REGISTRATION OF BIRTH
The birth of a child shall be registered in the Office of the Local
Civil Registry of the City of Valenzuela where the birth occurred
within thirty (30) days from the time of birth. • When the birth occurred in a hospital or a clinic or in a similar
institution, the administrator thereof shall be responsible for the
registration of birth. However, it shall be the attendant at birth
who shall certify the facts of birth • When the birth did not occur in a hospital, clinic or similar
institution, the physician, nurse, midwife or “hilot” or anybody who
attended the delivery shall register the birth

IN THE CIVIL REGISTER
In compliance with the Republic Act 9048, the Local Civil Registry of
the City Government of Valenzuela is authorized to correct a clerical
or typographical error in a civil registry entry and / or change of first
name or nickname in the civil register without need of a judicial order.
Further, with the passage of Republic Act 10172 the Local Civil
Registry is now authorized to correct gender and date of birth in the
civil register without need of judicial order.

parents (if minor) together with 1 government issued ID of the document owner (parents if minor) as well as that of the representative (if the document owner or his parents as the case maybe is outside the country, the authorization should be subscribed before the consular office in that country)
• Other documents as the civil registrar may require on a case to case basis to comply with the mandate of the Data Privacy Act of 2012.
If document owner or data subject is deceased:
• Accomplished application/request form • Valid government issued ID of document owner’s lawful heirs or
assigns • Notarized Affidavit of being lawful heir/nearest kin or assign
executed by the latter. • Written Authorization from the document owner’s lawful
heir/nearest kin or assign together with 1 government issued ID of the foregoing as well as that of the representative (if the lawful heir/nearest kin or assign is outside the country, the authorization should be subscribed before the consular office in that country)
• Other documents as the civil registrar may require on a case to case basis to comply with the mandate of the Data Privacy Act of 2012.
