Google Forms: Get speedier survey results
You spent a week perfecting the new product training. The first session was a success...or was it? Whether you want to gauge satisfaction on a training session, a team building activity, or some other event or effort, you can speed up your survey results by sending people a Google Form.
Setting it Up
Creating the form
To collect feedback, create a Google form:
1. Access your Google Drive at drive.google.com .
2. Click New > More > Google Forms .
3. Give your form a title by typing it in the Title field.
Give your form more pizzazz by selecting a theme. Themes apply a preset style to your form.

4. Add a description to your form by typing it in the Form Description field. This field usually explains the purpose of the form and what you'll do with the information. Example: This survey will be used to improve future product training.
5. Update the form settings based on who needs to respond to the form: ● Deselect Require <yourdomain> login to view this form if you want to be able to send the form to
people outside your organization.
● Select Automatically collect respondent's <your domain> username if you don't want the survey to be anonymous.
Adding questions to get feedback
1. Update the Question title with the text for your questions.
2. Add Help text to provide more information about a question.
3. Select the Item type for the question and add your response options.
4. Select the Required question checkbox to make all respondents complete that item.
5. Click Add item and repeat steps 1-4 until you've added all your feedback questions.
Use the Copy button to create similar questions faster.

Using item types
As you add items to your form, you'll need to select an item type . Here are some example of how you might use some of the item types in your feedback form:

Grid ● Use for multiple items with responses that use the same scale
● Use rows for questions or statements
● Use columns for the scale (Extremely satisfied > Extremely dissatisfied)
Rate your satisfaction with the following: ● Instructor ● Facilities
Previewing and testing your form
1. Click View live form to get a sneak peek of your feedback form.
2. Before sending the form to everyone, click Send form to send it to someone to test it for you.

Launching your feedback form
Sending the form
You can ask others to complete your feedback form without going to your inbox! While editing the form:
1. Click the Send form button on the top-right of the form.
2. Enter the names or distribution list of people you want to complete the form.
3. Add a subject and a note.
4. Click Send.
Choosing a response destination
After you send your form, you need to specify a spreadsheet for viewing people’s responses. You’ll be prompted with two choices:
● New spreadsheet: Select this option to view responses in a new Google Sheet. This is the default option.
● New sheet in an existing spreadsheet: Select this option to view responses in an existing spreadsheet already in your Google Drive.
● Keep responses only in Forms : Click this button if you don’t want your responses to go to a spreadsheet. Access responses by selecting File > Download as > Comma Separated Values .

Reviewing responses in a spreadsheet
You can view the responses in real-time as they are submitted in a Google Sheet.
1. Find the sheet with your responses in your Google Drive by visiting drive.google.com .
2. Each response creates a new row in your spreadsheet with a timestamp indicating when the response was submitted.
3. Get graphs and charts of the responses on demand by selecting Form > Show summary of responses.
Best Practices
You'll want to customize your feedback form based on your audience and purpose, but here are some general best practices for feedback forms:
● Don't overuse scales. They provide useful data, but items with scales take more mental energy to complete. Limit feedback fatigue by limiting your use of scales.
● Tell respondents if your form is anonymous. If responses are anonymous, add that to the form description. You can always provide an optional field at the end of the feedback form where people can give you their contact information if they want to identify themselves.
● Create an action plan related to your feedback. Once you've reviewed your feedback, identify actions you can take in response.
● Make it easy for people to respond . Embed your survey in a Google Site they visit often. You can also share the form via email and people can respond without leaving their inbox.
