16 Forming Intentional Disciples How can we transmit a living, personal Catholic faith to future generations? By coming to know Jesus Christ, and following him as his disciples. Forming Intentional Disciples How can we transmit a living, personal Catholic faith to future generations? By

coming to know Jesus Christ, and following him as his disciples.

Life-long learning opportunities 2016-2017 Our Parish builds families who “Go Make a Difference” through life long-learning

and service. Make this year your family tries one of the many opportunities a

available.

For Children K-2nd Grade: Weekly Children’s Liturgy of the Word at the 4:30, 9

AM, and 11 AM Masses.

Adult Faith Formation Opportunities: Many parish members have begun to

study the Word of God on a regular basis:

Bible Study - Times and Dates to TBA. Check the bulletin for updates. Ses-

sions are 90 minutes for 6 weeks.

A men's study group is available, Thursday evenings. Contact Paul Krzmarzick at

Other studies are also available for individual study or your small group. Please

Jr. High Youth Ministry is for any student in 6th thru 8th Grade who attend OLV or JFK and their friends. Youth Ministry is not another religion class, it is a time and place to make new friends, eat together, have fun and look at faith in a different way. It is a safe place where everyone can deepen their faith through various activities. Youth Nights– Monthly youth nights are scheduled from 6:00 pm until 8:00pm in the Youth Room. Pick up a schedule or check the bulletin for upcoming dates and themes. Come and check it out, make new friends, eat and have some fun. Jr. High Rally– High School Youth from around the Diocese plan this event with the help of adult Youth leaders. Music, Laughter and games are all part of this fun and faith-filled day in Iowa City. Faith Formation at Our Lady of Victory Parish will foster,

build and develop an understanding of the Roman Catholic

Faith for all members of the community through

sacramental life, community education and service.

Faith Formation will promote lifelong learning. We will be

Members are elected by the Parish in May and serve 3 years.

Policies directing this program are derived from the

Handbook of Policies of Diocesan Faith Formation

Programs (2001) and local implementations by the

Our Lady of Victory Board of Education. Complete

copies of these policies are available for review in the

Faith Formation Office.

Board of Education meetings are normally held the first

Tuesday of the month and open to the public. Interested

parishioners are encouraged to submit their names for

May election to the Board.

Volunteer Opportunities

Many parents and other interested individuals in our parish community

volunteer services to our Faith Formation program. For all services

involving children or vulnerable adults, the Diocese requires that volunteers:

1. Be registered parishioners for 6 months or more.

2. Must meet the requirements of the Diocese of Davenport

3. Must submit completed forms indicating their intended compliance with

Policies Relating to Sexuality and Personal Behavior (Revised 2006). A

copy is provided for each volunteer, and is available for review in the

office. Records of agreement to comply are kept on file indefinitely

4. Volunteers must attend the SAFE ENVIRONMENT on line training ses-

sion provided by the diocese and also complete all paperwork and con-

tinuing education required by Diocesan policies.

Salaried staff also follow Diocesan guidelines in these matters.

Discipline

Courtesy, compassion, and respect is the rule here. If there is classroom

disruption, the catechist may ask the child/youth to report to the office.

A student report of the incident will be filed. Usually students are asked

to write a short note of apology before returning to class. Communication

with parents will occur depending on the seriousness of the incident or with

a second reported incident. In case of recurring problems, a student may

be suspended for a short time. Open communication with parent and

student is the policy.

Procedure for Addressing Problems, Question & Complaints

1) Confer with the child’s catechist.

2) If satisfaction is not reached, confer with the

Director of Religious Education for Faith Formation

3) Confer with the Pastor

4) Submit a written complaint to the President of the Board of Education.

The President will determine whether or not the item will be brought

before the Board of Education for review.

A copy of Diocesan polices is kept in the Faith Formation office or

the policies can be viewed on line at our website www.olvjfk.com.

Drug and Alcohol Statement:

The parish prohibits the possession, use, passing, or selling of tobacco,

alcohol, drugs, or other narcotics in any form by students.*

Weapons: The parish prohibits weapons of any description, including, but

not limited to matches, cigarette lighters, guns, razor-blades or other sharp

objects. *

Harassment: Personnel must not engage in or tolerate physical,

psychological, written or verbal harassment of anyone.

Program Guidelines Registration

Families should be registered members of Our Lady of Victory

Parish. Parish registration forms are submitted to the Parish

Office. If you are not a member of Our Lady of Victory Parish

and have a special circumstance for attendance, please speak

with a Faith Formation Staff member.

Faith Formation registration forms are completed annually.

Registration begins in April and continues through

the first week of August during “Back to school”

registration weekend.

Baptismal Certificates Children enrolling in Faith

Formation for the first time must present an original

copy of their Baptismal Certificate. This will be copied for your child’s

file and returned. Please inform the office if your child was baptized at

OLV, in which case records are on file in the parish office.

Fees Program fees are due at the time of registration. The fees are

reviewed annually and set by the Board of Education.

Registered contributing members of the parish pay $85.00 with one

child; $110.00 for a family with 2 or more children. Active parish

participation is expected.

Parish membership also includes an understanding of stewardship of

time, talent, and treasure. The program fees cover only a portion of

actual costs. Your weekly participation at Mass and contributions to

OLV collections are essential component of quality programming.

Please take advantage of the opportunities that will help your family

with the registration fee if your time and talent outweigh your treasure.

