3. Printtheorderformandlistofavailabletests.

4. Haveyourprovidercompletebox3(ProviderOrder).Theprovidermustsigntheform.

5. Ifpayingbycreditcardhavetheproviderfaxtheformtotheaddressinbox4.Otherwise,mailtheformandchecktotheaddressinbox.
