2017 Youth Golf Program

GHA’s free summer youth golf program, sponsored by CSL Behring, kicked off on May 25, 2017 with a BBQ and presentation by golf pro Perry Parker at Quail Creek Golf Club. Following dinner, the kids practiced their swing with the help of Perry and

program leader Luis Aguayo. As a “Green Light” sport, golf is one of the National Hemophilia Foundation’s approved sports for children with bleeding disorders.

The youth golf program continued twice a week for 4 weeks at Quail Creek Golf Club and featured lessons on putting, chipping, driving, and body alignment.

Participants in GHA’s 2017 youth golf program were also eligible to apply to CSL Behring’s Gettin’ in the Game Junior National Championship, taking place in Phoenix, AZ from November 3-5, 2017. This event includes educational sessions for parents and caregivers, clinics for kids, and a chance for kids to compete against their peers from around the country.

The U.S. Food and Drug Administration (FDA) has approved expanded labeling for Epclusa®, a combination therapy for adults with chronic hepatitis C viral (HCV) infection. Manufactured by Gilead Sciences, Inc., Epclusa® encompasses two HCV therapies (Sofosbuvir/Velpatasvir) in a fixed-dose, orally administered tablet. The therapy first received FDA approval in June 2016 with an indication for all six major genotypes of the virus in patients with chronic HCV, both with and without cirrhosis. The expanded labeling will now also include patients co-infected with HIV.

The expanded FDA approval was based on data from the phase 3 AS-TRAL-5 study designed to evaluate 12 weeks of treatment with Epclusa® in 106 subjects with genotype 1-4 HCV infection who were co-infected with HIV and on stable antiretroviral therapy. The data showed that 95% of these patients (101/106) reached a sustained virological response by exhibiting undetectable hepatitis viral loads 12 weeks after completing therapy.

“HCV co-infection remains a major cause of morbidity in HIV-infected individuals. With this expanded use, Epclusa provides co-infected patients with a much needed one-pill-a-day regimen that works across all HCV genotypes and is compatible with widely-used antiretroviral regimens,” said David Wyles, MD, Chief, Division of Infectious Disease, Denver Health Medical Center; Associate Professor of Medicine, University of Colorado School of Medicine. “With Epclusa, physicians have an important new treatment option for their HCV/HIV co-infected patients.”

The updated label for Epclusa® also includes an FDA-imposed “boxed warning” specific to the risk of hepatitis B viral (HBV) reactivation in patients who had been co-infected with HCV/HBV. The warning advises that all patients with evidence of current or prior HBV infection should be screened prior to initiating treatment with Epclusa®.

The boxed warning coincided with an FDA safety announcement issued in October 2016 about the risk of HBV becoming an active infection again in any patient who has a current or previous infection with HBV and is treated with certain direct-acting antiviral (DAA) medicines (including Epclusa®) for HCV. The impetus for the warning were a handful of cases of HBV reactivation in patients treated with DAA drugs, resulting in serious liver problems or death.

The National Hemophilia Foundation’s (NHF) Medical and Scientific Advisory Council has also issued a pair of recommendations relevant to HCV treatment and screening. Access these documents on the NHF website:

GHA’s 21st Annual Family Education Weekend was held July 21-23, 2017 at the DoubleTree Chesterfield Hotel in Chesterfield, Missouri. Our largest FEW to date brought together 67 affected families, 46 industry members, and numerous volunteers and speakers, totaling 270 individuals who came together for a weekend of education, community, and fun.

Friday began with FEW’s first Advocacy Pre-Con, where GHA ambassadors sat down with GHA’s Executive Director, Joy Krieger, Executive Director of the Asthma & Allergy Foundation of America, and Deema Tarazi of the Hemophilia Federation of America to discuss legislation currently affecting the bleeding disorders community and ways to advocate for our community with legislators at a state and federal level. The advocacy pre-con was sponsored by CSL Behring.

That evening, attendees were welcomed by Bridget Tyrey and the GHA Board of Directors to the “Meet Your Board” and Major Donor Recognition Reception. At the reception, major donors from 2016 were recognized for their support and families explored the industry exhibit hall while enjoying drinks and appetizers. At several times throughout the day, community members could also visit our Infusion Suite to learn about self-infusion from one of the wonderful infusion nurses who volunteered their time and expertise at FEW. Thank you to nurses Peggy Devoy, RN, BSN, CRNI, Lydia Johnson, RN, BSN, Kathy Logan, RN, BSN, CRNI, Sue Pardeshi, RN, BSN, and Becky Toman, RN, BSN, CRNI.

