Adult Mental Health Services Recovery-based programs are available and accessible across Georgia. Many individuals approach the state service system looking for help. The Georgia Department of Behavioral Health and Developmental Disabilities (DBHDD) supports services for people with a diagnosed mental illness, whose level of functioning is significantly affected by the mental illness, and who are unable to pay for all of part of the services. The services a person receives depend on both a professional determination of need and the services and other community resources available.

Community-Based Mental Health Services

Community-based services are provided through contracts with private, for–profit, nonprofit, and quasi–public agencies under contract with DBHDD through the department’s regional field offices. Consumer choice is a value that is embraced throughout the system and is fostered through the development of different kinds of provider agencies including consumer-operated agencies. These organizations vary in scope of services provided, including those services that address more individualized needs.

1. Disaster Mental Health Services: In accordance with the Georgia Emergency Operations Plan Emergency Support

Function (ESF) 8 Public Health and Medical, DBHDD is responsible for behavioral health and community disaster mental health services during times of emergency, disaster or terrorist strike. Maintenance of continued behavioral health services during emergencies and disasters is essential. Under ESF 8, DBHDD is also required to offer mental health care to disaster survivors, bystanders, responders and their families, and other community caregivers. DBHDD has a comprehensive and coordinated plan for preparation, prevention, response, recovery and mitigation of behavioral health and community disaster mental health services for Georgia. Georgia’s disaster mental health website (www.georgiadisaster.info) provides resources that individuals, families, businesses and organizations can use to develop their disaster plans. 2. Pre-Admission Screening and Resident Review (PASSR): Medicaid regulations require states to maintain a PASSR program to screen nursing facility applicants and residents for serious mental illness. The purpose of PASSR is to ensure that nursing facility applicants and residents with mental illness and/or intellectual and developmental disabilities are

identified and admitted, or remain, in a nursing facility only if they can be appropriately served in that facility and provided with needed services including specialized mental health and intellectual or developmental disability services.

3. Projects for Assistance in Transition from Homelessness Program (PATH): Homeless outreach teams in Atlanta,

Augusta, Columbus, and Savannah go to the streets and homeless shelters to identify homeless individuals with mental health challenges. The teams engage these individuals in treatment and connect them to mental health services and mainstream resources to help end their homeless cycle. 4. SSI/SSDI Outreach, Access and Recovery (SOAR): SOAR is national project funded by the Substance Abuse and Mental

Health Services Health Administration (SAMHSA). It is designed to increase access to SSI/SSDI for eligible adults who are homeless or at risk of homelessness and have a mental health challenge and/or co-occurring substance use disorder. The SOAR Technical Assistance Center uses a three-pronged approach to strategic planning, training, and technical assistance to coordinate efforts at the state and community levels.

How to Access Adult Mental Health Services Georgia Crisis and Access Line (GCAL) 800.715.4225

DBHDD provides a statewide toll-free call center for consumers to access services. The call center operates 24/7 and has the capacity to screen and assess callers for intensity and service response.

Language Assistance: Telephone interpreting services are provided to callers with limited English proficiency. Emergencies: Individuals determined to be in immediate danger are “warm transferred” to the local 911 service in the area

where the consumer is located. Call center staff do not leave the call until they have confirmation that 911 responders are on-site with the caller.

Crisis Assistance: Individuals in need of crisis management may receive 24/7 mobile response to assess the situation, de-escalate the crisis, and for post-crisis follow-up to assure links with recommended services.

Routine Service Access: For consumers with less intense needs, the call center staffs are able to offer consumers choice of providers and to schedule appointments for services. Consumers are able to specify the distance they are willing to travel. Call center identifies service providers near the individual.

