
informal resolution of a concern is encouraged to seek advice from the Director of
Student Financial Aid.
Formal Complaints
Students may submit an official written complaint through BlazerNET under the

We encourage all students to make a reasonable effort to resolve issues before
submitting an official complaint. Complaints are reviewed with relevant faculty and/or

This federal program provides parttime employment for eligible
undergraduate and graduate students who demonstrate financial need as
established by the U.S. Department of Education. The federal share of work
study wages cannot exceed 75%; the employer share of wages is 25%.
Rates of pay are determined by the federal minimum wage, experience, and
nature of duties to be performed. The average hourly rate is $7.25.
Average schedule: 16 hours per week
Students must be enrolled in a program leading to a degree or certificate from
UAB.
Graduate are not eligible.
Work study students cannot hold more than one work study assignment.
Application Procedures for Employers:
Submit a job description on the WorkStudy Request form which can be
obtained in Student Financial Aid (SFA). Please include:
1. the name and address of the department, public agency, or nonprofit
organization
3. the student’s duties and responsibilities
4. the job qualifications
5. the job’s wage rate or range
6. the name and contact number of the student’s supervisor.
Offcampus agencies must also sign a contract approved by the UAB Legal
Office. You will be notified by SFA if your position qualifies for Federal Work
Study.
Job requests will be advertised on the bulletin board outside the SFA office
and the UAB Career Center website, DragonTrail Jobs.
Employers must notify SFA as soon as job vacancies are filled, so that the

Eligible workstudy students select a posted job.
Contact prospective employer for a job interview.
Bring workstudy award notification to the interview.
If student is offered and accepts the position, the WorkStudy Placement form
must be completed and signed by the employing department.
A new workstudy student, or a student whose work study assignment is being
reactivated in Oracle, must complete an I9 form, and a UAB application in the
hiring department. If the student is under the age of 18, a parent of the
student must fill out and sign the I9 form on the student’s behalf.
The new hire ACT document is created and submitted in the Oracle
Administrative System by the employing department. All hiring documents
including a copy of the Work Study Placement form are to be sent to UAB
Records Administration. The original Work Study Placement form and copy of
the main ACT document should be sent to the UAB Financial Aid Office for
review and approval once the ACT document comes into the SFA Office work
list in Oracle.
Students employed by offcampus agencies and the America Reads Program
must complete the hiring documents in the UAB Financial Aid Office within one
week of notification that they have been hired by the agency.
Transfers or Termination of Employment by Student:
Students transferring to a new work study position must do so at the
beginning of a new pay period.
To be eligible for transfer to another employer or to terminate workstudy
employment, the student should give the employer a 2 week notice.
You must obtain another WorkStudy Placement form from the new employing
department 2 weeks prior to transfer.
Termination or Problems with Employment:
Employers and students should make every effort to resolve problems. If a
resolution cannot be reached, please contact SFA.
Employers can dismiss a student whose performance is not satisfactory. The
employer must supply the reason for dismissal in writing to SFA.
The employer must monitor a student’s earnings to ensure the maximum
earnings eligibility is not exceeded. Once a student reaches the maximum
earnings allowed for the academic year, the employing department will be