Anyone experiencing financial difficulties can speak with the Pastoral

Associate to arrange an acceptable alternate payment plan. This

information will be kept confidential. No child shall be refused

instruction for inability to pay.

Your active support of Parish Fundraisers such as Victory Vouchers

and the yearly Gala activities benefits everyone in the parish. Look for

information about these opportunities.

Parental Support

Please support your child’s learning by weekly mass attendance

and place a priority on active participation for each class. In

addition -

Ask your child about his/her weekly lesson & work as a

family on any assignments that are sent home.

Read and sign papers sent home.

Inform catechists of any misunderstanding or lack of clarity about what

is taught.

Practice what you preach- take advantage of the adult learning

opportunities to study and learn more yourself.

Attend All Family Events with your child.

Attend Mass weekly as a family.

Yearly Calendar

A Faith Formation calendar is issued in the fall please keep this

calendar for reference during the year.

Watch for weekly church bulletin updates, available on line

as well at our website at www.olvjfk.com.

Absences & Tardiness

Please notify the office if a child is going to be absent. This may be

done by calling 391-8384, during school hours or immediately before a

scheduled class. Attendance records are kept by the office. If your

child is absent, and has not reported to the office a call to your home

will be made to verify the absence.

If your child is late please have them report to the office.

If your child needs to leave early a letter or phone call must be

made to the Faith Formation office. The child must be picked up in

the office in order to leave the building. 11

Correspondence

Children will have classroom work, newsletters and parish activity

information to bring home. Check EACH WEEK for these important

notes. Also check the weekly parish bulletin and our website

www.olvjfk.com for updates.

Homework

Your child’s catechist is encouraged to provide take-home work or

online exercises to reinforce lessons. This allows children to share

what they are learning at home and build family faith sharing

practice. Please set aside time for this kind of learning activity.

Help children become responsible for retuning assigned lessons on

schedule.

Wellness Program

All foods and beverages that are available during Faith Formation time

should contribute to meeting the dietary needs of students; that is, they

are from the 5 major food groups of the food group pyramid. If you are

providing a snack, please be sure it is a nutritious treat. Please be sure

your child’s catechist is aware of any food allergies your child may have.

Change of Address

Please notify the office of changes of address, phone, and medical

conditions which may affect learning.

Iowa Smoke Free Air Act

Please be advised that as of July 1, 2008 Our Lady of Victory is a

smoke free campus. Please refrain from smoking on the grounds

or in cars while dropping off or picking up children from class.

Cell Phone & Electrical Devices

Cell phones, computer games, i-pods, or any other electrical devices

are to be turned into the teacher at the beginning of class. If a student

violates the policy, the device will be taken away and can be picked up

in the office after class. If your child needs to make a phone call, the

phone in the office may be used before or after class.

Drop off and Pickup

K-2 Parents should come into the building to drop off and pick up

your student.

3-8 Parents be sure your child enters the building safely and

reports directly to class.

Please be careful when picking up children in the parking lot.

Please park your car and wait for your child to come out of the

building, this insures that no child steps in front of a moving vehicle.

Take special care to notify the office should you be delayed in

picking up your child. Children are advised to come directly to the

office in case of delay. NO CHILD MAY WAIT IN THE

PARKING LOT OR IN ANY UNDESIGNATED AREA OF

THE BUILDING.

Safety

Safety for all children is a priority. A hall monitor patrols halls during

session times. There is yearly instruction in emergency procedures.

Fire and safety drills are held each year.

Students who must leave early may only be dismissed to a parent/

guardian. Please send a note or call before class to inform the office

that your child will be leaving early. If a child must leave early, a

parent should report to the office for pickup. A staff member will be

sent to the classroom to pick up your child to ensure safety and

minimize disruptions.

Protocol 2000 policies are in effect when an event occurs which

results in the need for students to stay in classrooms with teachers

(e.g. intruder, or medical emergency).

Sacramental Preparation

Is available for children who have never been baptized or if there has been

no post-baptismal instruction. Rite of Christian Initiation Guidelines are

followed. An interview is important for correct placement.

Parent/guardian participation is required for parent meetings. Sacramental

preparation for Reconciliation, First Communion (most often in 2nd grade)

and Confirmation (most often in 8th grade) involve special parent sessions.

Please give support to your child/youth through this time of preparation.

The Diocese requires 2 years preparation for sacraments.

Faith Events and Materials

Several times a year we ask parents to come for programs and activities

designed to bring the faith home. Special attention is given to the seasons of

the Church year and full participation in church life. Take-home materials

and social activities are included. Parents may be asked to assist by donating

snacks, or assisting with a teacher’s preparation for a special program. Any

parent wishing to assist with planning events should contact the Faith

Formation office.

Inclement Weather

If it becomes necessary to cancel classes due to severe weather, we

will attempt to notify KWQC TV and utilize our texting/phone

notification service. When inclement weather comes up quickly or late in

the day, this is not always possible. Please keep in mind we will not hold

classes if:

1) The Davenport Schools have cancelled classes on Wednesday due to

severe weather conditions.

2) The Davenport Schools issue early dismissal on Wednesday due to

severe weather conditions.

3) JFK has cancelled school due to building problems such as broken

water main, etc.

If you are uncertain whether or not classes are going to be held, please

call the office before venturing out or use your best judgment 391-8384.