Assertive Community Treatment Assertive community treatment (ACT), is a consumer-centered, recovery-oriented, and highly intensive service that offers access to a variety of interventions 24 hours a day, seven days a week. ACT is a community-based alternative to hospitalization for people who have a severe and persistent mental illness (SPMI) which has interrupted their ability to live in the community successfully. In additional to having an SPMI, ACT enrollment criteria may include:

• Frequent admission to psychiatric hospitals • Significant functional impairments due to mental illness • Homelessness or risk of becoming homeless • Incarceration for reasons related to mental illness • Co-occurring substance abuse disorders

ACT is an option for people who have not had success in traditional outpatient behavioral health programs. It is often referred to as a “hospital without walls” because it provides a full range of treatment and supports that enable individuals with SPMI to live in the community. ACT supports recovery through skill-building, advocacy, case management, and psychiatry. ACT is administered by a multidisciplinary team of behavioral health professionals that consists of a team lead, psychiatrist, registered nurse, certified peer specialist, licensed clinician, vocational specialist, substance abuse counselor, and a paraprofessional. The teams provide intensive, community-based interventions, including: rehabilitation, crisis planning, mental health counseling, substance abuse treatment, medication management, housing and employment assistance, transportation, and other integrated services. ACT is a unique treatment model in which the majority of mental health services are provided directly by the ACT team in the consumer’s natural environment, which may be his or her home. DBHDD supports 22 ACT teams serving more than 1,700 people in Georgia. Teams typically serve between 75 and 100 consumers. ACT teams receive referrals from providers, clinics, hospitals and jails. ACT teams also coordinate transitions to the community for individuals being discharged from a hospital or jail. A large majority of ACT services are provided in the field, rather than in an office setting. Teams are also on call 24/7 to provide crisis intervention outside of regularly scheduled appointments. Because serious mental illnesses are episodic disorders and many consumers benefit from the availability of long-term treatment and continuity of care, individuals may remain in ACT as long as eligibility is met. This flexibility allows consumers to experience gains and setbacks along the process while continuing on the overall road to recovery.

Case Management Case management addresses the needs of individuals diagnosed with a mental illness who need help to successfully implement their individual recovery plan (IRP) and maintain their recovery in the community. Case management provides environmental support and care coordination that help the individual improve his or her functioning, gain access to necessary services, and create an environment that promotes recovery as identified in the IRP.

A case manager works closely with the individual, physician and other recovery-supporting service providers in the individual’s home and community to help:

1. Develop natural supports to promote community integration 2. Identify service needs 3. Refer and link the individual to services and resources identified through the service planning process 4. Coordinate services identified in the IRP to maximize service integration and minimize service gaps 5. Ensure continued adequacy of the IRP to meet the individual’s ongoing and changing needs

Community Support Team Community support teams (CST) address the intensive needs of individuals with severe and persistent mental illness living in rural areas who are difficult to engage in treatment and have a history of

• Multiple inpatient hospital admissions or extended stays, • Multiple discharges from crisis stabilization units, or • Discharge from correctional facilities or other institutional settings.

CST is a restorative, recovery-focused intervention that helps individuals access services, self-manage their illness, develop independent community living skills, and achieve stable living arrangements.

A multi-disciplinary team comprised of a licensed clinician, registered nurse, paraprofessional(s), and a certified peer specialist work closely with the individual, physician, and other recovery-supporting service providers to help the individual become as independent as possible, and ensure continuity of care without disruption. Team members deliver the majority of interventions in the individual’s home or community. Services include nursing, symptom management, medication administration, referral and linkage, care coordination, individual counseling, skills training, relapse prevention, and crisis planning.

Crisis Respite Apartments Crisis respite apartments offer a brief period of respite for individuals needing a supportive environment. This service is available:

1. When transitioning back into the community from a psychiatric inpatient facility, crisis stabilization unit (CSU) or 23-hour observation area or;

2. When preventing an admission or readmission into a psychiatric inpatient facility, CSU, or 23-hour observation area for an individual who can be safely served in a voluntary, community-based setting. Crisis respite services include individualized engagement, crisis planning, linkage to treatment, and other community resources necessary for the individual to safely reside in the community. Transportation assistance may be provided when needed to access appropriate levels of care. A typical period of crisis respite is 14 days or fewer, but an individual may stay in crisis respite for up to 30 consecutive days.