charged 100% of the student’s wages.
Off campus employers should notify SFA when a student is no longer
employed with their agency.
On campus departments should process an end assignment or termination in
Oracle when a student will no longer be working for the department.
Pay Periods/Timesheets:
The employer must maintain a record of the hours worked by the student.
Each student completes a UAB time sheet or departmental timesheet every
two (2) weeks. The number of hours worked each day is recorded on the time
sheet. The student and employer must sign the time sheet.
The employer’s timekeeper or the student may enter the hours worked on the
TEL document through Oracle. Approvers in the employer’s department must
verify the number of hours worked and submit the approved timesheet by the
published due date for biweekly timesheets in UAB Payroll Services. WORK
STUDY STUDENTS ARE NOT ALLOWED TO APPROVE THEIR OWN TIME
SHEETS.
Time sheets from offcampus agencies should be hand carried or faxed to the
Student Financial Aid Office by 5:00 on the Monday following the end of the
pay period.
Departments/Agencies that have new workstudy students and are unable to
retrieve a TEL document in Oracle should contact AskIT at 205-9965555
Payments to students are issued every other Friday by one of the following
methods:
1. Direct deposit to a checking or savings account
2. Check mailed to the student. Direct deposit is recommended for
students.
This form is available online by accessing the student’s SelfServices
Applications through the Oracle database at the UAB Administrative
Systems page.
The UAB Biweekly Payroll Schedule can be retrieved from the UAB Financial
Affairs Payroll Services website in the “Calendars and Important Dates”
section.
Employer Responsibilities:
A work study student’s hourly rate must be at least minimum wage. Please
keep in mind that students are awarded a total sum of workstudy funds for
the academic year and an increase in the hourly rate will decrease the hours
per week a student may be eligible to work.
Employers are responsible for making sure that the student’s hours worked
are submitted by TEL document in the Oracle database system. A student
may submit his/her own time using their Blazer ID but are not allowed to

Promote open communication with student employee to foster mutual
understanding.
Communicate job duties and work expectations in a clear and timely manner
to student employee.
Encourage student employee input, suggestions, and involvement.
Complete a performance evaluation on each student employee at least once
a year and provide the student with a copy.
Provide adequate training to assist student employee with the performance of
duties and personal growth.
Provide student employee with appropriate departmental policies as well as
university policies and procedures.
Adhere to the provisions outlined in the UAB Employee Handbook.
Remember that this employee is a STUDENT. Be sure that the student’s work
schedule will not interfere with classes and studying.
Student Responsibilities:
Be punctual, dependable, and demonstrate good attitude toward your job.
A request for time off must be submitted to your employer in advance. Ask
your employer how much advance notice is required. Failure to report to work
as scheduled is cause for termination.
Time off for illness or emergencies is not subject to advance notice. Be sure
to call your employer and let them know you will not be able to report for work.
Ask about office dress codes before you begin employment.
Provide your employer with your class schedule each semester, so that a work
schedule can be established.
Record only time worked on your timesheet. Do not include lunch breaks or
study breaks on your timesheet.
A student employee who falsifies the number of hours worked, or alters a
timesheet after the employer has signed the timesheet, will no longer be
eligible for the Federal WorkStudy Program. Any amounts paid based on a
falsified timesheet will be charged back to the student employee. The student
will be reported to the U.S. Department of Education and the UAB Non
Academic Judicial Officer.
A student employee that has access to confidential information must keep this
information confidential and should sign a confidentiality agreement.
Work study students should give their current employer a two week notice if
ending their work study job or changing work study jobs before the end of the
work study award period.

In order to receive federal financial aid, a student must make satisfactory progress
toward a degree. As required by federal regulations effective July 1, 2011, the University
of Alabama at Birmingham (UAB) has amended its SAP policy.
Changes will take effect when SAP is calculated at the end of fall 2011. Changes to the
previous policy are in bold.
Qualitative Policy
The minimum UAB grade point average required to receive and maintain eligibility for
financial aid is:
Quantitative Policy
In addition to GPA requirements, the law establishes a maximum time frame in which a
student must complete his or her educational program. The maximum time frame is 150
percent of the published length of the educational program. For example, a student
would be allowed no more than 180 attempted credit hours to complete a baccalaureate
degree that is normally earned with 120 earned credit hours. When measuring
compliance with this policy, all attempted credit hours are counted, including those
attempted at other schools, regardless of whether or not financial aid was received.
Undergraduate
1. An undergraduate student must complete degree requirements within 180
attempted credit hours. A student seeking a second undergraduate degree will
be allowed cumulative attempted credit hours of 270. Enrollment at other post
secondary institutions will be included when measuring compliance with this
policy, regardless of whether or not financial aid was received.
2. A student must complete 67% of course work each year in order to graduate
within 180 attempted credit hours. To meet this requirement, a minimum number
of hours must be completed each year (2 semesters of attendance). Remedial
courses are counted in attempted hours only. Remedial courses may not apply
toward completion of degree requirements and are not counted in the GPA

