It is the Unit 33, located on the third floor, which has undergone a complete refurbishment. The refurbishment has been done on some 1,526 square metres, and now offer 26 rooms (2 private ones), with a total of 50 beds. A new day-time room with ta- bles and chairs has also been built in the new Unit 33, to make hospital stay as pleasant as possible. Four of the above men- tioned rooms are suited as isolation areas for patients with contagious diseases. These rooms offer all the necessary amenities for the patient's comfort and the best working spaces for the health professionals. All bathrooms, where state- of-the art furniture has been installed, are suited for dis- abled people. For the second year, the cere- mony for the FJD Research Awards for excellence in re- search among its medical profes- sionals was held as a part of the proceedings. The award ceremo- ny was chaired by Dr. Joaquín Arenas Barbero, Assistant Gen- eral Manager of Research Evalu- ation and Promotion at the Health Institute Carlos III. Mr Arturo Canalda, Viceadvisor of Health Care and Infrastruc- tures from the Consumption and Health Care Regional Office, at- tended, among other guests, the traditional evening dinner. Dur- ing this event, he congratulated FJD on their good results achieved the past year. He high- lighted the decrease in waiting list-times, one of the main objec- tives for the Regional Office. nce again, the FJD's an-niversary was celebrated at

the Main Lecture Hall of this In-stitution. As every year, themost touching moment was theHonorary Diploma-giving cere-mony for those staff memberswho have devoted 25 years oftheir careers to the company.

For the second year, the cere-mony for the FJD ResearchAwards for excellence in re-search among its medical profes-sionals was held as a part of theproceedings. would like to take this opportunity presented to meby the publication of the sixth issue of our ImPULSO

magazine to express my satisfaction to all FJD's profes-sionals for the celebration, last February 13th, of ourClinic's 51st Anniversary and the Research Institute's71st Anniversary. Above all, it has been an honour for meto share these twelve months of work with all of them.

We are also celebrating our integration in Capio ayear ago. If I had to choose one term to define the workundergone at the Fundación during this past year, itwould probably be “personal investment”: investment ina common project, brought to life by the viability, devel-opment and ongoing improvement in data, health careand quality indicators.

The figures shown in the recently published 2005 An-nual Report reflect an intense year of efficient work andthe effort made by all our professionals.

Those figures also reflect the renewal works of hospi-talization Units, the investment in new technologies anda complete renovation of the communication systems.

Nowadays, we are facing a new challenge at the Fun-dación: the Future Plan for the coming years. This chal-lenge is giving us the opportunity to assess the Clinic'spresent situation, and to commonly plan the sort of Clin-ic we want for the future.

I have no doubts that this Plan will represent a newmilestone in the history of this Institution, where the careinterests will be integrated with those of the citizens bymeans of a clinical management, process orientation, in-ternal and external center coordination, all based on aPlan in which we all played an active role.

tional Meeting on Transla-tional Research and Individ-ual Medicine, the first multi-disciplinary meeting wherePharmacogenetic and Phar-macogenomic knowledge wasshared and updated. Themeeting was chaired and co-ordinated by Dr. Carmen Ayu-so, Chief Member of the Ge-netics department and FJD'sResearch Assistant Manager.

“Despite theoretically cor-rect diagnosis and therapeu-tics, approximately one out ofthree patients do not react ad-equately to the drug treat-ment” commented ProfessorJulio Benítez, Chief of theClinical Pharmacology Unitat the Infanta Cristina Univer-sity Hospital.

Pharmacogenetics, high-lighted Dr. Enrique Baca-Gar-cía from the FJD's PsychiatryService, “is a tool that, whencorrectly integrated withinconsensus, clinical lines andpathways, can help diminishthe variability of medicalpractice at the same time asan individual medicine is car-ried out”.

According to Dr. PedroMata, Chief Member of FJD'sInternal Medicine, “the bene-fits from the results in trans-lational research on familyhypercholesterolemia willrepresent a decrease in deathrates caused by prematurecardiovascular disease”.

