Two sets of people are involved:

1. Denture Expert Clinical Team

2. Dental Laboratory Technician

Your Denture Expert is the dentist who

will deal with your problem. He will carry out

the clinical aspects of your treatment and

ensure that the denture option you have

chosen is the one most suited to you. Moulds

and measurements will be taken and he will

work alongside the Dental Laboratory

Stages of denture production

It can take between 6-8 appointments to produce

the Perfect Fit Denture. More visits will be

required if you opt for the maximum stabilised

denture, which will result in a firm fitting

appliance.

In this booklet we have outlined the common

steps we follow at our Denture Clinic to make

the custom fitting denture set. These steps are

related to patients who require a full set of

denture teeth, but the same principle applies to

dentures which are fitted around existing teeth.

Examples of the care we have provided to our

patients are demonstrated in this booklet.

Visit two

Mould taking stage I

A mould (impression) is taken of your gums. This is

made of a soft material but quickly hardens within 30

seconds of gently placing it in the patient’s mouth.

This hardened mould is sent to the Dental

Technician who converts it to a stone model. This

acts as the framework for the denture design.

From the stone model previously constructed, a

second mould is constructed using a special pink tray

that fits more accurately around the gums.

Visit four

Bite registration and colour matching

This stage is very important as we involve our patient

to ensure the best material is chosen. It involves four

essential stages.

1. Recording the way your upper and lower

jaws meet

Some patients with denture problems do not have a

strong bite. For this reason the Denture Expert will

record the way your upper and lower jaws meet at

your most comfortable position.

He will ask you to gently bite down on the soft wax.

2. Making your smile natural

Your Denture Expert will record where the centre

of your front teeth should be in relation to your smile,

face and nose. This is to ensure that your smile looks

natural.

3. Your Denture Expert will mark the

“canine lines”

Marking the canine lines gives the indication of your

facial features. This helps to choose the correct size

and shape of teeth for your denture.

4. Choosing the right colour and shade of

teeth

You have the chance to choose the colour and shade

of your teeth.

A tip is not to go for extremely white teeth as

your smile may look unnatural.

Choose light colour and a smooth texture to

achieve a natural feel and appearance.

On your next visit, you have the chance to see if

you have made the right choice with regards to

choosing the right set of teeth.

Visit five

The wax try-in stage

Our Dental Technician will carefully analyse the

information we have gathered from visit 4 and will

build upon the framework. In this visit you will have

the chance to preview your denture set to see what

they look and feel like in your mouth. In particular,

your Denture Expert will:

Give you time to feel comfortable with your mock

denture setup and to ensure that you are happy

with the shape and colour of your chosen teeth

Ensure you can talk comfortably with your mock

dentures

Make any changes if required

Once you are happy, we will send the design back to

the Dental Technician so that your denture set can

be made into a beautiful hand-crafted design.

Visit six

Fit of your denture set

We will ensure the dentures fit comfortably in

your mouth and you can smile all the way home!

Visit seven

Review appointment

When we have finished making your dentures,

our care for you doesn’t stop there. We invite

our denture patients back a week later to make

sure that everything is comfortable. Sometimes, a

minor adjustment is required which we do as part

of the service.

Aftercare

Our care for you continues once your treatment

has been completed. You will be invited for your

routine dental inspection visits to ensure that you

are happy with your new dentures. Minor

adjustments to your dentures can be carried out if

necessary.

“What kind of teeth can I have on my

Perfect Denture Set?”

Gone are the days where patients were only offered

brilliant white or monochrome teeth. We give our

patients a choice when it comes to teeth selection and

smile design. There are many elements in choosing the

right set of teeth:

Shape

Colour and Shade

Texture

Material

Individual characteristics

If a picture was taken of a natural tooth and enlarged

on a big screen, you will be able to see many different

colours, textures and characteristics on this very

tooth. It is to this detailed attention that our Dental

Technician works towards to provide a natural smile

on your new denture set.

Size

The size of teeth depends mainly on the size of your

face and your preference. If you are a petite individual,

your smile will not match your facial features if you

chose an unusually large set of teeth. Your Denture

Expert will help you choose the correct size.

Some patients prefer to have exactly the same size

teeth as their existing dentures because they don’t

want people to know that they have drastically changed

their smile. We can cater for this requirement.

Shape

The shape of your front teeth matters. If you have a

narrow face and you choose bulky teeth, there will be

a mismatch in the final result and the denture may look

unnatural. Your Denture Expert will help you make

the right decision.