SAP Evaluation
Students failing SAP due to GPA or completion rate will be placed on “WARN – Warning,
Failing SAP” status. Students on this status are eligible for aid. There is no appeal
process to receive this status. It will be automatically granted the first time a student
falls below the GPA and completion rate SAP standards. Aid will be received for one
semester only. Student must be making SAP at the end of the warning period.
Students on “WARN – Warning, Failing SAP” status who fail to meet SAP standards
again will be placed on “FAIL – Failing SAP” status. Students on this status are not
eligible for aid. However, the student may appeal this decision, and if the appeal is
successful, can be granted probation. At the end of the probationary period, the student
is expected to be making SAP, or must be successfully following an academic
success plan contract designed to ensure that the student will meet SAP by a
specific point in time.
Appeal Guidelines
Students on failing SAP status may appeal if they have extenuating circumstances. The
Appeals Form, the Academic Success Plan Contract, and the Academic Success Plan
Signature Sheet can all three be found at www.uab.edu/sap or by clicking on the links
below. Students must complete the top section of the Academic Success Plan
Contract before visiting their advisor. The student can provide any supporting
documentation from a third party that documents the circumstances listed in the
appeal. Also, the student must submit an academic success plan contract
developed by their academic advisor (conditions this plan must meet are listed
below). Appeals, if granted, are for one semester only. Students cannot appeal
a second or third time, unless they have an academic success plan contract in
place and are meeting the conditions of the contract.
Academic Success Plan Contract
1. UAB GPA: The Academic success plan contract should outline the
length of time it will take for the student's GPA to meet the SAP
requirement. If appropriate, it should also include the courses that the
student should enroll in that would facilitate their successful
matriculation to meeting the SAP GPA standard.
2. Completion Rate: The academic success plan contract should include

the courses or recommended course load that would allow the student
to meet the completion rate standards.
3. Maximum Timeframe for Degree: The academic success plan contract
should outline the remaining courses required for graduation and the
length of time for the student to complete the remaining courses. It
should also provide an explanation if the student is exceeding the
maximum timeframe (i.e., admitted as a transfer student, major change,
and excessive repeptitive coursework, would be some possible
examples).
Re-Establishing Aid Eligibility
A student who is not making satisfactory academic progress may choose not to submit
an appeal. If this is the case, the student will not be eligible to receive federal student
financial aid until he/she is in compliance with all components of the satisfactory
academic progress policy. The student will need to attend classes until he is once again
in compliance with the policy. This may take several semesters to accomplish, but it will
be the responsibility of the student to improve his/her academic performance during this
time. Once the student feels that he/she is in compliance with the satisfactory academic
progress policy, he/she should request that his/her academic record be reviewed by the
Office of Student Financial Aid. If he/she is found to be in compliance, his/her eligibility
for federal student aid will be reestablished.
General Rules
1. Financial aid recipients should select a major and be admitted to a program that
will lead to the receipt of a degree or certificate by the time they reach junior
status. If you change your major more than once, it will be difficult to complete
degree requirements within the maximum time frame allowed.
2. A student who audits, repeats, and withdraws frequently from courses, or takes
courses not related to a degree objective is not making satisfactory progress
toward a degree, and is subject to the loss of student financial aid eligibility.
3. An appeal granted through the Office of the Registrar does not satisfy nor
replace the requirements of the Financial Aid Appeal process.
4. All academic work (including work voided by UAB’s New Start Option) must be

Dependent/Independent Policy The Federal Government has identified for student financial assistance programs
certain categories of students who must be considered independent financial aid
applicants. As a result, a student is considered an independent financial aid applicant if
he or she meets one of the following criteria:

An independent financial aid applicant is not required to submit parental information in
the application process. However, if the independent applicant is married, spousal
information must be reported. A student who cannot meet at least one of the above
criteria is considered a dependent applicant and must provide parental information in
the application process.

