MESSAGE FROM THE PRESIDENTIt is my utmost pleasure to welcome all fellows, members and distinguished delegates totheAnnualScientificCongressoftheCollegeofSurgeons,AcademyofMedicineofMalaysia, to be held in Pullman Hotel, Kuching, Sarawak on 19th to 21st May 2017.
IwouldliketofirstlytakethisopportunitytocongratulatetheOrganisingCommitteefortheir relentless efforte in ensuring that this Congress is a success. Being part of the global surgical fraternity, we are very much aware of the importance of breaking new
frontiers and hence shaping surgeons of the new era which is the theme for this year’s event.
Withthegrowinginterestandlocalparticipationinresearch,globalizationhasindeedmarkedasignificantimpact on our Society. This trans-national assembly therefore intends to not only discuss the wide-ranging research undertakings carried out by the clinical experts and academic institutions, but aims to share thevastpoolofknowledge,resourcesandexpertiseofboththescientificandclinicalcommunity,whichin turn generates substantial contribution for overall national progress. The sessions at this Congress, therefore, represent a unique platform in encouraging discourse among local and international scholars and professionals, besides sharing their invaluable insight on updates and the challenging day-to-day tasks.
We hope our comprehensive programme would inspire our delegates to think outside the box and re-examine the boundaries in refining the quality of care. Iwould like to thank the keynote speakers,presenters, authors for your proceedings and the delegates for your presence and contribution which an essentialinensuringaholisticandfulfillingexperience.

MESSAGE FROM THE ORGANISING CHAIRMANI would like to invite you to attend the 2017 College of Surgeons, Academy of Medicine of Malaysia (CSAMM) Annual Scientific Congress and Annual General Meeting inKuching, the capital and the largest city in Sarawak, Malaysia. It is known as “Cat City” and also the “City of Unity” where people of different racial origins and religious beliefs live and work in total harmony and unity. Kuching is now one of Asia’s most alluring cities with its unique brands of art, culture, fabulous food and hospitality.
This year the CSAMM congress and meeting will be held at the Pullman Hotel, Kuching, Sarawak from 19th to 21st May 2017. The theme is Dum Spiro Spero (While I breathe, I hope). It was the motto for the KingdomofSarawakundertheWhiteRajahs.Themottoreflectsthechallengesandendeavorsfacedbysurgeonsintheirday-to-daywork.Asurgeon’slifeisunique.Hecompletesfivetosixyearsofmedicalschool education, works for at least three years before spending another four years of training to acquire basic competency in surgical techniques and knowledge required of a surgeon. In between, he has to balance his work life with personal life, family and social responsibilities. He must be prepared to embrace changes and adapt to the technological advances to improve quality and safety of surgery. He must have humility to recognize his own shortcomings and has preseverance and discipline to succeed in a demanding work environment. Finally, he imparts his knowledge and his expertise to his trainees as long as he breathes, hopes and lives.
This year’s programme will continue a long-standing tradition of strong didactic sessions by world renowned experts combined with local faculties with the topics selected geared for the greater audience. The focus of topics chosen aims at thorough coverage of traditional topics combined with presentations of “cutting edge”material,designedtomeettheneedofpracticingsurgeons,surgeons-in-training,medicalofficers,nurses, other allied health professionals and medical students. The highlights of the Congress include the A M Ismail Oration, seven plenary sessions, twelve symposia, six meet the expert sessions, Ethicon Prize and How I Do It sessions. In addition, there will be concurrent sessions involving the breast chapter, national surgical nursing and medical student separately over three days.
On behalf of the Organising Committee, I would like to express our heartfelt thanks to the industrial partners, the Sarawak Convention Bureau, and the College secretariat for their unwavering efforts and support.