Colour and Shade

Your new denture teeth should complement the

colour of your face, lips and pupils. We don’t want the

teeth to be over-powering by choosing a completely

inadequate shade. We have got a selection of cloudy-

day, warm-ivory and naturally-white shades for you to

choose from.

Texture, material and individual characteristics

Implant-retained dentures

“What is a dental implant?”

This is a small rod of special metal which is neatly

tucked under the gum. You cannot see or feel it. It acts

as an anchor; a bit like a post in the ground. Various

components can then be attached on to the implant

head.

“Why would I need dental implants?”

If you have a gap between your teeth, a dental implant

will give you a natural looking prosthetic tooth without

cutting down healthy teeth adjacent to the gap. Using

the concept to attach components to the dental

implant, dentures can be stabilised so that they do not

rock in your mouth whilst you eat.

The following pictures demonstrate recurring gum

infection and the patient’s request to enhance her

smile.

The upper front teeth have moved considerably

and have very poor aesthetics. This is

compromising the patient’s smile, confidence,

speaking and eating abilities.

The bad teeth were removed and the extraction sites healed

in a few days. Immediately after the procedure, the patient

was given a temporary denture so that she did not leave our

clinic without her smile. A further period of three months

was observed to allow new bone to grow into the area

where the teeth had been extracted.

in the upper jaw. The protruding gold components are attached to the

underlying implants. The denture will fit over these gold components, thus

providing maximum stability and comfort whilst eating.

The upper denture is attached to the gold components and is very stable.

A conventional denture was also made in the lower arch.

“Why are my dentures loose?”

There are many reasons why dentures can be loose:

The lower jaw has less surface area so the

denture may not fit effectively

The action of the tongue can lift the denture out

of place

You may have lost your teeth due to

periodontal disease (loose teeth). In this

condition patients also lose the quantity of jaw

bone, which is vital to holding the denture in

place.

We have solutions for all these types of issues. The

following pictures demonstrate a case in which two

implants were placed in the lower jaw to stabilise

the denture.

When you look at the denture from beneath,

there are two female components

The male and female components “lock” into one another,

which holds the denture into a locked position and

prevents it from rocking. The patient had

a similar treatment in the upper jaw.

The teeth were positioned with deliberate mild

imperfections so that the result appeared natural.

With precise planning, this case took a few months to complete.

The dentures click into place.

The patient can remove the dentures to clean them

and they do not drop out whilst eating or

speaking. The patient can now sleep

with his dentures in his mouth.

There are many ways dental implants can help to restore a patient’s

confidence. The above cases illustrate some of the routine methods

we use at our clinic to keep dentures in a comfortable position.

There is no upper age limit to denture implant therapy. If you

can have a tooth removed in a dental clinic, then you should not have

any issues with implant treatment. Of course, having this type of

treatment involves thorough planning and execution. We involve our

patients in the process guided by our expert advice.

Various types of implant treatments

There are many types of implants, including

fixed teeth, which are permanently fixed in

your mouth. The optimum treatment is based

on the clinical findings as well as the patient’s

financial budget.

As a general rule, the stability of a

denture is increased in proportion to

the number of implants. A CT Scan of

your jawbone can indicate the optimum

number of implants that can be possibly

placed.

“I am worried about having a

dental implant”

Naturally, most patients are anxious of having any form of dental

treatment. At our clinic, we do our level best to ensure that our

patient is our number one priority so that they remain relaxed and

calm.

If you are a really nervous patient then we can arrange a visit from

a special doctor called an anaesthetist at the time of your

implant placement. The anaesthetist will give you a sedative during

the procedure so that you are relaxed and his team will look after

your well-being. You will need to bring a responsible adult who can

take you home after the procedure and you just need to rest for

the remainder of the day.

We will follow up your care by phoning you the next day to make

sure everything is going well and answering any of your queries.

Naturally, you are most welcome to come and see us if you prefer.

As a general rule, if you can tolerate having a tooth removed

at the dentist then you should also be a candidate for dental

implant treatment. Of course, dental implant treatment

requires precise planning and that is the reason your

Denture Expert will take measurements to ensure optimal

planning.

Problems with jawbone shrinkage

Jawbone shrinkage occurs when the dentures in

position are constantly pressing against the gum and

jawbone. Even if a conventional denture fits well when

it is first made, this denture may become loose over

time as the jawbone shrinks. When the next denture

set is made, there is less jawbone quantity and the

subsequent denture set may not have a tight fit.

Dental implants can minimise this jawbone shrinkage.

The pressure from the dentures is eased off from the

gums and supported by the implants. Jawbone

shrinkage is therefore minimised.

