Gestational trophblastic disease (GTD) is a rare tumour of placental origin. There is

a range of malignant potential from the relatively benign (premalignant) Hydatidiform

Mole which occurs about 1 in 1,500 pregnacies to the much rarer malignant form

(Choriocarcinoma, invasive mole, Placenta site trophoblastic disease and the new entity

which called epitheliod trophoblastic tumor. Its incidence is various depend on the

location from 1: 85 in Indonosia to 1; 1500 in Europe and USA . In Saudi Arabia the

incident is 2.6 per 1000 pregnancy. The chance to have chemotherapy after evacuation

oh moler pregnancy depends were you live if you are in UK is 7 % and in Canada is

30% and in Saudi Arabia is 25%.

The Journey of GTD began in 1956 when the sensitivity of gestational trophblastic

neoplasia (GTN )was found to Methotraxate . In 1970 the great Bagashow was used

the folonic acid to decrease the side effect of Methoraxate. and introduce the EMACO

regimen in the management of High risk GTN. Between 1970 and 1980 Actinomycin D

were introduced. different protocol in the management is used which has basically

similar good prognosis.

Since year 2000 we globally thankfully reach to a kind of uniform and

classification system which started to show effect on the way how to report the data

and compare data together. The scoring system validated in 2000 and adapted by

WHO and FIGO and its current classification most of us follow hopefully.

In the UK there is three registration for GTD and 100 % of patient with GTD

registered and death from GTN is now no more accepted especially in a low risk group.

Because the incidence in Saudi Arabia is very low it is very difficult for individuals

to establish and maintain expertise in the management of these rare diseases.

Therefore, the management should be centralized and establishment of Saudi GTD

registry need to in used.

Criteria Used to Diagnosed GTN

Objective

• Plateauing of HCG level over at least three

weeks.

• A rise of HCG of 10% or greater for three

values or more over at least two weeks.

• Persistent of HCG beyond six months after

molar evacuation

• Ensure adequate follow up of patient with

moler pregnancy and early detection of

GTN.

• If GTN diagnosed treatment is coordinated

with an expert in the field and help arrange

in the referal.

• After a successful registration Saudi GTD

will be following the patient you registered.

• Saudi GTD will be alerting physicians when

a laboratory exam is due or pending.

• Saudi GTD will follow the patient’s

EVALUATION.

• Hit Order New HCG link to bring the order HCG screen.

First alert, if the HCG not done in the right time.

Second alert, if the HCG is not going down.

Third alert, Does patient so far had presistent GTD is YES.

• WHO Scoring is auto calculate once you selected

each factor.

• The four first will be mandatory.

• Click on save button to proceed.

• You can close the screen without saving by clicking

on Close without save.