Financial aid awards are disbursed in equal amounts each semester and applied directly
to the student's account. If the award is more than the account balance, a disbursement

Institutional Refund Policy
Generally, students will not be charged tuition and fees for classes officially dropped
prior to the published drop deadline each term. All tuition and fee payments may be
refunded for the credit hours and fees associated with each class dropped before or
during this period. The deadline for dropping classes without incurring charges is
published each term in the UAB Academic calendar.
All first-time borrowers of a Federal Direct Stafford Loan must complete
entrance counseling prior to signing a promissory note or receiving funds.
A first-time borrower is a student who doesn’t have an outstanding Federal
Direct Stafford Loan or Federal Direct Unsubsidized Stafford Loan.
You’ll receive additional information when your award is finalized. Currently
enrolled students will accept awards on BlazerNET, UAB’s secure online
portal. New students who haven’t applied for a Blazer ID will accept awards
on Banner Self Service, our online tool to help you manage your personal
and academic information.
First-time borrowers can fulfill the entrance-counseling requirement online
at www.studentloans.gov . After you successfully complete the entrance
counseling, you will need to wait 48 hours before you can sign your master
promissory note, also at www.studentloans.gov .
If you’re approved for a Federal Perkins Loan, you’ll receive additional
information from Student Accounting Services after your award is finalized
and you’ve accepted the award.
Parents of dependent undergraduate students who want to apply for a
Federal Direct Parent Loan for Undergraduate Students (Federal
Direct PLUS) should complete the PLUS Loan Application at
www.studentloans.gov . You must also complete the Free Application for

proof of income earned and taxes paid. The new Department of

Return of Overpayment Policy Title IV financial aid recipients who withdraw from all classes, or who are administratively
withdrawn from all classes will be required to return a portion of the financial aid they
received. Calculations are done to determine the percent of financial aid earned and
unearned for the given term.
Title IV financial aid recipients who receive an overpayment resulting from changes in
enrollment, cost of attendance, general eligibility, and additional financial aid award(s)
will be required to return funds in the amount necessary to eliminate the overpayment.
The student will be notified if a return of Title IV funds is due. Failure to return Title IV
funds will result in the loss of eligibility for financial aid.

Before making written complaints, students are encouraged to seek resolutions by
discussing them informally with the relevant staff member who is most associated with
the matter. A staff member with whom a concern is raised by a student is expected to
deal with the matter in an open and professional manner and to take reasonable and
prompt action to try to resolve it informally. A student who is uncertain about how to seek
informal resolution of a concern is encouraged to seek advice from the Director of
Student Financial Aid.

This federal program provides parttime employment for eligible
undergraduate and graduate students who demonstrate financial need as
established by the U.S. Department of Education. The federal share of work
study wages cannot exceed 75%; the employer share of wages is 25%.
Rates of pay are determined by the federal minimum wage, experience, and
nature of duties to be performed. The average hourly rate is $7.25.
Average schedule: 16 hours per week
Students must be enrolled in a program leading to a degree or certificate from
UAB.
Graduate are not eligible.
Work study students cannot hold more than one work study assignment.
Application Procedures for Employers:
Submit a job description on the WorkStudy Request form which can be
obtained in Student Financial Aid (SFA). Please include:
1. the name and address of the department, public agency, or nonprofit
organization
3. the student’s duties and responsibilities
4. the job qualifications
5. the job’s wage rate or range
6. the name and contact number of the student’s supervisor.
Offcampus agencies must also sign a contract approved by the UAB Legal
Office. You will be notified by SFA if your position qualifies for Federal Work
Study.
Job requests will be advertised on the bulletin board outside the SFA office
and the UAB Career Center website, DragonTrail Jobs.
Employers must notify SFA as soon as job vacancies are filled, so that the

