Kaiser Permanente, et al.

To approve order accepting payment in the amount of $2,255.34 from StateFarm Insurance for their insured, John Sheller, as reimbursement of propertydamages sustained by Dallas County on or about June 2, 1997, in connectionwith a lawsuit filed by Dallas County in Cause No. JC-99-02493-A, styled DallasCounty v. John Sheller.

12. COURT ORDER:

To approve order accepting payment of claim in the amount of $593.60 fromCrawford Insurance for their insured, Billy Wayne McPherson, as reimbursementof property damages sustained by Dallas County on or about June 30, 2000.

13. COURT ORDER:

To approve order authorizing William Cyran to be paid $1,296.70 and State FarmInsurance to be paid $600.00 as full settlement by the County of Dallas, Texas,for incurred car rental charges in connection with an accident with a Countyvehicle on or about May 8, 2000.

14. COURT ORDER:

To approve order authorizing Gilbert Marcus to be paid $1,237.73 as fullsettlement by the County of Dallas, Texas, for property damages in connectionwith an accident with a County vehicle on or about April 28, 2000.

To approve order appointing presiding and alternate election judges to holdSpecial and General Elections for the 2000-2001 term, which begins August 15,2000 and ends July 31, 2001 .

To approve order appointing early voting judges for listed early voting locationsto hold Special and General Elections for the 2000-2001 term (8/15/2000through 7/31/2001) and appointing an early voting ballot board judge andalternate early voting ballot board judge.

To approve order approving Amendment Number 2 to the Agreement forEnhanced Emergency Number Service (E9-1-1) between Dallas County and GTESouthwest, Inc., and authorizing the County Judge to sign said document.

To approve order approving an agreement with the Dallas County CommunitySupervision and Corrections Department (DCCSCD), whereby Dallas CountyHealth and Human Services (DCHHS) will provide phlebotomy services at a flatrate of $3.00 per draw for the period from September 1, 2000 through August31, 2001, and authorizing the County Judge to sign said agreement on behalf ofDallas County.

26. COURT ORDER:

To approve order approving an additional $412,992.00 allocated by the TexasDepartment of Housing and Community Affairs (TDHCA) under the 2000Comprehensive Energy Assistance Program (CEAP) contract, and approving theoperational budget as submitted.

27. COURT ORDER:

To approve order approving the request for Ryan White CARE Act Title II fundsfor the purchase of capital equipment by currently funded HIV service providers,with Dallas County Health and Human Services to be responsible for completingthe request and submitting it to the Texas Department of Health (TDH).


28. COURT ORDER:

To approve order extending current contract (Bid No. 97-265 - Contract for thePrinting and Mailing of Tax Statements) with KPT, Inc. for the period from August12, 2000 through November 30, 2000 to allow the Tax Office ample time toprocess the October Property Tax Statements and any supplemental mailingswithout any delays or programming changes to the current tax statements, dueto the on-going consolidation conversion taking place in the Tax Office.

29. COURT ORDER:

To approve order extending current contract (Bid No. 98-287 - TravelManagement Services) with Wyndham Jade, based on a not-to-exceed date ofNovember 30, 2000, to allow ample time to revise the bid specifications, briefthe Court, solicit and award a new contract.

To approve order authorizing on-line purchasing from Dell Computer Corporationand authorizing the County Judge to execute an agreement between DellMarketing L.P. and Dallas County for said paperless purchase orders.

31 . COURT ORDER:

To approve order authorizing final payment in the amount of $65,254.00 toKONE, Inc. for the 1999 Major Elevator Upgrade (Bid No. 99-045) andacceptance of the contract as complete.

32. COURT ORDER:

To approve order authorizing the County Judge to execute Change Order No.1,in the increased amount of $7,718.00, to the contract with K C ConstructionServices, Inc. for the Dallas County Tuberculosis (TB) Laboratory Negative AirRepairs project (Bid No. 2000-034-237).

To approve order authorizing the County Judge to execute a Five Year, Management Services Contract with Central Parking System of Texas, Inc. for the

George L. Allen, Sr. Underground Garage, beginning September 1, 2000 andending August 31, 2005.

To approve order authorizing Commissioner of District No.2 to cooperate withthe City of Sachse in the emergency repair of the damaged bridge on SachseRoad for an estimated cost to the County of $24,725.00 forthis Type "B" roadwork.

To approve order authorizing the Public Works Property Division to conduct anappraisal to determine the fair market value of property in Windmill Hill Preservefor which the City of DeSoto has requested a utility easement.