While the adults were busy enjoying the breakout sessions, the kids and teens had their own programs to participate in. Children age 12 and under enjoyed a fun-filled weekend full of activities tied in with our theme of “Bleeding Disorders in Our World.” For a full day on Saturday and the morning on Sunday, kids were treated to a variety of adventures “around the world” created by GHA board member, Darlene Shelton. This was supported by a team of volunteers: Danny Shelton, James & Sandy Addie, Pam Carleton, Judy Bagato, Rylie, Brett, & Alyssa Frizzell, Selena Flannigan, Tesa Martin, Travis Probst, Hanna Smith, Celina, Kari Moellenhoff, Kim Pustinger, and educators from The Humane Society of Missouri. Thank you to every one of these amazing volunteers.

My Life, Our Future is a nationwide campaign for progress in hemophilia. Leaders in our community have come together to offer free genotyping so that participants may gain a deeper understanding of their hemophilia today, while helping to advance the breakthrough treatments of tomorrow.

Testing for MLOF will be ending December 31, 2017. Contact an HTC to schedule your genetic screening before the deadline. The focus of the program is now shifting, as planned, to the utilization of the MLOF Research Repository. The MLOF Research Repository is now ready to support research and analysis to benefit the care of people with hemophilia, including carriers/women with factor deficiencies.

The teens participated in the “Teen Track” led by Dan and Kristin Shelton. The teens’ breakout sessions included Advocacy in the Streets and Kinesio Taping, followed by a scavenger hunt at the Taubman Prestige Outlet Mall, swimming, and a Teen Lounge.

Tyrey Volunteer of the Year” award, in recognition of all she has done and continues to do for the bleeding disorders community, including the time, thought, and care she puts into Camp Notaclotamongus. GHA also recognized Trudy Stringer, whose tireless work in advocacy has advanced the causes of the

Choosing the Factor Replacement that is Right for You - by Morey Blinder, MD

Making choices is something we do every day. Supermarket

shelves are stocked with similar products. Ice cream parlors

offer dozens of flavors. Retirement funds provide a menu of

options to invest money. It is not surprising that we may reach

a point of choice overload in which whatever choice we make,

we worry that another choice would have been better.

Making decisions about our health can be even more difficult.

The information needed to make better choices is often

lacking or of poor quality so we depend on others to help us

with these decisions. While it is critically important to have

guidance and support in medical decision making, it is also

important for individuals and their families to have an

understanding about treatments that affect them. Rarely in

medicine do we find disorders with more treatment options

than in hemophilia and von Willebrand disease. Currently, 15

factor VIII replacement products are available for hemophilia

A, 10 factor IX replacement products are available for

hemophilia B and at least 5 products exist for von Willebrand

disease. What follows is a guide to highlight the choices for

bleeding disorder patients to help make the best decisions on

your factor replacement.

Source material

An important distinction to be aware of is to know the source

material used to make the factor replacement products.

Plasma-derived products, available for more than 40 years, are

pooled and processed from donated plasma. While the

earliest products were often the source of viral infections such

as HIV and hepatitis, advances in science since 1987 have

allowed for the development of methods to insure that these

infections are no longer transmitted. Recombinant products,

first available in 1992, are produced in cultured cells that have

had the human gene for factor VIII, factor IX or von Willebrand

factor inserted to make the product. Notably, both plasma-

derived and recombinant products are treated to remove any

contaminants and sources of infection.

Product Characteristics

Other features that vary among the replacement products

are the “shelf life” (storage time prior to reconstitution), the

product stability after reconstitution called the “half life” and

the presence or absence of additional proteins to stabilize the

product known as the “generation.” It is unlikely that any of

these characteristics become a deciding factor in product

choice but they may be important under certain

circumstances. For example, a product that is infused

infrequently may “expire” on your shelf before it is used and

so having products that have the longest shelf life might be

best. Lastly, products are available in a variety of vial sizes and

this may be important in some situations to achieve the most

precise target dose.

Effect of Products on Inhibitor formation

One of the most important considerations is understand the

risks of inhibitor formation. While several factors seem to

play a role in the development of inhibitors, including the

number of treatments received as well as the specific genetic

mutation that is identified, the type of factor that is infused

may also contribute to the risk. Most notably, the SIPPET

study published in 2016, suggested that inhibitors were less

likely to form in patients receiving plasma-derived compared

to recombinant products.