Eligible workstudy students select a posted job.
Contact prospective employer for a job interview.
Bring workstudy award notification to the interview.
If student is offered and accepts the position, the WorkStudy Placement form
must be completed and signed by the employing department.
A new workstudy student, or a student whose work study assignment is being
reactivated in Oracle, must complete an I9 form, and a UAB application in the
hiring department. If the student is under the age of 18, a parent of the
student must fill out and sign the I9 form on the student’s behalf.
The new hire ACT document is created and submitted in the Oracle
Administrative System by the employing department. All hiring documents
including a copy of the Work Study Placement form are to be sent to UAB
Records Administration. The original Work Study Placement form and copy of
the main ACT document should be sent to the UAB Financial Aid Office for
review and approval once the ACT document comes into the SFA Office work
list in Oracle.
Students employed by offcampus agencies and the America Reads Program
must complete the hiring documents in the UAB Financial Aid Office within one
week of notification that they have been hired by the agency.
Transfers or Termination of Employment by Student:
Students transferring to a new work study position must do so at the
beginning of a new pay period.
To be eligible for transfer to another employer or to terminate workstudy
employment, the student should give the employer a 2 week notice.
You must obtain another WorkStudy Placement form from the new employing
department 2 weeks prior to transfer.
Termination or Problems with Employment:
Employers and students should make every effort to resolve problems. If a
resolution cannot be reached, please contact SFA.
Employers can dismiss a student whose performance is not satisfactory. The
employer must supply the reason for dismissal in writing to SFA.
The employer must monitor a student’s earnings to ensure the maximum
earnings eligibility is not exceeded. Once a student reaches the maximum
earnings allowed for the academic year, the employing department will be

charged 100% of the student’s wages.
Off campus employers should notify SFA when a student is no longer
employed with their agency.
On campus departments should process an end assignment or termination in
Oracle when a student will no longer be working for the department.
Pay Periods/Timesheets:
The employer must maintain a record of the hours worked by the student.
Each student completes a UAB time sheet or departmental timesheet every
two (2) weeks. The number of hours worked each day is recorded on the time
sheet. The student and employer must sign the time sheet.
The employer’s timekeeper or the student may enter the hours worked on the
TEL document through Oracle. Approvers in the employer’s department must
verify the number of hours worked and submit the approved timesheet by the
published due date for biweekly timesheets in UAB Payroll Services. WORK
STUDY STUDENTS ARE NOT ALLOWED TO APPROVE THEIR OWN TIME
SHEETS.
Time sheets from offcampus agencies should be hand carried or faxed to the
Student Financial Aid Office by 5:00 on the Monday following the end of the
pay period.
Departments/Agencies that have new workstudy students and are unable to
retrieve a TEL document in Oracle should contact AskIT at 205-9965555
Payments to students are issued every other Friday by one of the following
methods:
1. Direct deposit to a checking or savings account
2. Check mailed to the student. Direct deposit is recommended for
students.
This form is available online by accessing the student’s SelfServices
Applications through the Oracle database at the UAB Administrative
Systems page.
The UAB Biweekly Payroll Schedule can be retrieved from the UAB Financial
Affairs Payroll Services website in the “Calendars and Important Dates”
section.
Employer Responsibilities:
A work study student’s hourly rate must be at least minimum wage. Please
keep in mind that students are awarded a total sum of workstudy funds for
the academic year and an increase in the hourly rate will decrease the hours
per week a student may be eligible to work.
Employers are responsible for making sure that the student’s hours worked
are submitted by TEL document in the Oracle database system. A student
may submit his/her own time using their Blazer ID but are not allowed to

