• An initiative designed to help the newly insured understand:
– What it means to have health insurance; – How to find the right provider; – When and Where to seek health services; and – Why prevention and partnering with a provider
is important for achieving optimal health.
To equip health care providers and staff with information and resources to help them better understand and connect with newly insured patients.

– Phase I – Environmental Scan – Sought to understand population and geographic
characteristics – Captured the patients’ perspectives – Engaged the provider community – Engaged community stakeholders
– Phase II – Engagement Strategy Development – Incorporated findings from Phase I and engage partners in
the local community/communities selected – Developed materials, implementation, and evaluation
plans
– Phase III – Pilot Implementation and Evaluation

Phase II: Pilot Design and Implementation
• Builds on existing networks and uses community partners to disseminate information to consumers
• Consists of 3 cohorts: Community partners, Consumers, and Providers and office staff
• Includes 9 communities:
– Arizona: Phoenix and Yuma
– California: Oakland, Santa Cruz, and Sacramento
– Michigan: Detroit (2)
– Mississippi: Jackson and Belzoni
