Be It Further Resolved, that the Board authorizes the District Clerk to execute said agreement on their behalf.

Be It Resolved, that the Board of Education approves the agreement between the Garden City Union Free School District and Maxim Healthcare Services for related services during the 2011/2012 school year at the hourly rates of $72 for occupational therapy, $62 for physical therapy, $72 for speech and language, $42 for registered nursing services, $35 for licensed practical nursing services, $18 health aide/certified nursing assistant and $28 for behavioral class aides/behavioral consultation.

37. Be It Resolved, that the Board of Education approves the agreement between the Garden City Union Free School District and Winston Staffing Services, LLC for nursing and nurse’s aide services during the 2011/2012 school year at a cost of $18.75 per hour for nurse’s aides and $52 per hour for nursing services.

38. Be It Resolved, that the Board of Education approves the agreement between the Garden City Union Free School District and Quality Evaluations & Consulting Services for evaluations during the 2011/2012 school year at a rate of $700 for psychological evaluations; $600 for educational evaluations; $550 for speech evaluations; $350 for social evaluations and $250 for occupational therapy evaluations.

39. Be It Resolved, that the Board of Education approves the agreement between the Garden City Union Free School District and Zycron Industries for Medicaid billing services for 15% of monies actually received and retained by the District as a result of claims submitted by consultant from Federal and/or State government for services rendered by the District to disabled students (service date of July 1, 2009 through contract year).

40. Be It Resolved, that the Board of Education approves the agreement with Spectrum Physical Therapy and Chiropractic of Levittown for a head athletic trainer for the period of August 16, 2011 through June 16, 2012 at a fee of $32,000 and for an assistant athletic trainer for the period of August 16, 2011 through June 16, 2012 at a fee not to exceed $9,000.
