2004-2005 System-wide Effects of Maximum Speed Limit Laws UAB Injury Control Research Center, CDC PI This study examined tested whether increased maximum speed limit laws on rural

interstate roads led to decreased driving fatalities on other roadways. 2004-2007 Impact of Changing Medicaid Policies on NH Quality NIA, R01 AG023622 Site PI The major goal of this project was to analyze the effect of state Medicaid policy on nursing

home quality of care using risk-adjusted resident-level quality information from the national Minimum Data Set.

2005-2006 Do Payment Incentives Influence Use of Home Health Care among the Elderly? Harvard Program on the Global Demography of Aging, NIA PI The Balanced Budget Act of 1997 changed Medicare home health eligibility and coverage

rules and reformed the payment methodology by implementing a prospective payment system. Using data from multiple waves of the National Home and Hospice Care Survey, this project investigated whether these changes had implications for functional status, utilization, mortality, and discharge to a hospice or hospital.

2005-2008 Using Incentives to Reduce Hospitalizations and Enhance Quality for Nursing Home

Residents in New York State Commonwealth Fund Site PI The major goals of this project were to analyze the costs, predictors and implications of

potentially preventable nursing home hospitalizations using New York State data. 2007-2009 Impact of Assisted Living Growth on the Market for Nursing Home Care Robert Wood Johnson Foundation PI The goals of this project were to document the growth of assisted living across markets in

the United States and analyze the relationship between this growth and nursing home utilization.

2008-2009 Is Nursing Home Quality Affected by Corporate Structure? Assistant Secretary for Planning and Evaluation Co-Investigator Through its Ownership Management and Tracking (OMT) system, the state of Texas has

collected detailed data around nursing home ownership and management structures that could offer useful examples for federal policymakers and other states to consider. Using the Texas OMT data, this project analyzed the corporate structures of nursing homes in Texas and how they have changed over time.

2006-2010 Disparities in Nursing Home Care by Race and Payer Status NIA, K01 AG24403 PI

The goal of this career development award was to provide further experience and training in nursing home quality and measurement. Using national administrative data over multiple years, the research project examined the underlying explanations for disparities in quality within and across nursing homes by payer status and race.

2007-2011 Nursing Home Ownership and Quality of Care: Current Issues and Future Challenges J.W. Kieckhefer Foundation Co-PI The nursing home industry has undergone important changes in terms of ownership

conversions and increased private equity investment. The purpose of this project is to examine the implications of these changes on different quality and cost outcomes.

2007-2012 UPR-CHA Research Center of Excellence: Making a Difference for Latino Health UPR/NCMHD, NIH P60MD002261 Co-Investigator The purpose of this Center is to reduce disparities in health and health services for Latinos,

through projects involving patient activation and provider education in asthma and mental health care. In addition, research will measure disparities in substance abuse and conduct a small project on mental health disparities in nursing homes.

2008-2011 Leveraging Medicare Payment and Reporting Policies to Improve Nursing Home Quality NIA, R01 AG030079 PI A series of policy changes have greatly expanded Medicare’s coverage of post-acute

nursing home care in recent years. Despite important differences in the needs of chronic and post-acute nursing home populations, quality of care within a facility has characteristics of a “public good” shared across all residents. The central implication of this observation is that Medicare resources—and policies directed at improving care for Medicare residents—may spillover to other residents. This project develops and applies a framework for evaluating the implications of Medicare’s increasing involvement in this market.

2008-2012 Evaluation of the Nursing Home Value Based Purchasing Demonstration Centers for Medicare & Medicaid Services, NVBPD-200 8-02 Site PI Dr. Grabowski is leading a Harvard team in the evaluation of the CMS Nursing Home

Value Based Purchasing Demonstration. Specifically, they will conduct analyses of administrative data to evaluate the Demonstration in terms of the validity of the experiment, the structure of the reward payments and the implications of the demonstration for quality and costs.

2007-2011 Post-Acute Costs and Outcomes after Medicare's Reimbursement Changes NIA, R01 AG031260 Co-Investigator Each year, more than ten million Medicare beneficiaries are discharged from acute care

hospitals into post-acute care settings including inpatient rehabilitation facilities, skilled nursing facilities, and homes with services from home health agencies. Whether beneficiaries receive post-acute care and the type and intensity of care they receive is

profoundly influenced by Medicare’s methods of payment. Consequently, payment changes will certainly affect the outcomes of post-acute care. How large those effects are and how they operate, however, are virtually unknown. The goal of this study is to answer these important questions.