FJD's Genetics and Oph-thalmology Services havebeen studying, for over 15years, the physiopathologicalbasis for retina dystrophy, a

group of progressive andhereditary degenerative dis-eases, which only in Spain af-fect some 15,000 patients, forwhom no treatment currentlyexists.

Dr. Carmen Ayuso, FJD'sChief Member of the Geneticsdepartment, commented “theknowledge of retina dystro-phy genetic basis and environ-mental modifying factors willbring about the identificationof new therapeutic targets”.

Fundación Jiménez Díazhosts a Prenatal DiagnosisUnit where translational re-search is currently being de-veloped. “Non-invasive fetaldiagnosis linked to the appli-cation of new molecular diag-nostic methods on reproduc-tive problems, are the twocurrently developing lines”,explained Dr. Carmen Ramos,FJD's Chief Member of theGenetics department.

Degenerative osteoarthritisand osteoporosis are two ofthe most frequent diseases inour society; often affectingthe same patient. The clinical

interference of these two ill-nesses on each other is a con-troversial still-not-solvedmatter. According to the re-sults obtained from animalmodel studies carried out byDr. Gabriel Herrero-Beau-mont, FJD's Osteo-ArticularPathology Lab Manager “os-teoporosis deteriorates os-teoarthritis progress. This ismost probably occurring inour patients, and implies ther-apeutic attitudes based on in-dividual medicine, which triesto offer each patient a tailoredsolution for their health prob-lems”.

SOCIAL WELFARE

Translational research mustbe regarded as a two-lanepathway: one leading from thepatient's bed to the lab, andthe opposite one, going fromthe basic lab to the patient'sbed. In any case, translationalresearch must rest on thehands of a multidisciplinaryteam made up of baseline re-searchers and general practi-tioners, all sharing basicknowledge from their respec-tive working fields.

To summarize, investing inresearch and applying it to thehealth care system affects so-ciety's welfare.

During the meeting, eightsymposia were held on the in-fluence of translational re-search on different special-ties: ophthalmology, orthope-dics and rheumatology, psy-chiatry, cardiovascular pathol-ogy, prenatal diagnosis, respi-ratory and inflammatory dis-eases, endocrinology and on-cology.

The discovery ofphi 29 polymerase is relevant tounderstand the replication mech-anisms of DNA, not just for thisvirus, but also for other virusessuch as polio, homologous serumhepatitis or hepatitis C viruses.Apart from the applications ofthis basic research work on Bio-medicine, the discovery of DNAphi 29 polymerase has also beencrucial for applications inBiotechnology, a DNA amplifi-cation system based on the DNAviral polymerase properties.

n 19th September 2005, theelectronic medical history

was introduced in the Emer-gency Service at FundaciónJiménez Díaz. Prior to this date,the computer system, which hadalready been developed in theother Capio Group hospitals inSpain, had been functioning intest mode on one of the hospi-tal's floors.

Until that day, the doctors inEmergency used to write theirreports on carbon copy paperwith all the advantages and in-conveniences that that entails.Among the advantages of thistype of report, one is clear: thefamiliar pen and paper. Tied tothis idea are the ease of execu-tion and its suitability to the tra-ditional filing method. Amongthe inconveniences are legibility,inflexibility in terms of errors orreinterpretations, excessive useof stationary material, and a ne-cessity for a voluminous filingsystem, in which files are easilymisplaced and often difficult tolocate.

NEEDS AND INITIAL IDEAS

All change generates anxiety. In-ertia when facing change is auniversal fact. To confront it,strong leadership and a clear vi-sion of the future from the Cen-ter's and the Group's manage-ment team is necessary and willhelp to combat the immediate in-conveniences that will irremedi-ably arise.

We need to rely on an IT sys-tems-company, in our case IN-DRA, and system managementdirectors with firm full support,who would not spare the techni-cal and human resources neces-sary to carry out the task.