44TH A M ISMAIL ORATIONCitation on Professor Dr David Galloway

Professor Dr David Galloway graduated in Medicine from the University of Glasgow in 1977. His early postgraduate training was focused in the West of Scotland and after becoming a lecturer in surgery in his alma mater, his specialty postgraduate training involved posts in London and in New York City where he completed a fellowship in surgical oncology at the Memorial Sloan Kettering Cancer Center.
On his return to Glasgow in 1988, he was appointed as a consultant surgeon with a particular interest in colorectal surgery. His clinical interests developed to build a tertiary
referral practice in gastrointestinal surgery, advanced laparoscopic techniques and more recently bariatric and metabolic surgery.
He relished the opportunities for clinical research that came with a teaching hospital appointment, has just over a hundred publications and has supervised numerous projects for others working towards a higher degree. He was appointed Honorary Professor in the College of Medical, Veterinary and Life Sciences, University of Glasgow in 2015.
In the Royal College of Physicians and Surgeons of Glasgow, he has held various roles notably as Honorary Treasurer (1999 - 2006), Vice President (Surgical) (2007 - 2012), Chair of the Joint Committee on Intercollegiate Examinations (2004 - 2008), and Chair of the Fellowship Committee (2012 - 2015). He was elected President in December 2014 and after a year in the role of President-Elect, he was invested as President in December 2015.
One of the highlights of his professional life has been a long association with colleagues in Malaysia. He firstattendedtheCollegeofSurgeonsAnnualScientificMeetinginCameronHighlandsin2002andhasmade visits to Malaysia virtually every year since then, examining in the Master of Surgery examination several times, giving lectures and taking part in many of our local meetings. He has also helped in the development of the Master of Surgery curriculum in the Department of Surgery, University of Malaya, where he was appointed Visiting Professor in 2013.
In his personal life, Professor Galloway is happily married to his wife, Christine, since 1979, and has two lovely daughters, one of whom is a doctor. He has two young grandchildren. Flying is one of his many hobbies and he holds a private pilot’s license. He is also an excellent golfer, and among his other hobbies are reading, cycling, tennis, computing, web design and digital video recording.
The College of Surgeons, Academy of Medicine of Malaysia recognizes Professor David Galloway as a doyen of surgery and a true friend of Malaysian surgeons.

M BALASEGARAM TRAINER AWARDIntroductionProfessor M Balasegaram was the chief of surgery at Hospital Kuala Lumpur in the 1960 to 1970s and had been instrumental in the education and training of the later generation surgeons throughout Malaysia. He was a surgeon par-excellence, winning many accolades from all over the world; especially for his work on liver resection. The later generation surgeons remembered his humility, dedication and passion for surgical training. With his passing in 2013, a memorial lecture to his honour was held in Hospital UKM in 2014. The CSAMM Council unanimously agreed to name a surgical education and training prize in his memory, to honour teachers and trainers in surgery in Malaysia. This award, in the form of a medallion and acertificate,maybepresentedeachyearattheAnnualScientificMeetingoftheCollege.
ObjectiveThe objective of this award is to honour any person/persons who has/have contributed to the training and educationofdoctorsinthefieldofsurgeryinMalaysia.
Eligibility1. Any person/persons, past or present, who has/have contributed to surgical education and training in Malaysia.2. Theperson(s)couldbe involved inanysurgicalfield, including fromallsurgicalspecialitiesorsub- specialities,orresearch-basedbutmusthavecontributedsignificantlyintheeducationandtrainingof surgeons.3. Not more than two may be selected per year.
Selection criteria1. The CSAMM shall circulate call letters to surgical societies and request for nominations for the award, at leastthreemonthsbeforetheAnnualScientificMeeting.Thenominationformshallcomprise: a. Name of nominee, proposer and a seconder b. A brief CV of the nominee c. Reasons for the award
2.SelectionCommittee:Thiswillcomprisefivemembers-threefromtheCSAMMExcoandtwoothers nominatedbytheCSAMMCouncil.Thenon-Excoofficialsshallbeseniormembers(pastpresidentsor council members) of the College of Surgeons.
3. Attributes used in selection: The selection is based on the nominee’s contributions - past and/or presentintraining,leadershipinthefield,resourcefulness,andcontributiontodevelopmentofthefield. The selection committee has the right to decide upon the weightage of the criteria. Their decision would befinal.
AwardThe awardee/representative would be called upon during the award ceremony and a short citation, as prepared by the selection committee, shall be read out.
A medallion that carries the title M Balasegaram Trainer Award and the CSAMM logo with the year of award and the name of the awardee would be presented. The cost of each medallion should not exceed RM1,000(RMOnethousandonly)andthevaluemaybereviewedbytheCSAMMExcoeveryfiveyears.Acertificateofexcellenceintrainingwouldalsobeissued.
SummaryProfessor Balasegaram’s award is an award to honour educators and trainers in surgery. The CSAMM recognizes that there would be many specialities competing for the award. This guideline shall be reviewed everyfiveyearsbytheCSAMMCouncilandamendmentsmadewhennecessary.

