    RE: YOUR NEW MEXICO DRIVERS LICENSE OR IDENTIFICATION CARD #_____________

    You must act within thirty (30) days from the date of this letter to avoid cancellation of your New

    Mexico drivers license or identification card.

    Dear Mr./Ms. Public:

    The public safety of New Mexico requires that State authorities identify and cancel the drivers licenses

    (licenses) and identification cards of Foreign Nationals who presented fraudulent documents or havemoved out of state since obtaining a New Mexico license or identification card. This compliance effort

    requires that you provide proof of your identity and residency to ensure that we have correct and current

    information. This process is not optional and you must comply with the detailed instructions below. This

    is notice that the Motor Vehicle Department (MVD) will take steps to cancel your license or identification

    card if you fail to comply with the appointment process within thirty (30) days of the date listed on this

    notification. The law states that the cancellation of a license is without prejudice and application for a

    new license may be made at any time after cancellation. NMSA 1978, 66-1-4.3.

    As part of this compliance process, you must provide sufficient proof of your identity and residency. You

    must provide the following documentation: one (1) proof of identification number, one (1) proof of

    identity, and two (2) proofs of residency showing a valid New Mexico address. Enclosed you will find a

    list of acceptable identification and residency documents. Any documentation not listed will not beaccepted.

    We have established a new office at the Bank of the West Building at the corner of Central Avenue and

    San Mateo in Albuquerque. The new office will handle the compliance review of all Foreign National

    driver licenses and identification card holders. By using a single, centrally located facility for this

    purpose, with MVD employees who are specially trained in the Foreign National compliance process, we

    will be able to efficiently confirm your identity and residency status. Our intention is to make your visit as

    quick and easy as possible.

    You can schedule your appointment by going on-line at http://www.mvd.newmexico.gov or by calling

    our Foreign National Call Center at 1-855-784-8407. Please note that only one appointment will be

    allowed per individual. Your appointment time is for you alone and cannot be used by anyone else. Pleaseappear for your appointment 10-15 minutes early so that we can serve you promptly at the scheduled

    time. Should you need to cancel your appointment, you must call the Foreign National Call Center, give

    them your existing appointment date and time, and ask to be rescheduled. Please note that an

    appointment is required and the new office will not service walk-ins without a scheduled appointment.

    Required Documents

    Two documents are required to prove your identification number and identity for your drivers license or identificationcard. One document must include:

    (1) a Matricula Consular card issued after February 1, 2005 by the Mexican Consulate in Albuquerque or El Paso;

    (2) a valid passport issued by your country of citizenship; or

    (3) an individual tax identification number (ITIN) card or the applicants letter from the IRS issuing the ITIN, if theITIN card or letter is accompanied by a Matricula Consular card issued after February 1, 2005 by the MexicanConsulate in Albuquerque or El Paso or a valid passport issued by your country of citizenship.

    And one additional document from the following list:

    (4) a Matricula Consular card issued after February 1, 2005 by the Mexican Consulate in Albuquerque or El Paso;

    (5) a valid passport issued by your country of citizenship;

    (6) an original or certified copy of a foreign birth certificate with a notarized English translation;

    (7) a valid permanent resident card issued by the United States government;

    (8) a valid I-551 resident alien card issued since 1997;

    (9) a court order for name change, gender change, adoption or divorce, as long as it includes the legal name, date

    of birth and court seal;(10) a marriage certificate issued by a state or a territory of the United States; or

    (11) a photo identification card issued by the United States military, United States coast guard or New Mexico

    national guard;

    (12) an identification document issued by the United States veterans administration, so long as it is accompanied by aUnited States veterans administration medical center ID card; or

    (13) a valid United States active duty/retiree/reservist military identification card (DOD ID DD-2).

    You must also provide current proof of New Mexico residency. The address on your residency documents must havethe same residential address. Each proof of residency document must show your New Mexico residence address. AP.O. Box address is not acceptable. All documents must be originals or certified copies.

    Applicants 18 years of age or oldermust provide two of the following proof of residency documents:

    (1) a real property rental agreement or purchase agreement;(2) a utility bill, such as water, gas, electric, waste, telephone, cable or satellite bill, but not a bill for a cell phone;

    (3) an insurance bill;

    (4) a bank statement;

    (5) an employment pay stub that contains your name and address;

    (6) a local property tax statement or mortgage documents;

    (7) proof of a minor child enrolled in a New Mexico public, private, or tribal school;

    (8) a current, valid motor vehicle registration;

    (9) original documents from a New Mexico community organization attesting to the fact that you are a New Mexicoresident;

    (10) original documents from a city, county, state, tribal or federal government organization attesting to the fact that

    you are a New Mexico resident;

    (11) a New Mexico medical assistance card; or(12) a New Mexico public assistance card.

    Applicants under 18 years of age must provide one of the following proof of residency documents:

    (1) proof that the applicant is enrolled in a New Mexico public, private, or tribal school;

    (2) a bank statement;

    (3) an Affidavit of New Mexico Residency of Applicant under Age 18 (MVD-10021);

    (4) original documents from a New Mexico community organization, or from a city, county, state, tribal or federalgovernment organization, attesting to the fact that the applicant is a NM resident;

    (5) documents from membership in a New Mexico religious organization; or

    (6) documents from membership in a New Mexico sports organization.