Staff training is needed. In thecase of Emergency, this involvesa lot of people because, apartfrom the technical staff, all theon-call doctors present in thisService will need to use themedical history system. Thetraining courses have to be con-ducted during working hours;therefore they have to be short.In addition, they need to be prac-tical and very much directed to-wards the objective. Finally, theyshould be immediate - in other

words they must precede the ar-rival of the system and take placeno later than 15 days prior to itsinstallation. Once installed, thetraining staff will need to tutor atthe premises -for a few days and24/7- the hospital staff who be-gin to use the system.

One of the most frequentlyexpressed concerns in recentdays is getting behind on pa-tients' care. One must type -something that not everyonemasters- and the number of com-puters must be great enough sothat doctors do nothave to queue. Fur-thermore, the pro-gram needs to be in-tuitive and logical, inother words sensible,from the point ofview of the healthprofessional, as wellas user-friendly andvisually attractive.

Another worry re-lated to the previouspoint is that the sys-tem could go down,that it could fail atany time. As men-tioned above, the computercompany responsible for thesystem must be not only com-pletely trustworthy but it shouldalso have previous and similarexperience in the health field,and the company must under-stand that this effort is exclu-sively focused to success. Sucha company needs to be willingto give their all in technical andhuman terms. That means,among other things, expert staffavailable around the clock, sev-en days a week.

Obviously, the system in-stalled needs to be appropriatefrom the onset, but it also has tobe flexible. It must have sector-growing potential and it needs tosupport improvement from expe-rience gained from day-to-daywork. As such, the excellence ofthe IT Company and of the di-

rectors of systems managementis, once again, fundamental.

RESULTS

Nearly four months after the im-plementation of the electronicmedical history system, I can saythat the acceptance from themedical and administrative staffhas been total. The trainingcourses have been developed asplanned, with sufficient flexibili-ty to be adapted to the some-times chaotic time constraints ofthe staff.

There has been nosingle delay in patientcare. The number ofavailable computer ter-minals has been morethan enough, as has thecapability and thenumber of printers.The program has beenprogressively refinedto become user-friend-ly for the doctors' pur-poses. Common itemsare filled in by default,saving doctors' time.The system is provingto be extremely robust

and reliable, and specific prob-lems of certain terminals areovercome quickly without com-promising adequate attention topatients.

The reports are perfectly legi-ble, there are no corrections oradditions, and they are arrangedin a logical and intuitive manner.The names of the doctors incharge of the case appear in cap-ital letters, so that it is perfectlyclear who is responsible for dis-charging the patient.

The archive is electronic. Theretrieval of all information re-garding past admissions is im-mediate. Information about pre-vious admissions is availablefrom Emergency and the floorprogress of certain interestingpatients can also be followed.The system ensures the confi-dentiality of medical informa-

tion, restricting the access to cer-tain data to specific users.

The system continually incor-porates new functions. Initially,it incorporated the request formedical image results. Later, itwas adapted to order Emergencylaboratory tests, and currentlywe are looking at medical treat-ments according to the hospital'shandbook. In the comingmonths, we will be able to di-rectly access the images generat-ed by Radiology, Nuclear Medi-cine, Gastroenterology, etc.Every incorporated sector iscontinually updated and im-proved by comments, criticisms,and contributions that are dis-cussed weekly with INDRA'stechnical staff.

Although this may not con-cern Emergencies, I do not wantto finish without drawing atten-tion to the management tool thatthe electronic medical historyrepresents. One can determinewaiting and treatment times, thatis: the time it takes from admit-tance, to assessment and to beseen by a doctor or specialist. Wecan also determine the numberof patients each doctor sees in agiven period of time, the order-ing of additional tests, the med-ications administered, and manyother types of data retrieval andstudies-both descriptive and sta-tistical-worth pointing out.