Promote open communication with student employee to foster mutual
understanding.
Communicate job duties and work expectations in a clear and timely manner
to student employee.
Encourage student employee input, suggestions, and involvement.
Complete a performance evaluation on each student employee at least once
a year and provide the student with a copy.
Provide adequate training to assist student employee with the performance of
duties and personal growth.
Provide student employee with appropriate departmental policies as well as
university policies and procedures.
Adhere to the provisions outlined in the UAB Employee Handbook.
Remember that this employee is a STUDENT. Be sure that the student’s work
schedule will not interfere with classes and studying.
Student Responsibilities:
Be punctual, dependable, and demonstrate good attitude toward your job.
A request for time off must be submitted to your employer in advance. Ask
your employer how much advance notice is required. Failure to report to work
as scheduled is cause for termination.
Time off for illness or emergencies is not subject to advance notice. Be sure
to call your employer and let them know you will not be able to report for work.
Ask about office dress codes before you begin employment.
Provide your employer with your class schedule each semester, so that a work
schedule can be established.
Record only time worked on your timesheet. Do not include lunch breaks or
study breaks on your timesheet.
A student employee who falsifies the number of hours worked, or alters a
timesheet after the employer has signed the timesheet, will no longer be
eligible for the Federal WorkStudy Program. Any amounts paid based on a
falsified timesheet will be charged back to the student employee. The student
will be reported to the U.S. Department of Education and the UAB Non
Academic Judicial Officer.
A student employee that has access to confidential information must keep this
information confidential and should sign a confidentiality agreement.
Work study students should give their current employer a two week notice if
ending their work study job or changing work study jobs before the end of the
work study award period.

In order to receive federal financial aid, a student must make satisfactory progress
toward a degree. As required by federal regulations effective July 1, 2011, the University
of Alabama at Birmingham (UAB) has amended its SAP policy.
Changes will take effect when SAP is calculated at the end of fall 2011. Changes to the
previous policy are in bold.

The minimum UAB grade point average required to receive and maintain eligibility for
financial aid is:

In addition to GPA requirements, the law establishes a maximum time frame in which a
student must complete his or her educational program. The maximum time frame is 150
percent of the published length of the educational program. For example, a student
would be allowed no more than 180 attempted credit hours to complete a baccalaureate
degree that is normally earned with 120 earned credit hours. When measuring
compliance with this policy, all attempted credit hours are counted, including those
attempted at other schools, regardless of whether or not financial aid was received.
Undergraduate
1. An undergraduate student must complete degree requirements within 180
attempted credit hours. A student seeking a second undergraduate degree will
be allowed cumulative attempted credit hours of 270. Enrollment at other post
secondary institutions will be included when measuring compliance with this
policy, regardless of whether or not financial aid was received.
2. A student must complete 67% of course work each year in order to graduate
within 180 attempted credit hours. To meet this requirement, a minimum number
of hours must be completed each year (2 semesters of attendance). Remedial
courses are counted in attempted hours only. Remedial courses may not apply
toward completion of degree requirements and are not counted in the GPA

SAP Evaluation
Students failing SAP due to GPA or completion rate will be placed on “WARN – Warning,
Failing SAP” status. Students on this status are eligible for aid. There is no appeal
process to receive this status. It will be automatically granted the first time a student
falls below the GPA and completion rate SAP standards. Aid will be received for one
semester only. Student must be making SAP at the end of the warning period.
Students on “WARN – Warning, Failing SAP” status who fail to meet SAP standards
again will be placed on “FAIL – Failing SAP” status. Students on this status are not
eligible for aid. However, the student may appeal this decision, and if the appeal is
successful, can be granted probation. At the end of the probationary period, the student
is expected to be making SAP, or must be successfully following an academic
success plan contract designed to ensure that the student will meet SAP by a
specific point in time.
Appeal Guidelines
Students on failing SAP status may appeal if they have extenuating circumstances. The
Appeals Form, the Academic Success Plan Contract, and the Academic Success Plan
Signature Sheet can all three be found at www.uab.edu/sap or by clicking on the links
below. Students must complete the top section of the Academic Success Plan
Contract before visiting their advisor. The student can provide any supporting
documentation from a third party that documents the circumstances listed in the
appeal. Also, the student must submit an academic success plan contract
developed by their academic advisor (conditions this plan must meet are listed
below). Appeals, if granted, are for one semester only. Students cannot appeal
a second or third time, unless they have an academic success plan contract in
place and are meeting the conditions of the contract.
Academic Success Plan Contract
1. UAB GPA: The Academic success plan contract should outline the
length of time it will take for the student's GPA to meet the SAP
requirement. If appropriate, it should also include the courses that the
student should enroll in that would facilitate their successful
matriculation to meeting the SAP GPA standard.

