Gurpreet’s Journey

Applying for Criminal Injuries Compensation Benefits

Webinar Format

This is a self-guided webinar. For the most effective learning experience, please follow these tips as you proceed: Please read the Case Study “Gurpreet’s Journey” as well as the Criminal Injuries

Compensation Board Advocacy Handbook before listening to this webinar. A sample Application form based on Gurpreet’s case is also attached.

We will be discussing how Gurpreet’s case progresses through the CICB system as a way to illustrate the process.

Applying for Benefits i) Obtaining and Completing the Application

Gurpreet can find the application forms at the CICB website or by requesting the form the Board by telephone or email. It can be completed online or in handwritten form and returned to the Board by

mail, email or fax. Since she does not require an extension to apply she does not need to complete

the Extension Request Form. A copy of Gurpreet’s application is attached for reference purposes.

i) Obtaining and Completing the Application, cont.

Once the application is filed, her case will be assigned a CICB number and if she

needs to send any information to the Board, it should always include this number. A Victim Support Line (1-888-579-2888) is available to assist with the completion

of the application. As well, there are detailed guides on the CICB website that will help Gurpreet

(and any community advocate assisting her) understand how to complete the application.

ii) Assignment of a Designated Staff Member

Once filed, Gurpreet will receive a letter providing her with her CICB case number and the name of her designated staff person assigned to her file.

If Gurpreet has questions or needs to send more information to the Board, that designated staff person is her ‘point person” going forward.

The designated staff person reviews the application and determines what additional information is required.

If medical/therapy reports are required, the designated staff person prepares standard forms to be completed by those treatment providers named in the application.

ii) Assignment of a Designated Staff Member, cont.

Generally, these forms are sent to Gurpreet as the applicant and she is responsible for distributing them to the various doctors, therapists and counsellors.

If the forms are completed and sent back directly to the Board (not to Gurpreet), the providers are paid $100/report for their time.

The designated staff person will also obtain the reports from the Sarnia Police Service and the Crown Attorney’s office regarding charges and the trial of Pedro.

Gurpreet will need to gather and send to the Board, statements from the various parties listed above, the tuition information, and copies of her transcripts.

iii) Next Steps

Once all the information is gathered, the designated staff person will advise the

Board and a decision will be made as to whether it will be an oral (in person) or a written (documentary) hearing. Gurpreet will receive a letter from the Board advising they are ready to schedule

a hearing. She should immediately request a complete copy of her CICB file. There is no charge for the Board to prepare and send the file to Gurpreet. Please

see Appendix C of the Handbook for a sample request letter. It is important to have the full file for use in preparing for the hearing.