CONCLUSION

In the preparatory meetings forthe installation of the IT system Itook on the responsibility of con-veying to my department staffthe inevitability of the change.That the change was going to oc-cur was as inevitable as the sunrising every day. We just had todo it under the best possible con-ditions: the best company, thebest IT system, the best profes-sionals, all necessary materialwithout financial constraints,etc. The question was notwhether or not we needed tocomputerize the medical historyinformation. The questions werewhen and how.

ue to the implementation ofThe Spanish Health Anti-

Smoking Measures Law last Janu-ary 1st, the already existing anti-smoking rules at the FJD havebeen reinforced. It implicatesworkers, patients and visitors andintends to reduce tobacco con-sumption at the hospital. At thesame time, the Prevention andWorking Health Service is coordi-nating the smoking-cessation pro-gram for those professionals whowish to follow it.

However, even though theSmoking Prevention Committeefounded in 2004 has alreadyachieved positive results, the finalgoal is to turn the hospital into acompletely smoke-free area.

FJD is part of the Smoke-freeHospital Network in the Regionof Madrid. Thanks to its participa-tion and the help of a supportingstructure and organization forkicking the smoking habit, thecenter has been given the chanceto participate in the Regional Planfor Smoking Prevention and Con-trol implemented by the RegionalDevelopment and Public HealthGeneral Office.

A new inscription period hasbeen opened for all FJD profes-sionals who wish to follow thesmoking-cessation program,funded entirely by the center.Those interested should contactthe Workplace Health Service(Ext. 3259) to sign up and inquireabout timetables.

Díaz's integration in Capio,the first “Capiometer” wascarried out amongst all staffmembers. The “Capiometer”is a survey to study thescope and development ofCapio's values and it is sentto all the centers whichmake up the company.

This 34-question surveyis a tool for all professionalsto express themselves andgive their opinion on severalmatters such as communica-tion, organization knowl-edge, development and pro-motion potential, etc.

It is an anonymous andconfidential yearly surveyand is done simultaneouslyat all Capio Group Europeancenters.

■ A GROUP OF SWEDISH POLITICIANS visited FundaciónJiménez Díaz at the end of 2005. The purpose of their visit was tolearn about our hospital's operations at first hand, as it is a mem-ber of the Swedish Capio Group.

MrJulio Albisua was in charge of theintroduction while the lecturerswere Mr Ignacio Basurte (Psy-chiatry Improvement Group), MsMarisa Sánchez (Admission Im-provement Group), Mr CarmeloFernández (Rehabilitation Im-provement Group) and Ms SilviaPolo (Kitchen ImprovementGroup). The self-assessment re-sults -according to the EFQMExcellence Model- were present-ed at the session. These resultswere obtained by the Managing

bodies, the Quality Core Promot-er, the Process Proprietors and theQuality Unit (methodologicalsupport). The EFQM Model'snine criteria were assessed; theresults are shown on the attachedtable. The results of an InternalQuality Audit were checked at thesession held in October, where allprocesses were revised, findingthe improvements accomplishedby the Working Groups.

Since last January, the peopleresponsible for the Quality Unithave been holding different meet-ings with the Managers and theProcess Proprietors to define the2006 Quality Plan.

The day began with a mass atthe Clinic's Chapel, followed bya Pathology Clinical Session on a“51-year-old HIV-positive pa-tient, with right flaccid hemi

paresis and brain damage”, intro-duced by Dr. Rafael Aldai, fromthe Neurosurgery Unit in 12 deOctubre Hospital.

After the clinical session, theHonorary Diploma-giving Cere-mony took place, chaired byProf. Julio R. Villanueva, Presi-dent of the FJD's GoverningCouncil who congratulated theaudience for the good function-ing of the Institution and the per-sonnel's effort.

Representing the honorarymembers, Valentín Calvo Her-rero, librarian at the FJD, sum-moned up with great affectionhis memories during these pastyears. To put an end to his

Dr. Arenas pointed out the relevanceof such initiatives as a recognizing andhelping tool for researchers. He also en-couraged the FJD's managing body tocontinue supporting the hospital activi-ty, which is a point of reference for cer-tain research areas. The Assistant Gen-eral Manager of the Institute Carlos IIIreminded those in attendance of the sci-entific background of the Fundaciónfrom its very beginnings, and highlight-ed the fact that its model based onhealth care, teaching and research hasbecome the reference point for modernhealth care systems.

