Chaperone Agreements
The American Cancer Society Relay For Life requires one chaperone older than 25 for every five youth team members. For this event, a youth is considered anyone younger than 18.
In addition to completing and turning in this agreement, the youth participant must also complete an event registration form (either online or a scannable form).
Curbing CancerTiffani Noah
• Curbing Cancer is a unique fundraiser our event holds each year on Thursday of the Power of Purple week where teams that are interested take to the streets to “panhandle” for 2 hours and raise money and awareness.
• When: Thursday June 21st 4 to 6pm• Sign up in the back of the room• Last year we raised $17,314.12 in just 2 hours!!

– Your information, a small amount of blood, and a commitment to answer questions when mailed to your home.
• When can I participate?– Willamette High School track, Friday July 27th 5-9pm
• Who can participate? Anyone 30-65 yrs old, never been diagnosed with cancer and willing to commit to a long term study.
