This is our 2014 Genesis Neighborhood Association, Inc. fourth annual report to the Mayor and Commissioners of the City of Lake Worth. Thanks to some generous corporations and non-profits we have evolved into Genesis Foundation managing The Leocadia Fonseca Endowment name after my departed grandmother who passed away several months after a massive heart attack at 88 years of age.TRANSCRIPT

Genesis Acquires Four long term Sponsors 3. 2014 Action Items Signature Healthcare committed to sponsor four community benefits projects One completed 921 South B Street First Recipient - Ms. Sue Grant Three more by end of 2014 Backyard BBQ Fundraiser to be held at Ms. Grants home with the new grill and patio set that Home Depot donated. Tickets will be available for purchase at www.genesislakeworth.org in the near future. All proceeds will fund The Leocadia Fonseca Endowment 4. Sue Grant and the Signature Healthcare Earthbound Angels 5. 2014 Overview Genesis Neighborhood Association is being restructured into The Genesis Foundation. $1,209.00 grants will be available for non- profits throughout the entire City of Lake Worth through the Leocadia Fonseca Endowment starting January 2015. 6. 2014-2015 Operational Timeline 7. Looking Ahead 2015 Genesis Foundation/Signature Healthcare/Hospice of the Palm Beach/The Shepards Heart AME Church Of Royal Palm Beach/WebMark Digital Productions joint planning meetings. 8. Conclusion Partnered with multiple community benefit sponsors Rolling out Genesis Foundation/The Leocadia Fonseca Endowment Paint three (3) more houses within the Genesis Neighborhood boundaries Complete Genesis Foundation/Leocadia Fonseca Endowment by- laws Meetings with legal team for formalizing of Genesis Foundation 501(c)3 9.