RECIPIENT OF INAUGURAL M BALASEGARAM TRAINER AWARD 2017
Professor Dato’ Dr P KandasamiDr Kandasami is Professor of Surgery at the International Medical University, Seremban, Negeri Sembilan. He received his medical degree from University of Bombay in 1975 and the Fellowship from the Royal College of Surgeons in Ireland, Edinburgh and Glasgow in 1980. After his surgical training in the United Kingdom, Dr Kandasami served as Consultant General Surgeon in several Ministry of Health hospitals before joining the International Medical University in 2001. He was appointed Dean of the Clinical School for the period 2004to2009.DrKandasami’smaininterestisinthefieldofuppergastrointestinalsurgery.Dr Kandasami has contributed substantially to the development of surgical services in
the country. He was Deputy Chairman of the National Perioperative Mortality-Review Committee and made significant contribution to thedevelopment of the ‘ComputerizedOTDocumentationSystem’ (COTDS).Heplayed a lead role in the establishment of the national credentialing system and the National Specialist Register.
Dr Kandasami is currently the Master of the Academy of Medicine of Malaysia. He is a member of the National Credentialing Committee, Chaiman of the General Surgery Credentialing Committee, Member of Upper GI Surgery Credentialing Committee. He has been a past President of the College of Surgeons, and the Malaysian Society of Gastroenterology and Hepatology.
An enthusiastic teacher and an experienced examiner, Dr Kandasami is much sought after among higher surgical trainees sitting for their examinations. He has been actively involved in undergraduate and postgraduate teaching since the beginning of his career.
In the early years, Dr Kandasami was actively involved in training surgeons in the then “parallel pathway” to complementthetrainingintheMastersProgramme.Hewasappointed‘Tutor’oftheRoyalCollegeofSurgeonsofEdinburgh and conducted regular postgraduate training programmes, supervised and trained several surgeons. Hehelpedpreparesurgical traineesfor the ‘oldFRCS’byorganizing the ‘TheNationalPreparatoryCoursefor the Final FRCS Examination’ annually. Dr Kandasami was also the local convenor for the Intercollegiate Basic Surgical Skills Course of the Royal Colleges of Surgeons UK and he organized the Intercollegiate Basic Surgical Skills course of the Royal Colleges regularly. This course was mandatory for the candidates preparing for the MRCS examination. He was Examiner at the FRCS and MRCS Examinations.
Dr Kandasami served as the Chairman of the Higher Surgical Training Committee of the College of Surgeons since its formation in 1997 to 2007. The Higher Surgical Training Committee was formed to oversee and co-ordinate the surgical programmes organized by the College. The Committee through an understanding between theCollegeandtheMinistryofHealthMalaysiaestablishedthefirst‘SurgicalSkillsTrainingLaboratory’inthecountry at the Institute for Medical Research, Kuala Lumpur. The Committee developed various surgical skills programme including basic surgical skills, courses on bowel anastomosis, basic and advanced laparoscopic courses. Over the years, this Committee successfully trained trainee surgeons and practicing surgeons in various surgical skills.
Affirmative,confidentanddisciplinedaresomeofDrKandasami’squalities;thesequalitieshavebeenpassedon to surgeons who trained under him.
Dr Kandasami has been an Examiner for the local Masters of Surgery Programme for nearly two decades. He has supervised and trained candidates in Masters in General Surgery for the UKM, UM and USM. Dr Kandasami has trained several surgeons during his career, many of whom are enjoying a successful career. He is often referred to as Master Surgeon, but this master had a master, none other than the late Professor M Balasegaram.
ProfessorDato’DrPKandasamihascontributedimmenselyinthefieldofsurgery,notonlyinMalaysia,butinthe Asian and Commonwealth countries. He is not only a highly skilled surgeon who served his time in public service until retirement; he is also an exemplary leader, an inspirational teacher and a dedicated trainer of future generations of surgeons.

This workshop is targeted to surgical trainees and medical students and provides a platform with hands-on opportunity where sound understanding and in-depth knowledge on device-tissue interaction and Science of Tissue Management (SoTM) can be obtained. It would involve sharing the latest information and innovations in Stapling, Energy Devices, Abdominal Wound Closure and Intra-Operative Bleeding Management and will be divided into a morning lecture session and an afternoon hands-on lab session.

PROGRAMME
INTRODUCTIONIn this day and age, advances in technology has revolutionised healthcare and changed the way we perform surgery. Laparoscopic surgery has become the gold standard for many operative procedures for the last 2 decades. Laparoscopic suturing is technically challenging but an important skill to acquire for surgical trainees as well as established surgeons. It is an important adjunct to laparoscopic surgery as it helps the performing surgeon to navigate through many tight situations.
This one day intensive workshop is directed to help participants to progress from basic handling skills to advancedlaparoscopicsuturing.Itisdeliveredbyestablishedlaparoscopicsurgeonintheirownfieldwithlectures and hands on laparoscopic suturing (both dry and wet models) using endo-trainer.
OBJECTIVES1. Technique of laparoscopic intra-corporeal and extra-corporeal knotting.2. Technique of laparoscopic tissue handling and suturing (interrupted, continuos).3. To know the requirement of laparoscopic suturing.

This two-day course is designed to advance the practical, theoretical and personal skills necessary for the care of critically ill surgical patients. The course is suitable for candidates who are now undergoing their surgical training (Master of Surgery) or just obtained their Master of Surgery. If you are in the earlier part of surgical training you must submit a CV with your application.
CONTENTThis course is designed to advance the practical, theoretical and personal skills necessary for the care of critically ill surgical patients. It is aimed at surgeons and those dealing with surgical patients who are in specialist training.
LEVELMaster of Surgery Students and SpecialistsIt is the responsibility of the course delegates to ensure that they fall within the relevant criteria for the course as detailed above.