“I have pain when I eat with my

dentures”

There are many reasons why dentures can be

painful on the gums:

You have dental disease and/or gum

disease

You dentures are not fitting accurately

over your gums

Your dentures are too small/big

Your teeth are loose

We would like painful dentures to be a thing

of the past. Dental implants secure the

dentures in position so that they do not rock

in your mouth.

An alternative for loose dentures

If you have good remaining teeth in your mouth, clasps can

be added to hold the denture in position

The metal clasps wrap around the existing teeth. These

clasps can also be made in tooth coloured material.

Worried about having your bad

teeth removed and not being able

to smile?

Leave the planning to your Denture Expert. We

have completed hundreds of cases. We are also expert

in routine general dentistry and can take teeth out

painlessly with revolutionary local anaesthetic

techniques. We will give you special instructions to

follow prior to your appointment so that you have a

gentle dental experience. Teeth are removed with

gentle pressure so that the surrounding bone is not

disturbed. This also ensures that our patients heal

quicker. We want our patients to be relaxed and

stress-free when we are treating them.

If you have your front teeth removed, please don’t

think we will let you go without your smile when you

walk out of our clinic. It takes some mindful planning to

avoid this situation. The following page demonstrates a

case we have recently completed.

This patient had loose front teeth which required removal. She was

very nervous and for obvious reasons did not want to go without

front teeth; neither did we want her to walk out of our clinic

covering her face:

The day her upper front teeth were extracted, a temporary denture

was provided so that the patient could smile. Please note, this

denture is not the final aesthetic denture but it was enough to see

the patient through for a couple of months until she was ready for

the final aesthetic denture. Nonetheless, the situation was a vast

improvement from her presenting issue:

Natural looking dentures

The attention to detail our Dental Technician works to. This

includes the position of the teeth, the mild imperfections that the

teeth are set at and the stippling texture on the gums.

This denture was made with pink gums, but on a transparent base

The upper and lower denture sets were fabricated on this special

machine. It is called an articulator. It simulates the way the patient

puts their teeth together and the Dental Technician can even dictate

which teeth should contact when the patient chews their food. The

Dental Technician works alongside the Denture Expert to achieve

this result.

“My dentures look unnatural

when I smile”

You can choose which level of service you prefer

denture characteristics, such as shape, colour and

texture of teeth on your final denture set.

“I would like natural looking

dentures”

This patient had a poor fitting upper denture. Two bad teeth were

removed from the front:

This denture was made using our Express Service so that she could

have her smile back in a short period of time. If you look closely near

the gums, you can see the teeth are blending in neatly:

Denture Aesthetics

This patient came to see us with a poor quality upper denture. The teeth

were too small and he had far too many gaps in between the teeth. A gold

crown had been added to the denture to give him some street credibility,

but the patient had since become a family man! The pink acrylic was

unnecessarily over-extended, which gave the appearance of an artificial

pink piece of plastic between his teeth:

The new denture was made on a transparent base so that the patient’s

gums could shine through when he smiles. Natural teeth were chosen in

accordance with the patient’s facial profile. A gap was deliberately placed

in the middle to even out the size of teeth so that nobody noticed that the

patient wore a denture:

The final result: The patient’s smile looked

natural and it was difficult to notice that he

wore a denture.

After a thorough consultation, the patient and Denture

Expert both agreed that the remaining upper teeth had to

be removed. A new denture was constructed, as shown

above. This improved her eating ability and appearance.

The patient regained the confidence of smiling. She later

decided to be placed on to our implant waiting list to have

the fixed teeth option.

Face-fitting dentures

This patient is not wearing dentures. The facial features appear

“squashed” and the face is looking unnaturally old. This situation can

also be seen in patients with old denture sets.

10 years younger: The difference that face-fitting dentures can make

to the appearance. The face is looking more youthful, natural and

the facial creases have almost disappeared.

“I have some teeth remaining. Do

I have all of them removed

dentures made?”

Some patients believe that if all their teeth are removed then it

will solve all their dental worries. In our experience, it is best to

keep healthy teeth and utilise them to hold the denture set in

place. The more teeth present, the more stable and comfortable

a denture will be. We will check to see if these remaining teeth

are healthy and have a good long term prognosis. Discuss your

options with your Denture Expert.

Later in this booklet, we have pictures of a case in which we

used the patient’s only tooth to help support the denture. This is

under the heading, “Telescopic crown denture”.

“Do I have both upper and lower

denture sets made together or do

I just get one jaw completed?”

