Saline resection is commonly used in Urologyfor prostate resection and for bladder tumorresection. Saline resection is also common inthe field of Gynecology for myomas andendometrial resections. The key difference forbipolar resections is the use of saline (0.9%sodium chloride) as an irrigant instead of1.5% glycine.

For bipolar resections, saline is used as anirrigation medium and as a conductor of theelectrosurgical current. Electrosurgical currentflows from the active electrode back to theworking element; therefore the use of aseparate grounding pad is not needed. Thecurrent does not flow through the patient’sbody but is conducted back to the electrodefork through the saline irrigation medium.

When the electrode is activated via thefootswitch, the following happens:• The electric current heats the salineirrigation and tissue via the cutting loop

• Bubbles are generated as the salinevaporizes

• The current ionizes the gas bubbles togenerate a plasma

• The plasma ignites and the loop isilluminated bright yellow

• The tissue vaporizes and the cutting loopbegins to resect tissue

Bipolar resection offers the followingadvantages;• Shallow coagulation• Minimal depth of thermal damage• No impairment of histological quality• Reduced irritation of the urethra (boththermal and mechanical)

Working elementsRichard Wolf offers several bipolar workingelements that are completely compatible withall existing 22 Fr. standard resectoscope

sheaths and larger, and all 24.5 Fr. CF-Resectoscope sheaths and larger. The bipolarworking elements work with all 0° to 30°Panoview endoscopes that are 4 mm indiameter and have a working length of atleast 30 cm.

Richard Wolf offers two different cuttingactions, passive / spring assisted or active /manual cutting. All bipolar working elementsare available with either an open handle or aclosed handle.

ElectrodesSpecial disposable electrodes were developedfor resection of the prostate and bladdertumors.

Thicker wire cutting loops, 0.3 mm Ø, weredesigned for increased hemostasis effect foruse in resection of the prostate. A thinnercutting loop, 0.25 mm Ø, was designed forsharp, smooth cutting as is often required forbladder tumors.

The structure of the electrode ensures that thecurrent flows from the cutting loop to theelectrode fork, without causing any thermalstress to the urethra.

Richard Wolf offers a true 22 Fr. cutting loop.The cutting characteristics of these electrodes,combined with the plasmic formation, permitremoval of more tissue.

Bipolar HF Connecting CableA special bipolar HF connecting cable,8108.232- 3 M WL or 8108.252- 5 MWL, is required for saline resections withRichard Wolf’s bipolar resectoscopes.The special bipolar HF connecting cableis specifically designed for use withERBE’s VIO 300D electrosurgicalgenerator, equipped with a Multi-Function port.

The black plug is connected to the blackHF post on the bipolar resectoscope, andthe grey plug is connected to the neutralpin on the side of the bipolarresectoscope working element.