Dr. Carmen Ayuso, FJD's AssistantResearch Manager, encouraged the In-stitution's professionals to take advan-tage of the help offered by these awardsto finalize their training and also ex-plained the strict evaluation systemused to assess the participating works.

In my opinion, the fact of fac-ing pain and suffering day after

day has taught us to become im-mune to it. Every day, we try andcool down our feelings to avoidour personal life feeling threat-ened by a feeling of pity and com-passion. But this defence mecha-nism, key to keeping our psycho-logical integrity, must not eraseour reaction capacity towards theinjustice around us.

That is why, the training doc-tors at Fundación Jiménez Díaz, incollaboration with Action AgainstHunger, have decided to begin asmall awareness project amongthe Fundación's personnel. It is ouraim to bring closer some real situ-ations, which in many cases arejust a three-hour-flight away.

We do not wish to get lost indead-end winding pathways or inabstract ideas never brought tolife; we invite you to participatein feasible projects, with specificmeans and goals. The objective isnot just about improving the star-

vation situation or the lack of ac-cess to health care for some peo-ple, but to modify our under-standing of reality, realize the re-al scope of our problems and feellittle bits of a whole.

The means… well, it would beredundant to say they are scarce;you all know it already. To solvethis, we are planning on imple-menting different fundraisingmechanisms this year. Thesefunds will be invested in projectsin those countries that need themthe most. They will be consolidat-ed projects, feasible projects, inwhich you may take part… Themain mechanism is already up

and running: at the nursing sta-tions and in many other hospitallocations, you will find ActionAgainst Hunger collaborationleaflets. By filling in and sendingone of these leaflets, you will helpfight against starvation in Malawi,especially amongst children. Ac-tion Against Hunger will organizethe investment of the collectedfunds on the different activities inthis country: opening Supplemen-tary Nutrition Centers, organizingeducational sessions on healthand nutrition in the different com-munities…, remaining alwaysaware of any emergency situationthat may require immediate ac-

JD's Prenatal DiagnosisUnit was founded in

1977, becoming the first of itskind in Spain. It is also a re-search center, which, sincethe 80s, has carried out an im-portant work of biotechnologydevelopment and integrationprocess into prenatal diagno-sis.

Since then, the members ofthis Unit have integrated allupdates, both obstetrical andanalytical, directed to a widefetal diagnosis. It is currentlya reputable and prestigioustraining center for other spe-cialists in prenatal diagnosis.

The Prenatal DiagnosisUnit consists of: Genetics andPrenatal Diagnosis Consulta-tion located on the first floorand the Obstetrical Scan Uniton the 4th and 2nd floorswithin the Gynecology Ser-vice.

s Esperanza Aguirre, President of the Au-tonomous Region of Madrid, presided

over the ground-breaking ceremony of the fu-ture Capio Valdemoro Hospital, a project in-cluded in the Community's Plan for Health In-frastructures.

An estimated 72,000,000 will be investedin building this new hospital. Capio's conces-sion includes the center's building and man-aging works for a 30-year period. After itsplanned opening in 2007, the hospital will in-clude 133 beds and will benefit over 100,000inhabitants from the towns of Ciempozuelos,San Martín de la Vega and Titulcia, all on theoutskirts of Madrid. Firstly, it will hold a to-tal of 8 operating theatres, 2 recovery andbirth wards, 35 emergency positions, includ-ing boxes and care and observation stalls; 38external consultations, 42 examination of-fices, 10 radiology wards and up to 28 dialy-sis stalls. These figures will be gradually in-creased until 2017, when the forecasted num-bers are 10 operating theatres, 4 birth and re-covery wards, 44 emergency stalls, 107 exter-nal consultations, examination offices and ra-diology wards.
