INTRODUCTION
Primary and Secondary Healthcare Department (P&SHD) delivers quality healthcare
services to the community through an efficient and
effective service delivery system that is accessible, equitable, culturally acceptable,
affordable and sustainable. P&SHD aims to improve
the health and quality of life of all, particularly women and children, through access to
essential health services.
The Primary and Secondary Healthcare Department strives to reform and strengthen the
critical aspects of the health systems and enable it to:
Provide and deliver a basic package of quality essential health care services
Develop and manage competent and committed health care providers
Generate reliable health information to manage and evaluate health services
Adopt appropriate health technology to deliver quality services
Finance the costs of providing basic health care to all
Reform the health administration to make it accountable to the public
Primary and Secondary Healthcare Department (P&SHD) is the key department entrusted
by the people of Punjab with the fundamental responsibility for the health of communities
and the entire population. P&SHD delivers primitive, preventive as well as curative health
care services of Primary Health Care level to Tertiary Health Care level.
Free of cost consultation, diagnostic facilities and medicines are provided to the patients
particularly focusing on the poor and marginalized segments of the society. Health
Department also provides different programs like
Hepatitis Control Program
Aids Control Program
Expanded Program for Immunization
TB Control Program
IRMNCH
Malaria Control Program
Infection Control Program
Besides measures to prevent, treat and control other communicable diseases and Epidemics
/ Disasters, these vaccines are provided through Expanded Program on Immunization (EPI)
for children under 2 years and pregnant ladies. Health Department is producing its own
trained and qualified Human Resource keeping the HR development needs and
requirements of Health Care Infrastructure in mind.
1. EOI ISSUING AUTHORITY
This Expression of Interest is issued by Primary & Secondary Healthcare
Department, Government of the Punjab to invite potential firms/companies having
expertise of Installation and Operations of Medicine Dispensation Machines in all
health establishments of Primary & Secondary Healthcare Department across the
Punjab. The department reserves the right to reject any or all the offer without

3. AVAILABILITY OF THE EOI DOCUMENTS
EOI documents can be downloaded from the Department website
(http://www.pshealth.punjab.gov.pk/). It may be collected in free of cost from the
office of Procurement Specialist, Primary & Secondary Healthcare Department, 31-
E/1, Shahra-e-Imam Hussain, Gulberg-III. The participants are expected to
examine all instructions, forms, terms, project requirements and other details in the
EOI documents. Failure to furnish complete information as mentioned in the EOI
documents or submission of a proposal not substantially responsive to the EOI
documents in every respect will be at the bidder's risk and may result in rejection of
the proposal
4. VENUE & DEADLINE FOR SUBMISSION OF PROPOSALS
EOI Letters, in its complete form in all respects as specified in the EOI, must be
submitted to, Primary & Secondary Healthcare Department, Government of the
Punjab, 31-E/1, Shahra-e-Imam Hussain, Gulberg-III, Lahore.
P&SH Department may, in exceptional circumstances and at its discretion, extend
the deadline for submission of proposals by issuing an addendum to be made

available on the Department’s website, in which case all rights and obligations of
P&SHD
Scope of Work (Services to be carried out by Pre-Qualified
Firms/Companies/JVs)
The firm shall install, operate and maintain the Medicine Dispensation
machines in all/selected health establishments of P&SHD across the
Punjab.
The Service Provider will be responsible for all the human resource for
these operations.
The Procuring Agency (P&SHD) has the right to monitor by itself or by
Third Party Monitoring/TPV of these project/operations.
Project Period
Firm will be pre-qualified for 2 years and may further be extended for 1
year upon need & performance of the firm.
Payment Mechanism
Payment shall be made within 30 days from the date of issuance of invoice
by service provider
Bulk Coverage
This pre-qualification is attempting to outsource Operations of Medicine
Dispensation Machine in bulk of the DHQs/THQs/RHCs/BHUs in the
Punjab Province.
Eligibility Criteria
Following are the details on the basis of which Firm will be shortlisted:
Pre-Requisites
Registration with SECP, Registrar of Firms, FBR etc. (as required under law).
National Tax Number (NTN)
Sales Tax Registration Number (STRN)
Punjab Sales Tax Registration Number (PSTRN) (if applicable)
Undertaking by the Firm/Company on a legal format, that it has not been blacklisted by
any Government Agency/Authority nor is under any litigation.
Firm Experience
Firms should provide information regarding;
General experience by providing the list of total number of projects/assignments
performed so far as per Annex - A

Relevant experience in the subject field by submitting the detail of (5 Nos.)
relevant projects undertaken in the past (10 years) as per
Annex -B.
Detailed description of the Firm Profile by providing its organizational structure,
list of permanent staff and quality management system.
Human Resource Capacity
List of key personnel proposed having expertise in the area of Installation & Operations
of Medicine Dispensation Machines (Vending Machines) Annex -C.
CVs of key personnel proposed as per Annex - D.

Firm must have annual average turnover of Rs. 10 Million over the previous 3 financial
years. Detail of financial capabilities to be provided as per Annex- E.
*The aforementioned documents must be submitted by following a;
Covering Letter which should contain the detail of power of attorney for signing the PQD.
Name, address and firm registration details (including the year of incorporation and the
number of years of relevant experience). Please also attach copy of certificate of
registration/ incorporation /commencement.
Copy of registration certificate being issued from the professional body.
Copy of National Tax Number, Sales Tax Registration Number and Punjab Sales Tax
Registration Number (if applicable)

Application status of the Firm/Company, i.e., whether to apply in its own name or in the
form of a consortium or of a joint venture (in case of a consortium or a joint venture, copies
of the agreement indicating the nature of the association or entity to be furnished)
Projects/assignments to be submitted by the Firm/Company must be supported with the
copies of completion certificates issued by the procuring/executing agencies.
CV of the proposed professional staff/key personnel must be signed by him/her and
counter signed by the authorized signatory as proposed in the covering letter.
Audited Annual Statements for the previous 3 financial years.
Affidavit on the stamp paper that the Firm has neither been Blacklisted from any
Government (Federal, Provincial, District and / or any other Government owned Authority)
nor is under any Litigation.
All documents should be signed by the authorized signatory as proposed in the covering
letter.
All documents should be in English.
* Any misinformation, false and forged statement will lead to
disqualification from being shortlisted/pre-qualified and any other action as per
applicable laws.

proportionate basis keeping in view the profiles of the bidding firms.