Extended Half life Products

Over the last 3 years, another important development is the

use of extended half- life of products for both hemophilia A

and B. These products are all recombinant and offer the

potential of fewer infusions to maintain safe levels of factor

replacement. While these products offer modest

improvements in hemophilia A, they are particularly helpful

for hemophilia B patients, decreasing the number of infusions

needed over time to maintain good prophylaxis.

Summary

Choosing wisely involves identifying your needs and

preferences and working with your providers to optimize

treatment decisions with the currently available products

while keeping informed on the future developments.

Insights from NHF’s 69th Annual Meeting - by Bridget Tyrey, GHA Executive Director

NHF’s 69th Annual Meeting was held in Chicago, IL

August 24-26, 2017. This was an amazing opportunity to learn about the latest innovations and products in the bleeding disorders community along with approximately 3,000 people from all over the country. Community members took advantage of opportunities to meet new friends and catch up with old, as well as choose from a selection of 9 different tracks consisting of three days full of educational sessions, networking opportunities, and access to the exhibit hall. At Opening Session, Val Bias, NHF CEO stated,

I asked a GHA member who attended NHF, what their biggest take away was from the Annual Meeting. Tyler K. said,

“NHF 2017 was my first conference, as well as the first time ever visiting Chicago. NHF was a fantastic way to network and be able to see all of the available resources for people with bleeding disorders, from ultrasounds to gene therapy. There is truly something to be learned for everyone. I could not believe the amount of people that are affected by bleeding disorders all in one place. The opening session really helped bring it together to make everyone feel like there was always an "us" instead of me or I.

I have taken away many things from my first NHF.

1. Medic alerts are very important! I only used to wear mine for long trips or vacations; I am now looking into ordering one to wear all the time.

2. Another thing I learned is just because your joints aren't bothering you doesn't mean you may not have a bleed. Micro bleeds are very common and can wreak havoc on joints.

3. I also spoke with a representative of the factor I take and found out more options for their dosage which included different vial sizes.

4. One of the most important things I’ve learned is to be an advocate for yourself and the community. Know your options for treatment and explore and research for yourself and discuss these other alternatives with your doctor. I used to always just rely on my doctor for information and after attending NHF I feel more informed than ever. At the end of the day it is your health that you must protect.

I hope I will be able to attend many more NHF’s in the future to keep learning and networking with those in the community.”

This year eight families received scholarships to attend NHF within the GHA community. When I asked Lori M. why she would recommend that families attend NHF, she stated, “We would recommend NHF for the fun, fellowship and some of the BEST classes we have attended. We met new families that are in the same boat as our family. We received some new tips on how to deal with bleeding disorders. We learned how big of an organization NHF is, that they REALLY do care about each one of us.”

“We are more than our bleeding disorders. We are people who work, go to school, play sports, have kids. We are people first. We are lucky to have access to so many nurses and doctors, PTs and social workers, who don’t view us as a collection of symptoms. They see us as people. Our commitment to access to care is not just about public policy, and it’s not just about research. It’s about investing in people who care for us.”

Austin H. was asked what he learned from the meeting. He stated, “The cure for hemophilia talk to the stigma of AIDS is such a variety of topics. My most memorable part of NHF was learning about the people and their families that went through the AIDS epidemic, which showed that we can't forget the ones we lost. Some of the stories shared were very memorable and we can learn from them. It was beneficial and very interesting learning about gene therapy and the cure for hemophilia. I believe our technology is advancing and I will definitely keep an eye out for the new advancements.”

Insights from NHF’s 69th Annual Meeting (cont’d)

I attended NHF this year as the Co-Chair, which was an honor to meet so many wonderful individuals working to better the world of bleeding disorders. This community continues to inspire me. It was a pleasure on behalf of GHA’s Board of Directors to accept the NHF Partnership Chapter of Excellence Award. GHA scored in the top 15% of all chapters reviewed in this category. For this award, we received a travel scholarship to the 2018 NHF annual meeting in Orlando, FL. On August 17, 2017, over 160 wine lovers gathered at Farotto’s Pasta & Pizzaria in Rock Hill to raise money for GHA’s Matthew Wilson Memorial Scholarship Fund. Event-goers were treated to beautiful weather, a selection of wines donated by PRP Wine International and Bommarito Wines, delicious hors d’oeuvres from Farotto’s, a silent auction, and live music. For an additional donation, attendees could partake in a special cigar and liquor tasting.

Thanks to the generosity of our donors and sponsors, GHA will be able to award $9,000 in scholarships to college students with bleeding disorders for the 2018-2019 school year.