This depends on what your Denture Expert recommends and

what you feel comfortable with. The following pictures demonstrate

a case where both the upper and lower sets of teeth required

removal. However, we concentrated on the lower set first. By using

this simple approach, the following was achieved:

We were able to use the upper teeth as a reference point so

that the patient’s bite position did not alter.

We gave a patient the chance to get used to a denture because

this was the first time he was wearing a set.

The lower denture set was constructed up until the wax try-in stage

“I have loose teeth but it does not

bother me”

It is not necessary for a patient to have symptoms or pain if they

have an existing dental problem. The same applies to decayed

and loose teeth.

It is best to remove diseased teeth, as soon as possible and

replace the gaps with dentures, bridgework or implants.

The consequence of keeping diseased teeth is that the jaw bone

is constantly fed with infection. This results in the overall loss of

jawbone. Loss of jawbone can have the following consequences:

Less jawbone for denture stability or implant placement

Aesthetic issues

Changes in facial appearance

“How do I clean my dentures?”

We would recommend that after eating you rinse

your mouth (tap water is fine; you don’t have to

use mouthwash all the time). You can rinse your

denture set under running water. A soft brush can

be used to gently polish your denture set.

Hot water should not be used to clean your

dentures, as the plastic can warp.

There is no need to keep your dentures in a glass

by your bedside! Well-fitting dentures are

designed so that you can sleep with them in.

We provide our patients with a denture care pack

and cleaning kit when we have completed their

treatment.

“I think I have gum disease and

my teeth are loose”

A tooth is surrounded by jawbone. A tooth is similar to a post

embedded in the ground. Teeth become loose when the supporting

jawbone recedes around the teeth.

The common reason why the jawbone shrinks away from the teeth

is due to periodontal disease (more commonly known as “gum

disease”).

Periodontal disease is a very common disease. Some patients don’t

know they have this disease because it can present without

symptoms. The level of periodontal disease depends on various risks

factors and your medical health status.

Periodontal disease ranges on different levels of severity. On the

minor scale, a patient could present with a small level gum irritation

which can be solved by a scale and polish. On the other side of the

spectrum, patients can require extensive treatment to save their

teeth.

The good news is that most types of periodontal disease are

treatable, which maximises the survival of teeth. Regular visits for

gum and teeth cleaning and adopting good oral hygiene methods at

home minimise the rate at which the jawbone is lost from around

the teeth.

“Does mouthwash eradicate gum

disease?”

A mouthwash containing the chemical, Chlorhexidine,

is a strong antiseptic. It is common practice to use this

type of mouthwash if your gums are heavily inflamed,

but only for a few days. Prolonged use may cause the

following:

Teeth staining

Antimicrobial resistance

Temporary loss of taste

Mouthwash does not eradicate

gum disease

Please see your dentist if you think you have this

problem.

Telescopic crown denture: This is a denture that is held in by natural teeth.

It is important to hold on to healthy strategic

teeth rather than remove them. They can help

to “lock” the denture in position.

This is a picture of the last remaining upper tooth. The Denture

Expert identified it as being healthy and decided that was in the

patient’s best interest to keep hold of this tooth. A special gold

crown was placed over this tooth (telescopic crown). This is the

“male” crown.

This is a photograph of the denture when you are looking at the fit

surface, which goes over the gums. Another special crown is

embedded inside this denture. This “female” crown locks into the

male crown and this provides extra support. It is better if more than

one tooth is present, but in this case it was sufficient to provide the

extra retention.

“Do I have metal or plastic

dentures made?”

The denture base material can be made of metal

(Cobalt-Chromium) or plastic (high impact

acrylic). Your Denture Expert will choose the

best material for your case.

Metal denture bases are firm, durable and have a

solid feel to them. However, they are mainly used

when the remaining teeth are in good condition

and have an adequate bone support around them.

Plastic denture bases are predominantly used in

those cases where the patient does not have any

teeth or where the remaining teeth have

reasonable prognosis. In order to emulate the

metal strength, we use high impact acrylic on

the dentures we provide.

Fixed bridge

A dental bridge is permanently glued in and does not need to be

removed by the patient. The teeth either side need to be healthy to

support this type of bridge.

The missing teeth have been replaced. Good colour matching

techniques means that no one will know the patient ever had missing

teeth.

Independent Dental Implants

These are a permanently fixed option. The reason why they

are independent is that no adjacent healthy teeth need to be

cut down to provide a natural looking smile.

This young patient had two missing front teeth. Providing a

bridge would have meant cutting down the adjacent teeth.

This should be avoided, if possible, as cutting down healthy

young teeth may lead to further problems in the future.