2009-2011 Selection and the Quality Impact of Nursing Home Ownership NIA, R01 AG034179 PI Although a large literature examines for-profit and nonprofit nursing home quality, the vast

majority of the existing studies are limited in that they fail to address differential selection across for-profit and nonprofit nursing homes. By using "differential distance" to the nearest nonprofit nursing home relative to the nearest for-profit nursing home, we can mimic randomization of residents into more or less "exposure" to nonprofit homes when estimating the effects of ownership on quality of care.

2010-2011 Telemedicine and Nursing Home Hospitalizations Commonwealth Fund PI This study evaluates a randomized intervention of two-way video conferencing within a

Massachusetts nursing home chain on the hospitalization of nursing home residents. 2011 Home Health Study and Report Centers for Medicare & Medicaid Services Site PI This study consists of analyzing issues and concerns associated with the current the

Medicare Home Health Prospective Payment System. Activities include reviewing the current case mix algorithm, reviewing comments related to the current case mix algorithm, and proposing alternatives to the case mix algorithm.

2011-2013 Health Care Expenditures for Nursing Home Residents with Advanced Dementia NIA, R21 AG039658 Co-PI with Susan Mitchell (Hebrew Senior Life) We leverage the unique CASCADE dataset by linking it to Medicare and Medicaid claims

files in order to describe and examine factors associated with the expenditures on care for nursing home residents with advanced dementia.

2011-2014 Public Policy and the Demand for Long-Term Care Insurance NIA, R01 AG041109 Co-PI with David Stevenson (HMS)

By shedding light on the key factors that influence the demand for the private long-term care (LTC) insurance and the early experience of the LTC Partnership program, our analyses will offer key insights for policymakers as they implement and refine the government’s approach to the financing of long-term care.

2011-2015 Green House Evaluation: Impact on Costs and Quality Robert Wood Johnson Foundation PI Using linked administrative data for the period 2005-2010, this study examines the

2012-2015 Improving Nursing Home Compare for Dually Eligible Consumers AHRQ, R21 HS021866 Co-Investigator This study will examine whether and to what extent duals have benefited less from public

reporting and to assess several primary obstacles to effective use of public reporting by dually eligible individuals.

Co-Investigator Program Project grant studying the effects of Medicare policy changes on the use of and

outcomes Medicare’s post-acute care benefit and how those policies interact with long term care provider markets and states’ Medicaid policies.

2014-2020 Health Care Markets and Regulation Lab Laura and John Arnold Foundation Co-Investigator To help the U.S. healthcare system move forward by providing critical evidence, analyses and tools necessary to design health care regulations and markets to promote high quality care and innovation at a sustainable cost.

NIA, R01 AG047194 Principal Investigator To examine the implications of treating nursing home residents with dementia and severe mental illness in specialized environments. We will examine both the direct impact but also any potential spillovers to other residents in these nursing homes.

Impact of Enhanced Primary Care on Medicare Spending and Utilization for Long-Stay Nursing Home Residents Donaghue Foundation Principal Investigator The objective of this study is to understand the impact of an enhanced primary care model on outcomes for long-stay nursing home residents, including emergency department and acute care inpatient utilization. This study will provide the first large-scale evaluation of whether a Medicare Advantage plan with an increased clinical presence can improve outcomes, which may have a profound impact on the delivery of services to the nearly one-million long-stay nursing home residents in this country.

2015-2020 Medicare in a Restructured Delivery System NIA, P01 AG032952 Project Leader To study the integration of financing and care in the Medicare program. This Program

Which Post-Acute Care Setting is Best for Patients' Outcomes? NIA, R01 AG054656 Co-Principal Investigator We examine how various patient, provider and market factors influence access to rehabilitation services across different post-acute settings, and how care in these alternate post-acute care settings impacts outcomes for different patient groups. The goal of this research project is to examine the comparative effectiveness of different postacute care settings.

2017-2019 Using Telemedicine to Reduce Hospital Transfers

Co-Investigator Donaghue Foundation In independent living communities, a transfer to the emergency department is often the default response to a call for help. We are studying whether providing access to physicians via video (telemedicine) reduces unnecessary ED visits for residents of independent living communities.

2018-2019 Evaluating the Economic and Clinical Impact of the Commonwealth Care Alliance Model for Disadvantaged Populations Principal Investigator Commonwealth Fund Commonwealth Care Alliance (CCA) in Massachusetts is a leading organization that has insured, organized, and delivered care to a large and vulnerable population through an innovative model focused on intensive patient outreach, care coordination, and social determinants of health. We will conduct a rigorous scientific evaluation of the CCA model.
