Gazette Officer /Bank Manager/ Post Master 

 

Documents required for Digital signature 

1. PAN CARD 2. Govt. ID 3. Voter ID 4. Adhar Card 5. Passport 6. Driving Licence  7. Copy of Passbook  

*Any two of the above mentioned documents  *Use only BLUE INK for form filling. *All Signatures should be as per in ID. *And Two Passport Size Photographs. 

*Email Id & a valid Mobile Number. 

*All the attached Documents should be self signed and attested by Gazetted Officer.
