QUE ES TERAPIA TEMPRANA DIRIGIDA POR OBJETIVOS?

• Involucra ajustes en:

PRECARGA CONTRACTILIDAD POSTCARGA

Page 3: Haemodynamic monitoring in the critical care pregnant patient

PUNTOS CLAVE DE LA EGDT

Page 4: Haemodynamic monitoring in the critical care pregnant patient

PERO…Y COMO SE TRADUCE ESO EN LA PRACTICA?

• PRECARGA (VENOSA) = Volumen= Presión Venosa Central o Presión en Cuña de la Arteria Pulmonar.• POSTCARGA (ARTERIAL)

= Resistencia Vascular Sistémica (RVS).• CONTRACTILIDAD

(CARDIACA)= Gasto o Indice Cardiaco (IC)

MONITOREO INVASIVO VS NO INVASIVO?

Todos las pacientes en shock:

1. Presión arterial (de forma invasiva o no invasiva)

2. ECG.3. Pulsoximetría.

4. Control de diuresis.5. Presión venosa central. 6. Saturación venosa

central de O2.

EN QUE ESCENARIOS MONITOREAR?

• PREECLAMPSIA• SEPSIS• PERIOPERATORIO ALTO RIESGO• CARDIACA

Cuándo monitorizar el gasto cardíaco??

Page 9: Haemodynamic monitoring in the critical care pregnant patient

EN QUE ESCENARIOS MONITOREAR GASTO CARDIACO?

• Shock refractario.• Reanimación en pacientes con fallo cardíaco.• Pacientes posquirúrgicas con alto riesgo.• Shock cardiogénico.

Page 10: Haemodynamic monitoring in the critical care pregnant patient

• CLINICA.1. SENSORIO2. LLENADO CAPILAR3. PULSO4. PRESION ARTERIAL

MEDIA O PRESION ARTERIAL SISTOLICA.

5. GASTO URINARIO.

• PARACLINICA.• SatVo2

• LACTATO SERICO (≥4mmol/lts).• DEFICIT DE BASE.• DISFUNCION DE

ORGANOS:1. RENAL.2. HEPATICO.

COMO EVALUAMOS LA HIPOPERFUSION TISULAR?

Page 11: Haemodynamic monitoring in the critical care pregnant patient

COMO EVALUAMOS LA HIPOPERFUSION TISULAR?

• RECUERDEN: la medición del ácido láctico o el DEFICIT DE BASES nos permiten controlar la evolución del estado de shock

CLEARANCE DE LACTATO

Page 15: Haemodynamic monitoring in the critical care pregnant patient

Lactate clearance of 10% or more predicts survival from septic shock. The lactate clearance was defined by :[(lactate initial − lactate delayed)/ lactate initial] x 100%, 2 normal lactate levels (18 mg/dL [2 mmol/L]) at least 2 hours apart as evidence of ongoing adequate tissue oxygenation

Page 16: Haemodynamic monitoring in the critical care pregnant patient

MONITOREO INVASIVO VS NO INVASIVO?

Invasivo No invasivo

Page 17: Haemodynamic monitoring in the critical care pregnant patient

• Métodos para determinar el gasto cardíaco:

• Invasivos.• Mínimamente invasivos.• No invasivos.

HEMODINAMIA EN EL ESCENARIO CRITICO

Page 18: Haemodynamic monitoring in the critical care pregnant patient

INVASIVOS

• ESTATICAS: 1. Presión Venosa Central (PVC).2. Presión de Oclusión Pulmonar (POP)

• DINAMICAS:…

Page 19: Haemodynamic monitoring in the critical care pregnant patient

HEMODINAMIA MATERNA EN EL ESCENARIO CRITICO• Flujo: CO (CI), PCCO (PCCI) y VS (VSI)• Precarga: GEDV (GEDI), ITBV (ITBI), VVS, VPP• Poscarga RVS (RVSI) y PAM• Contractibilidad: DPmax, GEF Y CIF (índice de función cardiaca

medido en min-1)• Función órgano: Pulmonar: EVLW (ELWI), PVPI y cardiaco CPO (CPI):

potencia cardiaca medido en w/m2• Oxigenación: ScVO2, DO2 (DO2I) y VO2 (VO2I)

Page 20: Haemodynamic monitoring in the critical care pregnant patient

INVASIVOS

• Método de termodilución transcardíaca (Swan-Ganz)

Page 21: Haemodynamic monitoring in the critical care pregnant patient

• Análisis del contorno del pulso sistólico. PICCO®• Análisis de la potencia de pulso. LiDCO®• No calibracion: Flotrac/Vigileo®

Page 28: Haemodynamic monitoring in the critical care pregnant patient