the courses or recommended course load that would allow the student
to meet the completion rate standards.
3. Maximum Timeframe for Degree: The academic success plan contract
should outline the remaining courses required for graduation and the
length of time for the student to complete the remaining courses. It
should also provide an explanation if the student is exceeding the
maximum timeframe (i.e., admitted as a transfer student, major change,
and excessive repeptitive coursework, would be some possible
examples).
Re-Establishing Aid Eligibility
A student who is not making satisfactory academic progress may choose not to submit
an appeal. If this is the case, the student will not be eligible to receive federal student
financial aid until he/she is in compliance with all components of the satisfactory
academic progress policy. The student will need to attend classes until he is once again
in compliance with the policy. This may take several semesters to accomplish, but it will
be the responsibility of the student to improve his/her academic performance during this
time. Once the student feels that he/she is in compliance with the satisfactory academic
progress policy, he/she should request that his/her academic record be reviewed by the
Office of Student Financial Aid. If he/she is found to be in compliance, his/her eligibility
for federal student aid will be reestablished.
General Rules
1. Financial aid recipients should select a major and be admitted to a program that
will lead to the receipt of a degree or certificate by the time they reach junior
status. If you change your major more than once, it will be difficult to complete
degree requirements within the maximum time frame allowed.
2. A student who audits, repeats, and withdraws frequently from courses, or takes
courses not related to a degree objective is not making satisfactory progress
toward a degree, and is subject to the loss of student financial aid eligibility.
3. An appeal granted through the Office of the Registrar does not satisfy nor
replace the requirements of the Financial Aid Appeal process.
4. All academic work (including work voided by UAB’s New Start Option) must be
considered in determining federal financial aid eligibility.
5. Students who continue to pursue multiple degrees in excess of two (2) may be

interpreted as not making satisfactory academic progress.
6. Financial aid regulations permit payment of Title IV funds to students ONE time
for retaking previously passed coursework.

Students must give written notice, including name and student ID number, to the aid
office if changes occur in:
income estimated on the financial aid applications
the amount of external scholarships, employee benefits, UAB
assistantships/fellowships, tuition waivers, etc., that you will receive
graduation, separation date.

Dependent/Independent Policy The Federal Government has identified for student financial assistance programs
certain categories of students who must be considered independent financial aid
applicants. As a result, a student is considered an independent financial aid applicant if
he or she meets one of the following criteria:
was born before January 1, 1991 (2014-2015 year)
is a veteran of the U.S. Armed Forces
is currently serving on active duty in the U.S. Armed Forces for purposes
other than training
has a child who receives more than half support from student
has a dependent (other than child or spouse) who lives with and will receive
more than half support from student through June 30, 2015
is a married student
is a graduate or professional student
at any time since the age 13, if both parents are deceased, was in foster care
or was a dependent or ward of the court
is an emancipated minor as determined by a court in state of legal residence
is/was in legal guardianship, as determined by a court in state of legal

was at any time after July 1, 2013, as determined by the high school or school
district homeless liaison, an unaccompanied youth who was homeless

An independent financial aid applicant is not required to submit parental information in
the application process. However, if the independent applicant is married, spousal
information must be reported. A student who cannot meet at least one of the above
criteria is considered a dependent applicant and must provide parental information in
the application process.

Disbursement/Payment Policy
Financial aid awards are disbursed in equal amounts each semester and applied directly
to the student's account. If the award is more than the account balance, a disbursement
will be generated and issued to the student in the form of direct deposit or check. If the
award is less than the account balance, the student will be billed for the remaining
amount due on the account.

Institutional Refund Policy
Generally, students will not be charged tuition and fees for classes officially dropped
prior to the published drop deadline each term. All tuition and fee payments may be
refunded for the credit hours and fees associated with each class dropped before or
during this period. The deadline for dropping classes without incurring charges is
published each term in the UAB Academic calendar.
